Raynaud’s Phenomenon: When Cold Hands Are Extreme

This triphasic color change happens because blood flow shuts down almost completely (white), the remaining blood loses its oxygen (blue), and then blood rushes back in when the vessels reopen (red). Episodes can be painful and last anywhere from a few minutes to over an hour.

There are two types. Primary Raynaud’s is the more common form. It most often affects women under 30 and isn’t linked to any other disease. It’s uncomfortable but generally harmless. Secondary Raynaud’s is tied to an underlying condition, most commonly autoimmune diseases like scleroderma, lupus, rheumatoid arthritis, or Sjögren syndrome. Secondary Raynaud’s tends to be more severe and can, in rare cases, lead to skin sores or tissue damage from prolonged blood flow blockage.

The key distinction matters because primary Raynaud’s is managed with lifestyle adjustments, while secondary Raynaud’s requires treatment of the underlying disease. If your episodes started after age 30, affect only one hand, or come with joint pain, skin changes, or sores on your fingertips, that pattern points toward the secondary form.

Low Thyroid Function and Metabolism

Your thyroid gland sets the pace for your metabolism, which directly determines how much heat your body generates. When thyroid hormone levels drop, as in hypothyroidism, your metabolic rate slows down. You produce less internal heat, and your body’s ability to regulate temperature suffers. Cold hands and feet are one of the earliest and most common symptoms.

Hypothyroidism affects roughly 5% of adults and is far more common in women, especially after age 60. Other signs include fatigue, weight gain, dry skin, and feeling cold all over, not just in your hands. If cold extremities are part of a broader pattern of sluggishness and cold sensitivity, a simple blood test can check your thyroid levels.

Iron and B12 Deficiency

Your hands get cold because your core loses heat, so keeping your torso warm with an insulating layer actually does more for your fingers than gloves alone. That said, gloves matter too. Mittens outperform fingered gloves because they let your fingers share warmth. Compression gloves can help by gently encouraging blood flow and retaining heat close to the skin.

Quitting smoking, if applicable, produces measurable improvements in peripheral circulation within weeks. Managing stress through whatever works for you, whether that’s exercise, breathing techniques, or reducing your commitments, can dial down the nervous system response that constricts your hand vessels. Eating a balanced diet that includes enough iron-rich foods (red meat, lentils, spinach) and B12 sources (meat, eggs, dairy, fortified cereals) addresses the nutritional angle.
