Why Do We Get Blackheads and How to Prevent Them

Blackheads form when a pore fills with a mix of oil and dead skin cells, and the top of that plug sits exposed to air. The exposure triggers a chemical reaction that turns the plug dark, giving blackheads their name. They’re one of the most common skin concerns at any age, affecting roughly half of women in their 20s and remaining common well into the 40s.

What Happens Inside a Pore

Every pore on your skin contains a tiny oil gland. These glands produce sebum, a waxy substance that keeps your skin moisturized and protected. Under normal conditions, sebum travels up through the pore and spreads across the surface of your skin without you noticing.

Problems start when dead skin cells don’t shed the way they should. Instead of sloughing off, they stick together inside the pore and mix with sebum, forming a plug. If that plug stays beneath a thin layer of skin, it remains flesh-colored and becomes a whitehead (a closed comedone). But if the pore stays open, air reaches the plug. Oxygen reacts with the material inside, and melanin granules in the trapped skin cells darken. That’s why blackheads look dark brown or black. It’s not dirt. It’s oxidation, the same chemical process that turns a sliced apple brown.

Why Your Oil Glands Go Into Overdrive

The single biggest driver of blackheads is hormones, specifically androgens like testosterone. Your oil glands have receptors that respond directly to androgens by ramping up both cell growth and oil production. The glands on your face and scalp are especially sensitive because they produce high levels of an enzyme that converts testosterone into its most potent form.

This explains why blackheads tend to cluster on the nose, forehead, and chin. It also explains the timing. Oil gland activity surges during puberty, when androgen levels spike, and stays elevated through the late teens. But hormonal fluctuations don’t stop there. Menstrual cycles, pregnancy, polycystic ovary syndrome, and even stress-related cortisol shifts can all push oil production higher at any age. Clinical data shows that about 50% of women in their 20s, 33% in their 30s, and 25% in their 40s still deal with acne. Among men in their 40s, about 12% report ongoing breakouts.

How Diet Plays a Role

What you eat can influence how much oil your skin produces. Foods that cause a rapid spike in blood sugar, like white bread, sugary drinks, and processed snacks, trigger a surge of insulin. High insulin levels do two things that promote blackheads: they speed up the turnover of skin cells lining your pores (creating more material to form a plug), and they stimulate your body to produce more androgens, which in turn increases sebum output.

This doesn’t mean a single cookie causes a breakout. The effect is cumulative. A diet consistently high in refined carbohydrates keeps insulin elevated, which keeps oil glands working harder than they need to. Reducing those blood sugar spikes by swapping in whole grains, vegetables, and proteins with a lower glycemic index can lower both insulin and the androgen signals that drive oil production.

Skincare Products That Clog Pores

Some of the products you use to improve your skin may actually be feeding the problem. Ingredients rated as highly comedogenic (meaning they’re proven to block pores) include isopropyl myristate, wheat germ oil, algae extract, coconut oil, cocoa butter, and isopropyl palmitate. These substances are common in moisturizers, foundations, and hair products that drip onto your forehead and jawline.

Coconut oil is a frequent offender because it’s widely marketed as a natural moisturizer, but it scores a 4 out of 5 on comedogenic rating scales. If you’re prone to blackheads, look for products labeled “non-comedogenic” and check ingredient lists for the names above. Switching to a lighter, oil-free moisturizer is one of the simplest changes you can make.

Other Common Triggers

Friction and pressure on the skin can push dead cells and oil deeper into pores. Tight headbands, phone screens pressed against your cheek, and chin straps are classic culprits. Humid environments increase sweating and oil flow, which is why blackheads often worsen in summer. Overwashing your face can also backfire. Stripping your skin of oil signals your glands to compensate by producing even more sebum, creating a cycle that makes blackheads more persistent.

Clearing and Preventing Blackheads

Because blackheads are open to the surface, topical treatments can reach the plug directly. Salicylic acid is the most widely recommended ingredient for blackheads because it’s oil-soluble, meaning it can dissolve into the sebum inside a pore and break apart the plug from within. Products containing 2% to 4% salicylic acid are effective for most people. Start at the lower end if your skin is sensitive, and use it consistently for several weeks before judging results.

Retinoids work differently. They speed up the rate at which skin cells turn over, preventing dead cells from accumulating and forming plugs in the first place. Over-the-counter options containing adapalene are available without a prescription and are a good starting point. Prescription-strength retinoids like tretinoin are stronger but can cause dryness and irritation, especially in the first few weeks.

Manual extraction (squeezing blackheads yourself) is tempting but risky. Pressing too hard can damage the pore wall, push debris deeper, and cause inflammation or scarring. If you have stubborn blackheads that don’t respond to topical products, a dermatologist or licensed esthetician can extract them with sterile tools and controlled pressure.

For long-term prevention, the most effective routine combines a gentle cleanser twice daily, a salicylic acid or retinoid product in the evening, a non-comedogenic moisturizer, and sunscreen during the day (retinoids make skin more sun-sensitive). Consistency matters more than intensity. A simple routine you follow every day will outperform an aggressive one you abandon after a week.
