Swollen feet happen when fluid leaks out of tiny blood vessels and collects in the surrounding tissue faster than your lymphatic system can drain it away. This fluid buildup, called edema, is extremely common and ranges from a harmless response to standing all day to an early sign of a heart, kidney, or vein problem. Whether one foot or both are swollen, and how quickly it came on, tells you a lot about what’s going on.
How Fluid Ends Up in Your Feet
Your capillaries constantly filter small amounts of fluid into the tissue around them. Normally, your lymphatic system reabsorbs most of that fluid and sends it back into your bloodstream. Swelling becomes visible when the rate of fluid leaking out exceeds the rate of fluid draining back in.
Several forces control this balance. Pressure inside your veins pushes fluid outward, while proteins in your blood pull fluid back in. Anything that raises venous pressure, lowers blood protein levels, or damages the lymphatic system tips the balance toward swelling. Gravity does the rest: fluid pools in whatever body part is lowest, which is why your feet and ankles are usually first.
One Foot vs. Both Feet
This distinction matters more than most people realize. If only one foot is swollen, the cause is almost always local: a problem in that specific leg rather than something systemic. About 40% of acute one-sided leg swelling turns out to be a muscle strain or soft tissue injury. Blood clots in a deep vein account for a significant share of the rest, along with infections, a fluid-filled cyst behind the knee, or blocked lymph drainage.
When both feet swell at the same time, the cause is more likely to involve your whole body. Heart failure, kidney disease, liver disease, hormonal changes, and medications all produce symmetrical swelling. Chronic venous insufficiency (weak vein valves that let blood pool in both legs) is another frequent culprit and can affect one or both sides.
Common Everyday Causes
Most foot swelling has a straightforward explanation. Sitting or standing in one position for hours, especially during long flights or desk-bound workdays, lets gravity pull fluid downward. Hot weather dilates blood vessels near the skin, which increases fluid filtration into surrounding tissue. Eating a very salty meal can expand your blood volume temporarily, though research suggests this effect is most dramatic when you shift from a very low-sodium diet to a high one rather than from day-to-day fluctuations in a typical diet.
Tight shoes, minor ankle sprains, and even a long walk on hard surfaces can cause localized puffiness that resolves on its own. If your swelling comes and goes with activity and disappears overnight, these benign causes are the most likely explanation.
Medications That Cause Swelling
Certain blood pressure medications are notorious for causing puffy feet and ankles. A class of drugs commonly prescribed for high blood pressure works by relaxing blood vessel walls, which lowers blood pressure but also lets more fluid seep into surrounding tissue. Between 1% and 15% of people taking standard doses develop ankle swelling, and at higher doses the rate can exceed 80%. Some hormone therapies, anti-inflammatory painkillers, diabetes medications, and steroids also promote fluid retention.
If your swelling started within weeks of beginning a new medication, that connection is worth exploring with whoever prescribed it. Adjusting the dose or adding a complementary medication can sometimes cut the swelling significantly. In one trial, combining a blood pressure medication with another type of drug reduced the rate of edema from nearly 19% to under 8%.
Venous Insufficiency and Vein Problems
Veins in your legs contain one-way valves that push blood upward against gravity. When those valves weaken or fail, blood pools in the lower legs and increases pressure inside the veins. This is chronic venous insufficiency, and it follows a predictable progression.
Early on, you might notice spider veins or small varicose veins with no swelling at all. As the condition advances, swelling appears around the inner ankle first and creeps upward with prolonged standing. One distinguishing feature: the swelling in venous insufficiency tends to spare the front of your foot, unlike swelling from lymph problems, which makes the whole foot puffy including the toes. Left untreated over years, the sustained pressure can darken the skin, harden the tissue beneath it, and eventually lead to open sores on the lower leg.
Heart, Kidney, and Liver Conditions
When the heart can’t pump blood forward efficiently, blood backs up in the veins. That backup raises pressure inside the capillaries, forcing fluid into surrounding tissue. The kidneys sense reduced blood flow and respond by holding onto salt and water, which makes the swelling worse. This cycle is why foot and ankle swelling is one of the most recognizable signs of heart failure.
Kidney disease causes swelling through a different route. Damaged kidneys can leak protein into the urine, lowering the concentration of protein in your blood. Since blood proteins act like sponges that hold fluid inside your vessels, losing them lets fluid escape more easily into tissue. Liver disease, particularly cirrhosis, reduces protein production and raises pressure in the veins that drain the abdomen, which can also drive fluid into the legs.
Swelling During Pregnancy
Some ankle swelling during pregnancy is completely normal, especially in the third trimester, as blood volume increases and the growing uterus puts pressure on pelvic veins. However, swelling that appears suddenly in the hands, arms, or face, or that comes with rapid unexpected weight gain, can signal preeclampsia, a serious pregnancy complication.
Preeclampsia is defined by blood pressure at or above 140/90 along with protein in the urine or signs of organ stress. Severe cases involve blood pressure of 160/110 or higher, severe headaches, vision changes, upper abdominal pain, or reduced urine output. Ankle swelling alone during pregnancy is not cause for alarm, but swelling above the ankles combined with any of those symptoms needs prompt evaluation.
How to Tell if Swelling Is Serious
A quick self-check: press a fingertip firmly into the swollen area for about five seconds, then release. If it leaves a visible dent that slowly fills back in, that’s pitting edema. Mild pitting (a shallow 2 mm dent that rebounds immediately) is common and often benign. Deeper dents of 5 mm or more that take 15 seconds to a minute to refill suggest more significant fluid accumulation. At the most severe end, an 8 mm pit that takes two to three minutes to rebound warrants medical attention.
Swelling in one leg that comes on suddenly, especially with calf pain, warmth, or a red or purple discoloration, raises concern for a deep vein blood clot. This is particularly true if you’ve recently been immobile for a long stretch, had surgery, or are on hormonal medications. A blood clot can break loose and travel to the lungs, causing sudden shortness of breath, chest pain that worsens with breathing, a rapid pulse, dizziness, or coughing up blood. Those pulmonary symptoms are a medical emergency.
Reducing Everyday Swelling
For swelling that’s positional or lifestyle-related, elevation is the simplest fix. Propping your feet above heart level for 20 to 30 minutes several times a day lets gravity work in your favor. Moving regularly throughout the day, even just flexing your ankles or taking a short walk, activates the calf muscles that pump blood back toward your heart.
Compression stockings apply graduated pressure that’s tightest at the ankle and loosens toward the knee, helping push fluid upward. Low-pressure stockings (under 20 mmHg) are available without a prescription and work well for mild, occasional swelling. Stockings rated at 20 mmHg or higher require a prescription and are typically used for chronic venous problems or more persistent edema. Reducing sodium intake, staying hydrated, and maintaining a healthy weight all help keep fluid balance in check over time.
If your swelling is new, persistent, worsening, or accompanied by shortness of breath, chest pain, or skin changes, those patterns point beyond lifestyle factors and deserve a clinical evaluation to identify the underlying cause.
