Why Do So Many Cats Get Kidney Disease?

Kidney disease is extraordinarily common in cats, affecting up to 40% of cats over age 10 and roughly 80% of cats over age 15. No other companion animal comes close to these numbers. The reasons are partly biological, partly dietary, and partly a consequence of how cats age, but the honest answer is that veterinary science still can’t pinpoint an exact cause in most individual cats.

What Makes Cats So Vulnerable

The most common form of feline kidney disease is chronic interstitial nephritis, a slow, progressive scarring of kidney tissue. Unlike acute kidney failure from a toxin or infection, this type creeps in over months or years. Scar tissue gradually replaces functional kidney cells, and those cells don’t regenerate. By the time symptoms appear, the damage is often substantial.

What makes cats uniquely prone isn’t fully understood, but several factors converge. Cats evolved as desert hunters who concentrate their urine heavily, which puts constant stress on kidney structures. Their kidneys work harder per unit of body weight than a dog’s kidneys do. Over a lifespan of 15 to 20 years, that cumulative workload takes a toll that shorter-lived or larger animals may never experience. In most cases, it’s simply not possible to identify the exact trigger in an individual cat.

The Role of Diet and Phosphorus

Phosphorus is one of the clearest dietary links to kidney disease progression in cats. Healthy kidneys filter excess phosphorus out of the blood, but as kidney function declines even slightly, phosphorus starts to build up. The body responds by pulling calcium from bones and ramping up parathyroid hormone production, a chain reaction that further damages the kidneys and can cause calcium deposits in soft tissues.

This creates a vicious cycle: mild kidney damage leads to phosphorus retention, which accelerates further kidney damage. Commercial cat foods, especially those high in meat byproducts, tend to be phosphorus-rich. For cats already losing kidney function, that dietary load can push the disease forward faster. This is why phosphorus-restricted diets are one of the most effective interventions once kidney disease is diagnosed.

Chronic Inflammation and Dental Disease

Dental disease is nearly universal in aging cats, and it may be doing more damage than most owners realize. Research has found that cats with moderate to severe periodontal disease have about 1.5 times the risk of developing kidney disease compared to cats with healthy gums. The working theory is that chronic oral bacteria and inflammation create a low-grade inflammatory state throughout the body, reducing blood flow to the kidneys and promoting the kind of scarring that defines chronic kidney disease.

The timing matters here: in studies, the dental disease typically appeared before the kidney diagnosis, which suggests the relationship may be more than coincidence. Bacterial kidney infections (pyelonephritis) from other sources can also contribute. Bacteria from the urinary tract can climb into the kidneys, especially when kidney or ureter stones block normal urine flow.

How Hyperthyroidism Hides the Problem

Hyperthyroidism, another extremely common condition in older cats, can mask kidney disease for months or years. An overactive thyroid speeds up metabolism and increases blood flow through the kidneys, making them appear to filter more efficiently than they actually do. Hyperthyroid cats also retain extra fluid and lose muscle mass, both of which artificially lower the blood markers vets use to assess kidney function.

This is why many cats are diagnosed with kidney disease shortly after starting treatment for hyperthyroidism. The thyroid medication didn’t cause the kidney problem. It simply removed the hormonal boost that was hiding it. Vets often monitor kidney values closely after beginning thyroid treatment for exactly this reason.

Why It’s Caught Late

Cats are notoriously good at hiding illness, and kidney disease produces subtle early signs: slightly increased thirst, a bit more time at the water bowl, mildly increased urination. Most owners don’t notice these changes until the disease is well advanced.

Traditional blood tests compound this problem. The standard kidney marker, creatinine, doesn’t rise above normal until a cat has already lost 50% to 75% of its functional kidney tissue. A newer blood test called SDMA can detect kidney damage earlier, flagging problems at around 40% loss of function, an average of 17 months before creatinine would catch it. This earlier detection window is why many vets now include SDMA in routine senior bloodwork.

How Staging Predicts Outcomes

Once diagnosed, kidney disease is classified into stages based on blood values and other markers. The stage at diagnosis has a dramatic effect on how long a cat can live comfortably with the disease. Cats diagnosed at an early-to-moderate stage (stage 2) have a median survival of about 1,151 days, roughly three years. Those caught at stage 3 survive a median of 778 days, just over two years. Cats not diagnosed until stage 4, when the kidneys are severely compromised, have a median survival of only 103 days.

These numbers underscore why early detection matters so much. A cat diagnosed at stage 2 through routine bloodwork has years of quality life ahead with dietary management and monitoring. The same cat, left unscreened until stage 4 symptoms become obvious, faces a very different timeline.

What Actually Helps

Managing feline kidney disease centers on slowing down the cycle of damage. Phosphorus-restricted, kidney-supportive diets are the single most impactful intervention and can significantly extend survival. Maintaining hydration is critical, since cats with kidney disease lose more water through dilute urine than they can easily replace by drinking. Many owners learn to give subcutaneous fluids at home, a simple process that most cats tolerate well.

Keeping up with dental care throughout a cat’s life may reduce the inflammatory burden on the kidneys. Regular bloodwork starting around age 7 gives vets the best chance of catching the disease early, when interventions make the biggest difference. Because kidney cells don’t regenerate, every month of function preserved through early management is time that can’t be recovered once lost.
