Sweating in your sleep happens when something raises your core body temperature or disrupts the system your brain uses to regulate it. Sometimes the cause is as simple as a warm bedroom or heavy blankets. Other times, night sweats point to hormonal shifts, medications, sleep disorders, or less commonly, an underlying illness. Understanding the most likely triggers can help you figure out whether your night sweats are a nuisance or something worth investigating.

How Your Body Controls Temperature at Night

Your brain has a built-in thermostat located in the hypothalamus, a small region that sets a target for your core temperature and constantly adjusts to stay close to it. When your core temperature rises above that set point, your nervous system kicks off a cooling response: blood vessels near the skin dilate, and sweat glands across your body activate. This happens whether you’re awake or asleep.

What makes nighttime different is that your body naturally cools itself while you sleep. Core temperature peaks around 37.5°C (99.5°F) in the afternoon and drops to roughly 36.3°C (97.3°F) in the early morning hours. That downward shift is part of healthy sleep, but anything that interferes with it, or that pushes your temperature above the set point, can trigger sweating. A too-warm room, synthetic bedding that traps heat, or even a fever from a mild illness can be enough.

Your Sleep Environment May Be the Simplest Explanation

The ideal bedroom temperature for sleep is around 65°F (18.3°C), with a comfortable range between 60 and 68°F. If your room is warmer than that, or if your mattress and bedding retain heat, your body has to work harder to cool down, and sweating is the result. Memory foam mattresses, in particular, tend to trap body heat more than innerspring or latex options. Mattresses with higher heat capacity actually help draw warmth away from your body and promote deeper sleep.

Before investigating medical causes, it’s worth ruling out the basics: a cooler room, lighter blankets, and breathable bedding fabrics like cotton or linen resolve night sweats for many people.

Hormonal Changes, Especially Around Menopause

Fluctuating estrogen levels are one of the most common reasons for night sweats in women in their 40s and 50s. The mechanism is more nuanced than simply having “low estrogen.” What actually triggers hot flashes and night sweats is the withdrawal effect: when estrogen levels drop after a period of being high, the brain releases a surge of norepinephrine (a stress-related chemical messenger) that narrows the body’s thermoneutral zone. That’s the range of temperatures your body tolerates without triggering a heating or cooling response. When that zone shrinks, even a tiny increase in core temperature can set off a full-blown sweat response.

This is why night sweats tend to be worst during perimenopause, when estrogen levels swing dramatically, rather than years after menopause when levels are consistently low. The same mechanism can cause night sweats during other hormonal transitions, including pregnancy, the menstrual cycle, and certain thyroid conditions where hormone levels fluctuate.

Medications That Cause Night Sweats

If your night sweats started around the same time as a new prescription, the medication is a likely culprit. Antidepressants are among the most common offenders. Clinical trials show that 7% to 19% of people taking SSRIs (the most widely prescribed class of antidepressants, including sertraline, fluoxetine, paroxetine, and escitalopram) experience excessive sweating as a side effect. SNRIs like venlafaxine and duloxetine carry a similar risk. These drugs affect serotonin and norepinephrine levels, both of which play roles in temperature regulation.

Other medications commonly linked to night sweats include fever reducers like acetaminophen and ibuprofen (which can cause rebound sweating as they wear off), blood pressure medications, hormone therapies, and some diabetes drugs. If you suspect a medication is causing the problem, talk to your prescriber about alternatives or timing adjustments rather than stopping on your own.

Sleep Apnea and Night Sweats

About 30% of people with obstructive sleep apnea report night sweats, a connection many people don’t expect. When your airway repeatedly closes during sleep, your blood oxygen levels drop. These episodes of low oxygen trigger frequent partial awakenings that jolt your sympathetic nervous system, the same fight-or-flight system responsible for stress sweating. The more time you spend with oxygen saturation below 90%, the more likely you are to experience night sweats.

If your sweating comes with loud snoring, gasping during sleep, morning headaches, or daytime fatigue, sleep apnea is worth investigating. Treating the apnea, usually with a CPAP device, often resolves the sweating along with the other symptoms.

Alcohol and Diet

Drinking alcohol in the evening is a surprisingly common cause of sleep sweating. Alcohol widens blood vessels near the skin’s surface and increases heart rate, both of which raise skin temperature and trigger perspiration. As your body metabolizes alcohol overnight, the process itself generates heat. People who drink heavily may also experience mild withdrawal effects during sleep, which further activates the stress response and sweating.

Spicy foods, caffeine, and large meals close to bedtime can have similar effects on a smaller scale. Your body generates heat during digestion, and compounds like capsaicin (the chemical that makes peppers hot) directly stimulate the same receptors involved in temperature sensing.

Infections and Illness

Night sweats are a well-known symptom of several infections. Tuberculosis is the classic example, though it’s uncommon in many countries today. Other infections that can cause drenching night sweats include bacterial endocarditis (an infection of the heart valves), osteomyelitis (bone infections), and HIV. In these cases, the sweating is driven by your immune system’s fever response, which cycles through the night as your body fights the infection.

Even common illnesses like the flu or a mild viral infection can cause temporary night sweats. These usually resolve within a few days as you recover.

When Night Sweats Signal Something Serious

Most night sweats have a benign explanation. But certain patterns warrant prompt medical attention. In the context of lymphoma and other cancers, clinicians look for what are called “B symptoms”: drenching night sweats severe enough to require changing your bedclothes, unexplained fever above 100.4°F (38°C), and unintentional weight loss of more than 10% of your body weight over six months. The presence of all three together is a red flag, though even one persistent symptom deserves evaluation.

Swollen lymph nodes that last more than four to six weeks, especially combined with night sweats, are concerning enough to warrant a biopsy rather than a wait-and-see approach. Clinically significant unexplained weight loss, generally defined as more than 5% of your body weight over six to twelve months, is another signal that something beyond simple overheating is going on.

The key distinction is between occasional sweating on a warm night and persistent, drenching sweats that soak through your sheets repeatedly over weeks. The first is normal physiology. The second is your body telling you something has shifted, and figuring out what that is starts with a straightforward conversation and some basic blood work.
