Why Do You Get a Sore Throat When Sick?

A sore throat during illness is your immune system’s inflammatory response to infection, not the infection itself doing most of the damage. When a virus or bacterium invades the tissue lining your throat, your body releases chemical signals that trigger pain, swelling, and increased blood flow to the area. These chemicals are what make swallowing feel like pushing past sandpaper, and understanding how they work explains why your throat can hurt so intensely from something as routine as a cold.

How Infection Triggers Throat Pain

When a pathogen lands in your throat, your immune system detects it and launches a local inflammatory response. Immune cells flood the area and release a mix of chemical messengers, but two dominate the pain picture: bradykinin and prostaglandins. Bradykinin is a potent stimulator of pain nerve fibers. It also widens blood vessels and makes capillary walls more permeable, which is why your throat swells up and turns red. Prostaglandins, particularly one called prostaglandin E2, amplify the pain signal and sustain the inflammation over hours and days.

These chemicals don’t just activate pain nerves directly. They also lower the threshold at which those nerves fire. Normally, the pain receptors in your throat’s lining require a strong stimulus to send a signal to your brain. During infection, bradykinin and prostaglandins sensitize those receptors so thoroughly that normal body temperature, the pressure of swallowing, or even breathing can trigger them. This is why your throat can hurt constantly, not just when you swallow. The nerve endings are essentially stuck on high alert, reacting to stimuli they’d normally ignore.

Interestingly, the chemical byproducts of this process closely resemble capsaicin, the compound that makes chili peppers burn. Bradykinin activates pathways in nerve endings that produce metabolites with a similar three-dimensional shape to capsaicin. Researchers have proposed that these metabolites act as the body’s own version of capsaicin, stimulating the same pain receptors. That burning sensation in your throat during a cold is, in a very real sense, your body creating its own internal pepper spray to signal that something is wrong.

Viruses vs. Bacteria: Different Routes to the Same Pain

Most sore throats are caused by viruses, including rhinoviruses (the common cold), influenza, and others. Viral infections typically provoke inflammation across a broad area of the throat, which is why you feel a general soreness rather than pain in one specific spot. The virus infects cells in the throat lining, your immune system responds with inflammation, and the resulting chemical cascade produces pain through the mechanisms described above.

Bacterial infections, most notably group A strep, work differently. Strep bacteria attach to specific sites in the throat using surface proteins called adhesins, then colonize and compete with your normal bacterial residents. Some strains produce toxins that act as “superantigens,” essentially overstimulating your immune system’s T cells and prompting a massive release of inflammatory signals. This tends to produce more intense, localized pain, often concentrated around the tonsils. The M protein on strep bacteria’s surface is particularly important for invasive infection and can provoke a strong immune reaction. Some strains also produce toxins responsible for the rash of scarlet fever.

The practical difference: viral sore throats tend to come with other cold symptoms like a runny nose, coughing, and sneezing. Strep throat more often hits suddenly with severe throat pain, fever, and swollen lymph nodes in the neck, but without the typical cold symptoms.

Why It Hurts More in the Morning

If your sore throat feels worst when you first wake up, two overnight factors are working against you. The first is mouth breathing. When your nose is congested, you breathe through your mouth while you sleep. Nasal breathing keeps moisture circulating through your mouth and throat, but mouth breathing dries everything out. Your saliva either evaporates or ends up on your pillow as drool, leaving your throat without its natural lubricant for hours.

The second factor is dehydration. Eight to ten hours of sleep is a long time without any fluids. If you went to bed already somewhat dehydrated (common when you’re sick and not eating or drinking as much), your body produces less saliva overnight. Less saliva means your mucous membranes can’t maintain their protective moisture layer, and the already-inflamed tissue in your throat dries out and becomes even more irritated. Drinking water before bed and keeping a glass on your nightstand can take the edge off morning throat pain.

Post-Nasal Drip Adds to the Irritation

Inflammation from infection doesn’t stay neatly contained in your throat. When you’re sick, your nasal passages produce excess mucus, and much of it drains down the back of your throat. This post-nasal drip acts as a constant irritant, keeping the already-sensitized tissue in your throat inflamed. Your tonsils and surrounding tissue can swell in response, compounding the discomfort. This is why a sore throat often lingers even after the worst of a cold has passed: the drip continues as long as your sinuses are still clearing out.

How Long It Typically Lasts

Most sore throats from viral infections resolve within three to ten days. The pain usually peaks in the first two or three days, then gradually eases as the inflammatory response winds down. Viral infections generally clear on their own within a week without any specific treatment. Bacterial infections like strep throat respond to antibiotics, and throat pain typically starts improving within a day or two of starting treatment.

A sore throat that persists beyond ten days, or one that keeps getting worse after the first few days instead of improving, is worth getting checked out. The same goes for a sore throat with a high fever, severe difficulty swallowing, or neck swelling.

Signs That Need Prompt Attention

The vast majority of sore throats are uncomfortable but harmless. A few warning signs, however, can point to complications like a peritonsillar abscess (a pocket of infection near the tonsils) or other serious conditions:

  • Muffled or “hot potato” voice: your voice sounds like you’re talking with something in your mouth
  • Trismus: difficulty opening your mouth fully
  • Drooling or inability to swallow saliva
  • Noisy breathing or stridor (a high-pitched sound when inhaling)
  • Stiff neck or neck swelling
  • Appearing very unwell with high fever and difficulty breathing

These signs suggest the infection may have spread beyond the throat lining into deeper tissues, and they warrant urgent medical evaluation. A peritonsillar abscess, for instance, produces fever, severe pain on one side of the throat, swelling that pushes the uvula to one side, and difficulty opening the jaw. It’s treatable, but it won’t resolve on its own.
