Why Do My Ears Drain at Night? Causes Explained

Ears that drain at night are usually releasing fluid that has been building up during the day, and lying down is what finally lets it escape. The most common causes are middle ear infections, outer ear infections, and ruptured eardrums. The shift from upright to flat changes how fluid moves through your ear canal and Eustachian tubes, which is why you notice drainage on your pillow in the morning even if your ears seemed fine during the day.

Why Lying Down Makes It Worse

When you’re standing or sitting, gravity keeps fluid trapped in your middle ear or pooled in certain parts of your ear canal. Once you lie down, that changes. Your body position alters the pressure dynamics inside your ear: blood pressure in the veins around your ear increases, the tissue lining your middle ear swells slightly, and fluid that was sitting in one spot can shift and find its way out through the ear canal.

This is also why children with ear infections often have pain that gets worse at bedtime. The increased pressure from lying flat pushes against already-inflamed tissue. If there’s a hole in the eardrum, whether from an infection or ear tubes, that same pressure can push fluid outward onto your pillow overnight.

The Most Common Causes

Middle ear infections are the leading cause of ear drainage overall. Fluid and pus build up behind the eardrum, and if the pressure becomes great enough, the eardrum can rupture. You’ll typically notice a sudden, sharp pain that fades quickly, followed by thick yellow or bloody fluid leaking from the ear. The pain relief actually comes from the pressure being released. These ruptures usually heal on their own within a few weeks.

Outer ear infections (sometimes called swimmer’s ear) affect the ear canal itself rather than the space behind the eardrum. The canal becomes inflamed and may ooze fluid, especially when you’re lying on the affected side. The key difference: outer ear infections cause intense itching and pain when you tug on your earlobe, while middle ear infections produce deeper pain and sometimes fever, vomiting, or appetite changes. These two conditions require completely different treatments, so the distinction matters.

If you’ve had ear tubes placed, drainage is a known possibility. It occurs in up to 17% of ear tube cases. The tubes create a direct path from your middle ear to the outside, so any fluid that accumulates drains out through the tube rather than building up pressure behind the eardrum.

Earwax or Infection: How to Tell the Difference

Not every wet spot on your pillow means infection. Earwax can soften and become more liquid in warm environments, and body heat from your pillow can be enough to melt it. Softened earwax is typically light brown to dark orange, has a waxy texture even when runny, and doesn’t smell particularly strong.

Infectious drainage looks and smells different. Yellow, white, or green fluid suggests active infection, especially if it’s thick or opaque. Foul-smelling discharge points to something more serious, like a chronic infection or a cholesteatoma, which is an abnormal buildup of dead cells inside the middle ear. A cholesteatoma produces a watery, distinctly smelly discharge and can cause recurring ear infections, hearing loss, ringing in the ears, and dizziness if left untreated. In rare cases it can lead to a brain abscess or meningitis.

Bloody drainage usually signals a ruptured eardrum or, in children, a foreign object lodged in the ear canal. Clear, watery fluid is the trickiest to interpret. It could be something as benign as ear eczema, or, after a head injury, it could be cerebrospinal fluid leaking from the brain through a fracture at the base of the skull. That last scenario is a medical emergency. If you’ve recently hit your head and notice clear or blood-tinged fluid draining from your ear, get to an emergency room. Doctors can test the fluid for a specific protein called beta-2 transferrin to confirm whether it’s cerebrospinal fluid.

What the Color Tells You

  • Clear: Ear eczema, or after head trauma, a possible cerebrospinal fluid leak
  • Yellow or green: Middle ear infection, either acute (with pain) or chronic (sometimes painless)
  • Bloody or blood-tinged: Ruptured eardrum, foreign object, or head injury
  • Foul-smelling: Cholesteatoma or chronic middle ear infection
  • Light brown, waxy: Likely just softened earwax

Managing Drainage While You Sleep

If your ear is actively draining, sleep with the affected ear facing down on a clean towel placed over your pillow. Gravity will help the fluid exit rather than pool inside the canal, and the towel protects your bedding. Switch to a fresh towel each night to avoid reintroducing bacteria.

Elevating your head slightly with an extra pillow can reduce the pressure changes that come with lying flat, which may slow drainage and ease discomfort. Avoid plugging your ear with cotton balls or earplugs, as trapping moisture in a warm ear canal creates ideal conditions for bacterial or fungal growth. If you’re dealing with an outer ear infection, keeping the ear dry is especially important.

Signs That Need Prompt Attention

Thick, yellow, bloody, or foul-smelling discharge from the ear warrants medical evaluation, according to the American College of Emergency Physicians. The same goes for ear pain accompanied by fever, sudden hearing loss in one or both ears, or dizziness and vertigo alongside the drainage. These combinations can indicate infections that have spread beyond the ear canal or structural problems that won’t resolve on their own.

Drainage that keeps coming back over weeks or months, even without severe pain, also deserves investigation. Chronic suppurative otitis media can produce ongoing white, yellow, or green discharge through a persistent hole in the eardrum, sometimes with surprisingly little pain. Left unaddressed, it can cause progressive hearing damage. A cholesteatoma follows a similar pattern of persistent, smelly drainage and carries the risk of serious complications if it grows unchecked.
