It comes down to the physical properties of the drug, how it interacts with your tissue, and the specifics of the injection itself.

What Happens Under Your Skin

Emgality is a monoclonal antibody, which means it’s a large, complex protein molecule dissolved in liquid. When that fluid gets pushed into the subcutaneous layer (the fatty tissue between your skin and muscle), it has to force its way into a space that’s already occupied. The tissue there is made up of collagen, hyaluronic acid, and fluid, all packed together in a structured matrix. Injecting more fluid creates pressure that stretches and distorts the tissue, activating nearby nerve endings.

Research on subcutaneous injections shows that the back pressure created during delivery rises with faster flow rates. Autoinjectors, which Emgality uses, tend to deliver fluid quickly compared to a slow manual push with a syringe. That rapid delivery means more sudden tissue stretching and a sharper sensation. The viscosity of protein-based drugs also increases the pressure needed to push the fluid through the needle, adding to the force your tissue absorbs.

The Loading Dose Makes It Worse

Your very first Emgality dose for migraine prevention is a loading dose of 240 mg, which requires two back-to-back injections of 120 mg each. That means two separate needle sticks and two rounds of tissue distension in one sitting. For many people, this first experience sets a painful impression of the medication.

After that initial visit, the standard monthly dose drops to a single 120 mg injection. Many people notice the pain becomes more manageable once they’re past the loading dose, partly because there’s only one injection and partly because they’ve learned what to expect and can prepare accordingly.

How Emgality Compares to Similar Medications

Emgality is not the only injectable migraine preventive in its class, and it does appear to cause more injection site discomfort than some alternatives. What the Pain Actually Feels Like

Most people describe the injection itself as a sharp sting or burning sensation that lasts several seconds while the fluid is being delivered. After the needle is out, the area often feels sore, warm, or tender for minutes to hours. Some people develop redness at the site (about 3 to 4% in trials) or itching (2 to 3%). Swelling is less common, occurring in roughly 1% of people on the standard dose. About 86% of those who reported pain said it happened within the first 60 minutes after injection.

It’s worth noting that in clinical trials, even participants receiving a placebo injection (saline with no active drug) reported injection site pain at a rate of 9.5%, compared to 10 to 12% for Emgality. This tells you that a meaningful portion of the discomfort comes from the needle and the act of injecting fluid itself, not just the medication. The drug does add some extra irritation, but the physical process of subcutaneous injection is inherently uncomfortable for many people.

Reducing the Pain

The single most effective thing you can do is let the pen sit at room temperature for 20 to 30 minutes before injecting. This is simple, costs nothing, and makes a real difference in how much the fluid stings going in. Never microwave or heat the medication artificially.

Applying an ice pack or cold compress to the injection site for about 15 minutes beforehand numbs the skin and dulls the initial needle prick.
