Why Do You Cough More at Night? Causes Explained

Coughing gets worse at night primarily because lying down changes how gravity acts on your body, allowing mucus to pool in your throat, stomach acid to creep upward, and fluid to shift into your lungs. On top of that, your airways naturally narrow during the nighttime hours due to your body’s internal clock, making you more sensitive to irritants exactly when you’re trying to sleep.

Gravity Works Against You When You Lie Down

During the day, gravity helps mucus drain downward through your nose and throat without you noticing. When you lie flat, that drainage doesn’t stop. Instead, mucus from your sinuses trickles backward into your throat, a process called post-nasal drip. This pooling mucus hits cough receptors concentrated in your pharynx and larynx, triggering the reflex. If you have a cold, allergies, or a sinus infection, you’re producing extra mucus, and lying down turns a manageable daytime drip into a persistent nighttime cough.

Your cough reflex exists to protect your airways from irritants, mucus, and foreign substances. The receptors responsible for triggering it respond to mechanical, inflammatory, and chemical stimuli, all connected to nerve pathways running through the vagus nerve. When mucus sits on those receptors rather than flowing past them, the signal to cough fires repeatedly.

Your Airways Naturally Narrow at Night

Your body’s circadian rhythm, the internal clock that regulates sleep and wakefulness, also controls how open your airways are. Lung function dips to its lowest point around 4:00 AM. Research published in PNAS found that airway resistance increases by about 20% during the biological night compared to daytime, even when sleep itself is taken out of the equation. For people with asthma, this means nighttime brings a measurable drop in airflow that can trigger coughing, wheezing, and shortness of breath.

Sleep itself adds another layer. The same study found that the act of sleeping independently worsens lung function on top of the circadian effect. The worst breathing measurements occurred at the end of the first third of the sleep period. People with asthma in the study were four times more likely to need a rescue inhaler during the circadian night than during the day. Even people without asthma experience some degree of airway narrowing at night, though it’s rarely enough to cause symptoms on its own.

Acid Reflux Peaks When You’re Horizontal

Stomach acid is another major nighttime cough trigger. When you’re upright, gravity keeps acid in your stomach. Lying flat removes that advantage, allowing acid to travel up the esophagus and sometimes reach the throat or even the airways. This can cause coughing through three routes: tiny amounts of acid reaching the airways (microaspiration), acid irritating the throat and voice box directly, or acid in the esophagus triggering a nerve reflex that signals the lungs to cough, even without acid physically reaching them.

Up to 48% of people with chronic cough show a statistically significant link between reflux events and cough episodes, with reflux preceding the cough more often than chance would explain. Many people with reflux-related cough don’t experience obvious heartburn. Instead, they have a dry, persistent cough that worsens after lying down or eating late. Researchers believe these individuals have heightened sensitivity in both the esophagus and the airways, meaning even small amounts of weakly acidic reflux can set off the cough reflex.

Your Bedroom May Be Full of Triggers

Dust mites thrive in warm, humid environments, and your bed is their ideal habitat. They colonize mattresses, pillows, and blankets, and their waste particles become airborne when you shift in your sleep. For people with dust mite allergies, this means allergen exposure spikes during the exact hours you’re in bed. Symptoms are worst while sleeping or cleaning, precisely when allergens are most likely to be disturbed and inhaled. The resulting inflammation in your airways leads to coughing, congestion, and sometimes wheezing.

Bedroom air quality matters too. Indoor humidity below 40% dries out your throat and nasal passages, making them more reactive to irritants. Humidity above 60% encourages mold growth and dust mite proliferation. The optimal range for minimizing respiratory irritation is 40 to 60%. A simple hygrometer can tell you where your bedroom falls, and a humidifier or dehumidifier can bring you into range.

Heart Failure Can Cause Nighttime Cough

A less common but more serious cause of nighttime coughing is heart failure. When the heart can’t pump efficiently, fluid accumulates in the lower legs during the day. At night, lying down redistributes that fluid from the legs and abdomen into the chest, a phenomenon called rostral fluid shift. This can cause temporary fluid buildup in the lungs, leading to coughing, shortness of breath, and the need to prop yourself up on pillows to breathe comfortably. If you notice ankle swelling during the day, unexplained weight gain, and a cough that only appears or worsens when you lie flat, these signs together warrant a conversation with a doctor.

How to Reduce Nighttime Coughing

Elevating your head and upper body is one of the most effective changes you can make. Clinical practice uses a 30 to 60 degree elevation to keep airways open and reduce both post-nasal drip and acid reflux. You don’t need a hospital bed to achieve this. A wedge pillow or raising the head of your bed by 6 to 8 inches with blocks can approximate a 30-degree angle. Simply stacking regular pillows tends to bend you at the waist rather than elevating your torso, which can actually worsen reflux.

For dust mite allergies, encasing your mattress and pillows in allergen-proof covers makes a noticeable difference. Washing bedding weekly in hot water (at least 130°F) kills mites. If dry air is irritating your throat, a cool-mist humidifier set to keep the room between 40 and 60% humidity can calm the cough reflex overnight.

If you’re dealing with a cough from a cold or upper respiratory infection, honey is worth trying. A study comparing honey to two common over-the-counter cough suppressants in children found that a 2.5 mL dose of honey before bed reduced cough frequency significantly more than either medication. Cough frequency scores dropped from about 4.1 to 1.9 with honey, compared to 4.1 to 3.1 with no treatment. This applies to children over one year old and adults. Honey should never be given to infants under 12 months.

For reflux-related coughing, avoid eating within two to three hours of bedtime and keep your head elevated. If you suspect acid reflux is driving your cough, particularly a dry cough without heartburn, an antacid trial can help clarify the connection.

When Nighttime Cough Signals Something More

A nighttime cough from a cold or mild irritation typically resolves within a couple of weeks. If yours persists beyond that, it may point to an underlying condition like asthma, reflux, or chronic sinusitis that needs targeted treatment. Coughing up thick green or yellow phlegm, blood-tinged mucus, or experiencing wheezing, chest pain, shortness of breath, or ankle swelling alongside the cough are signs that something beyond a simple irritation is going on.
