Why Do My Knees Hurt So Much? Causes and Relief

Knee pain has dozens of possible causes, but most cases trace back to a handful of common culprits: osteoarthritis, patellofemoral pain (often called runner’s knee), meniscus tears, tendinitis, or bursitis. Which one is behind your pain depends on where it hurts, when it started, and what makes it worse. The knee is the largest joint in your body and bears enormous force with every step, so even small problems can produce surprisingly intense pain.

The Force Your Knees Absorb Daily

Your knees handle far more weight than you might expect. When you walk on flat ground, each knee absorbs about one and a half times your body weight with every step. A 200-pound person puts roughly 300 pounds of pressure on their knees just by walking. Stairs, squatting, and running multiply that force even further. This constant loading explains why knees are so vulnerable to wear and injury, and why even a few extra pounds of body weight can noticeably increase pain.

Osteoarthritis: The Most Common Cause

Osteoarthritis is the single most common type of arthritis affecting the knee. It’s a wear-and-tear condition where the cartilage that cushions the joint gradually breaks down over time. Healthy cartilage constantly rebuilds itself, balancing the natural breakdown of its structure with new repair. In osteoarthritis, that balance tips. Enzymes that break down cartilage ramp up and overwhelm the body’s ability to repair the damage, creating a self-reinforcing cycle of degradation.

The breakdown starts at the surface layer of cartilage, where tiny cracks and rough spots appear. Over months and years, these progress deeper until large areas of cartilage erode completely, leaving bone closer to bone. The joint lining also becomes inflamed, which adds swelling and stiffness to the pain. This is why osteoarthritis tends to feel worse in the morning or after periods of inactivity, then loosens up somewhat with gentle movement before worsening again with prolonged activity.

Age, excess body weight, previous knee injuries, and repetitive stress from certain jobs or sports all raise the risk. If your knee pain is a deep ache that has gradually worsened over months or years, especially if you’re over 50, osteoarthritis is a leading possibility.

Runner’s Knee and Front-of-Knee Pain

Patellofemoral pain syndrome, commonly called runner’s knee, causes pain in or around the front of the knee, specifically behind or around the kneecap. Its defining feature is pain that gets worse when the knee is bent under load. Climbing stairs, squatting, lunging, and even sitting for long periods with bent knees can all trigger it. Descending stairs is often more painful than going up.

The most reliable sign during a physical exam is pain when squatting. Despite the nickname, you don’t need to be a runner to develop it. Anyone who regularly loads their knees through bending, whether from exercise, work, or daily activities, can end up with this condition. It’s diagnosed based on symptoms and a physical exam rather than imaging, since X-rays and MRIs often look normal.

Weakness in the quadriceps, the large muscles on the front of the thigh, plays a significant role. These four muscles share a common attachment to the kneecap and control how the kneecap tracks in its groove during movement. When they’re weak or imbalanced, the kneecap can shift slightly out of alignment, creating friction and pain. Strengthening the quadriceps is one of the most effective treatments.

Meniscus Tears

Each knee has two menisci, C-shaped pads of tough, rubbery cartilage that sit between the shinbone and thighbone and act as shock absorbers. A meniscus tear is one of the most common knee injuries, often caused by a sudden twist or pivot, though in older adults the cartilage can tear more easily from normal activity as it weakens with age.

The hallmark symptoms are a knee that “locks” or feels blocked when you try to move it, a sensation of instability (like the knee might give way), and swelling. Some people feel a pop at the moment of injury. Many meniscus tears, particularly those that don’t cause locking or catching, become less painful over time on their own. MRI is the best imaging tool for confirming a tear, since the meniscus is cartilage and won’t show up on X-rays.

Other Common Causes

Several other conditions frequently cause significant knee pain:

  • Tendinitis occurs when a tendon around the knee becomes irritated and inflamed, most often the patellar tendon just below the kneecap. It’s common in people who do a lot of jumping, running, or cycling.
  • Bursitis involves inflammation of the small fluid-filled sacs that cushion the outside of the knee joint. It often follows a direct blow to the knee or prolonged kneeling.
  • IT band syndrome happens when the thick band of tissue running from the hip to the outer knee becomes tight and rubs against the thighbone. It causes pain on the outside of the knee, especially during repetitive bending.
  • Gout and pseudogout cause sudden, intense knee pain when crystals build up inside the joint. Pseudogout actually affects the knee more than any other joint.
  • Referred pain from the hip or foot can cause knee pain indirectly. If you change the way you walk to avoid pain elsewhere, the altered movement pattern places extra stress on the knee.

Why Quad Strength Matters So Much

The quadriceps muscles are the primary stabilizers of the knee. They control every motion that involves bending or straightening the joint, and weakness in these muscles is linked to worse outcomes across nearly every type of knee problem. Research on patients recovering from knee ligament surgery has found that persistent quadriceps weakness leads to abnormal knee mechanics, slower rehabilitation, lower rates of returning to sport, and even faster progression of osteoarthritis over time.

A variety of exercises are needed to fully engage all four quadriceps muscles, since different parts of the muscle group activate more during different types of movement. Both open-chain exercises (like leg extensions, where the foot moves freely) and closed-chain exercises (like squats and leg presses, where the foot is planted) play a role. For general knee pain, strengthening sessions two to three times per week with 48 to 72 hours of recovery between sessions is a reasonable frequency for most people.

Managing a Sudden Knee Injury

If your knee pain started after a specific injury, the traditional approach of rest, ice, compression, and elevation still has value in the first few days, but sports medicine has moved toward a broader framework. The updated approach, sometimes called PEACE and LOVE, emphasizes protecting the injured joint early on while avoiding excessive rest. Gentle, pain-free movement in the days after an injury promotes better tissue healing than complete immobilization.

One notable shift: anti-inflammatory medications, while helpful for pain relief, may actually slow the body’s natural tissue repair process. Inflammation in the first few days after an injury is part of healing, not just a symptom to eliminate. Using anti-inflammatories sparingly rather than routinely is worth considering for soft tissue injuries. As the acute phase passes, gradually increasing activity and loading the joint helps stimulate recovery rather than hinder it.

Signs That Need Prompt Attention

Most knee pain improves with activity modification, strengthening, and time. But certain symptoms point to something more urgent. A hot, swollen knee with redness could indicate a joint infection, which requires emergency treatment. After a trauma, the inability to bear weight, significant swelling, or an inability to bend the knee past 90 degrees or straighten it fully all warrant prompt evaluation. A knee that locks completely, meaning you physically cannot move it through its range of motion, also needs medical assessment rather than a wait-and-see approach.

Knee pain that wakes you from sleep, pain accompanied by fever, or pain following recent surgery or a known infection are additional reasons to seek care quickly rather than managing things at home.
