Hypothyroidism causes weight gain primarily by slowing your metabolism, reducing your body’s ability to burn calories at rest and generate heat. But the number on the scale is often less dramatic than people expect. The American Thyroid Association estimates that most people with hypothyroidism gain about 5 to 10 pounds from the condition itself, depending on severity. The rest of the story involves water retention, changes in fat processing, and shifts in how your body handles energy at the cellular level.
Your Metabolism Runs on Thyroid Hormone
Thyroid hormone acts like a thermostat for your cells. It regulates your basal metabolic rate, which is the energy your body burns just to keep itself alive: breathing, circulating blood, maintaining body temperature. When thyroid hormone levels drop, that thermostat gets turned down, and every cell in your body becomes less metabolically active.
At the cellular level, thyroid hormone drives mitochondria, the structures inside cells that convert food into usable energy. It stimulates how quickly your cells break down glucose and fatty acids, controls the activity of ion pumps that move sodium and potassium across cell membranes, and influences how your muscles use fuel. In a hypothyroid state, all of these processes slow down. Your cells extract less energy from food, burn less fat, and produce less heat. The result is that you store more of what you eat and burn less throughout the day, even if your diet hasn’t changed.
Much of the Weight Is Water, Not Fat
A significant portion of hypothyroid weight gain comes from fluid accumulation rather than new fat tissue. In hypothyroidism, certain sugar-protein molecules called mucopolysaccharides build up in the skin and connective tissues. These molecules are hygroscopic, meaning they attract and hold water. The result is a specific type of swelling called myxedema: a boggy, non-pitting edema that makes your face, hands, and legs look puffy.
Thyroid hormone also directly controls the sodium-potassium pumps in your cell membranes. These pumps regulate how much fluid sits inside versus outside your cells. When thyroid levels are low, this balance shifts, and more sodium and water accumulate in your tissues. This fluid retention can account for several pounds on its own and is one reason people sometimes notice relatively quick weight loss once they start treatment.
Your Body Loses Its Ability to Generate Heat
One of thyroid hormone’s most important jobs is activating brown fat, a special type of fat tissue that exists specifically to burn calories and produce heat. Brown fat is packed with mitochondria and contains a protein called UCP1 that essentially short-circuits normal energy production, converting calories directly into warmth instead of storing them.
Thyroid hormone is essential for brown fat to develop, differentiate, and function properly. In hypothyroid animals, cold exposure causes severe, sometimes fatal hypothermia because brown fat thermogenesis is so impaired. In humans, the effect is less extreme but still meaningful. Your body becomes less efficient at burning calories for warmth, which is why feeling cold is one of the hallmark symptoms of hypothyroidism. That reduced heat production represents real calories that would otherwise have been burned but instead get stored.
Fat Processing Slows Down
Hypothyroidism changes how your body handles fat at the enzyme level. One key enzyme, lipoprotein lipase, is responsible for clearing triglycerides (a type of fat) from your bloodstream and moving them into tissues. In hypothyroid patients, the activity of this enzyme in fat tissue drops to roughly half its normal level. Research published in the Journal of Clinical Endocrinology & Metabolism found that hypothyroid patients had lipoprotein lipase activity of about 1.5 units per million cells compared to 3.3 in healthy controls.
This creates a paradox: your body becomes worse at processing and clearing fats from the blood (which is why hypothyroidism often causes high triglycerides), while simultaneously becoming less efficient at breaking down stored fat for energy. Thyroid hormone normally promotes fat mobilization, increasing the concentration of fatty acids in the blood so they can be burned. Without adequate thyroid hormone, stored fat stays put and circulating fats get cleared more slowly. After treatment, lipoprotein lipase activity increased by an average of 163%, returning to normal levels.
Appetite Signals May Shift
The relationship between hypothyroidism and hunger is more complicated than a simple increase in appetite. Ghrelin, the hormone that triggers hunger, tends to run slightly higher in people with hypothyroidism compared to healthy controls, though the difference isn’t always statistically significant. Researchers believe elevated ghrelin in hypothyroidism may be a compensatory response to the metabolic slowdown and changes in food intake that come with low thyroid levels.
Thyroid hormone also affects how your body handles blood sugar. It promotes glucose absorption in the intestines, influences insulin secretion, and affects insulin sensitivity. When thyroid levels drop, these glucose pathways become less efficient, which can contribute to fatigue and cravings as your body struggles to maintain steady energy levels. The weight gain from hypothyroidism, in other words, isn’t simply about eating more. It’s about your body being less able to use the food you do eat.
Subclinical Hypothyroidism Is Different
If your thyroid levels are only mildly off, sometimes called subclinical hypothyroidism (where TSH is elevated but thyroid hormones are still in the normal range), the weight impact is likely minimal. A study from the Cardiovascular Health Study that followed elderly participants found no clinically significant difference in weight change, lean mass, fat mass, or body fat percentage between people with subclinical hypothyroidism and those with normal thyroid function. This held true whether the subclinical state was measured once or persisted over two years. Substantial weight gain is more closely linked to overt hypothyroidism, where thyroid hormone levels are clearly below normal.
What Happens After Treatment
Starting thyroid hormone replacement doesn’t produce dramatic weight loss for most people. About 52% of patients lose weight within the first 24 months of treatment, with an average loss of roughly 8 pounds. That aligns with the American Thyroid Association’s estimate that only 5 to 10 pounds of total weight gain is directly attributable to the thyroid condition itself.
The weight that comes off first tends to be the excess water and salt that accumulated in tissues. Fat loss is slower because treatment restores your metabolic rate to normal but doesn’t push it above normal. If you gained additional weight during the hypothyroid period due to fatigue-related inactivity or changes in eating habits, that weight won’t automatically reverse with medication. Your metabolism returns to its baseline, but it doesn’t compensate for the surplus that built up. This is why many people feel disappointed by the scale after starting treatment, even when their lab values have normalized. The thyroid was responsible for a real but limited portion of the total gain.
