When a dog over six years old has a seizure for the first time, the cause is almost always an underlying medical condition rather than epilepsy. Unlike younger dogs, who are often diagnosed with idiopathic (genetic) epilepsy, senior dogs who start seizing typically have a metabolic disorder, a structural brain lesion like a tumor, or a vascular event like a stroke. Understanding which category your dog falls into shapes everything that comes next.
Brain Tumors Are the Leading Structural Cause
Brain tumors are one of the most common reasons older dogs develop seizures. About 90% of primary brain tumors in dogs are meningiomas (roughly 50%), gliomas (roughly 35%), or choroid plexus tumors (about 7%). Most are diagnosed in dogs older than five, with meningiomas typically appearing around age 10.5 and gliomas around age 8.
Seizures are the single most common sign of a brain tumor in dogs. In one study of 97 dogs with confirmed brain tumors, 45% had seizures as their initial symptom. Among dogs older than eight with tumors in the front part of the brain, that number jumped to 65%. Veterinary neurologists generally recommend considering a brain tumor in any dog that has its first seizure after age four.
Certain breeds face higher risk. Brachycephalic (short-nosed) breeds like Boxers, Boston Terriers, Bullmastiffs, and English and French Bulldogs are significantly overrepresented in glioma diagnoses. If your senior dog belongs to one of these breeds, a tumor is especially worth investigating early.
Metabolic Problems That Trigger Seizures
Not all seizures start in the brain itself. “Reactive” seizures happen when something elsewhere in the body disrupts normal brain function. In a retrospective study of 877 dogs with seizure disorders, about 11% had a metabolic or toxic cause. The two most common were poisoning (39% of those cases) and low blood sugar, or hypoglycemia (32%).
For older dogs specifically, the metabolic triggers that matter most are tied to organ decline. Liver disease can cause hepatic encephalopathy, a condition where toxins the liver normally filters out build up in the bloodstream and reach the brain. Kidney failure produces a similar effect called uremic encephalopathy, where waste products accumulate to levels that interfere with brain signaling. Low calcium, low blood sugar from insulin-producing tumors, and an underactive thyroid gland can also provoke seizures in aging dogs.
The critical point here is that these seizures are a symptom, not the primary disease. Treating the underlying organ problem or correcting the metabolic imbalance often reduces or eliminates the seizures entirely, which is why thorough blood work matters before assuming a brain-centered cause.
Strokes and Vascular Events
Strokes happen in dogs more often than many owners realize, and they become more likely with age. When blood supply to part of the brain is suddenly cut off, the resulting damage can trigger seizures. Studies have documented seizures in up to 59% of dogs during the acute phase of an ischemic stroke.
The reassuring finding is that most of these seizures are isolated events. In a multicenter study of 125 dogs with confirmed ischemic strokes, 11 had seizures in the acute phase, but only one of those went on to develop ongoing epilepsy afterward. For comparison, the rate of post-stroke epilepsy in humans is about 8.7%. So while a stroke can absolutely cause a seizure in your older dog, it doesn’t necessarily mean seizures will become a recurring problem.
What Seizures Look Like in Older Dogs
Most people picture a seizure as a full-body convulsion where the dog falls on its side, paddles its legs, and loses consciousness. That’s a generalized seizure, and it does happen. But older dogs also commonly have focal seizures, which are easier to miss. A focal seizure only affects one part of the brain, so you might see twitching on one side of the face, repetitive chewing or lip-smacking, or one limb jerking while the rest of the body stays still.
One distinctive type is the “fly-biting” seizure, where a dog suddenly snaps at the air as if catching invisible flies. These episodes typically start without warning while the dog is resting, and the dog often stays aware of its surroundings. Some dogs can even be distracted out of the episode by their owner. Because these look more like quirky behavior than a medical emergency, they’re frequently overlooked, especially in older dogs whose owners may chalk it up to age-related oddness.
The Recovery Phase After a Seizure
After a seizure ends, dogs enter what’s called the postictal phase, a recovery period that can be alarming to watch. Your dog may seem disoriented, pace compulsively, stumble as if drunk, or appear temporarily blind. In a survey of dog owners, about half reported this phase lasted between 1 and 30 minutes, while another 20% said it lasted 30 to 60 minutes. Disorientation was the most commonly reported sign, followed by compulsive walking and temporary vision loss.
Owners consistently rate this recovery phase as one of the most distressing parts of their dog’s seizure disorder. Knowing it’s temporary and expected can help you stay calm, which in turn helps your dog. Keep the environment quiet, block access to stairs, and avoid putting your hands near your dog’s mouth during or immediately after a seizure.
The Connection Between Seizures and Cognitive Decline
There’s a meaningful link between seizures and cognitive dysfunction in dogs, the condition often called “dog dementia.” Dogs with epilepsy have a significantly increased risk of cognitive dysfunction compared to healthy dogs, and the risk scales with seizure severity. Dogs with a history of cluster seizures (multiple seizures in a short period) and higher seizure frequency score notably worse on cognitive assessments.
This appears to work in both directions. Repeated seizures may cause cumulative disruption to neural activity patterns, leading to permanent cognitive changes over time. The impairment looks different from typical age-related dementia, tending to show up earlier and progress differently. This is one reason veterinary neurologists emphasize getting seizures under control quickly in older dogs: reducing seizure frequency may help protect what cognitive function remains.
How Veterinarians Diagnose the Cause
When an older dog has its first seizure, the diagnostic workup typically starts with comprehensive blood work and a urinalysis. These tests screen for the metabolic causes: liver and kidney function, blood sugar, calcium levels, thyroid hormones. If those come back normal, the next step is usually advanced brain imaging with MRI or CT, sometimes combined with analysis of cerebrospinal fluid. This is where tumors, strokes, and inflammatory brain diseases become visible.
Advanced imaging isn’t always financially feasible, and that’s a real consideration. Your vet can often narrow down the likely cause based on the blood work, the type of seizure, how quickly symptoms appeared, and what other neurological signs are present. A dog that’s also circling to one side or has a head tilt points toward a structural brain problem. A dog with weight loss, increased thirst, and poor appetite alongside seizures may point toward organ disease.
Managing Seizures in Senior Dogs
Treatment depends entirely on the underlying cause. If a metabolic issue is driving the seizures, correcting that problem is the priority. If a brain tumor or another structural cause is identified, treatment may involve surgery, radiation, or anti-seizure medication to manage symptoms.
Anti-seizure medications require extra caution in older dogs because aging kidneys and livers process drugs less efficiently. Phenobarbital, one of the most commonly used seizure medications in dogs, carries a known risk of liver toxicity with long-term use, requiring blood level and liver function monitoring every 6 to 12 months. Other medications that are filtered through the kidneys need dose reductions if kidney function is already compromised. Your vet will factor in your dog’s overall organ health when choosing which medication to use and at what dose.
The goal with older dogs is often realistic rather than absolute: reducing seizure frequency and severity enough to maintain quality of life, while minimizing medication side effects that could compound existing age-related challenges.
