Why Does It Feel Like Something Is Crawling on Me?

That creeping, crawling feeling on your skin, even when nothing is there, is a real sensation with a medical name: formication. It comes from the Latin word for “ant,” and it describes exactly what most people report: the feeling of tiny insects moving across or just under the skin. Your brain is processing touch signals that don’t actually exist, creating a convincing but false alarm. This can happen for a surprisingly wide range of reasons, from hormone shifts to vitamin deficiencies to anxiety.

What’s Happening in Your Nervous System

Your brain has dedicated areas that work together to process signals from your sense of touch. Normally, nerve endings in your skin detect pressure, temperature, or movement and send those signals up to the brain for interpretation. In formication, those brain areas behave as though they’re receiving signals from the body even when no such signals exist. The result is a tactile hallucination: your skin feels something that isn’t there.

This can also happen in reverse. Sometimes the nerve endings themselves misfire, sending garbled signals to a brain that interprets them the only way it knows how. Damaged or irritated peripheral nerves, the ones running through your hands, feet, arms, and legs, are especially prone to generating these phantom sensations. The crawling feeling is essentially your nervous system’s version of static on a bad phone line.

Hormonal Changes, Especially Menopause

Declining estrogen is one of the most common triggers for skin-crawling sensations in women over 40. Estrogen plays a dual role here: it supports the skin’s outer barrier, collagen production, and moisture retention, and it also helps stabilize the nervous system. As estrogen levels fluctuate and drop during perimenopause and menopause, skin becomes thinner, drier, and more sensitive. At the same time, the way nerves send and process sensory signals can change, contributing to abnormal sensations like pins-and-needles or the feeling that something is moving across your skin.

There isn’t strong data on exactly how many menopausal women experience formication specifically, but altered skin sensations are a recognized symptom of the hormonal transition. If the crawling feeling appeared around the same time as hot flashes, sleep changes, or irregular periods, the hormonal connection is worth exploring.

Vitamin B12 and Other Deficiencies

Vitamin B12 plays a crucial role in producing myelin, the protective coating that surrounds your nerves and helps them transmit sensations accurately. Without enough B12, that coating breaks down. Nerves become more prone to damage and start misfiring, which often shows up as tingling, pins-and-needles, or crawling feelings, particularly in the hands and feet.

Folate and B6 deficiencies can cause similar problems. Other signs of B12 deficiency include fatigue, low mood, lethargy, and shortness of breath. A standard blood test can identify the deficiency, and the nerve-related symptoms often improve once levels are corrected, though recovery depends on how long the deficiency has been present and how much nerve damage has occurred.

Nerve Damage From Diabetes and Other Conditions

Peripheral neuropathy, damage to the nerves outside your brain and spinal cord, is one of the most common medical explanations for persistent crawling sensations. Diabetes is the leading cause. Chronically elevated blood sugar slowly damages small nerve fibers, and the earliest symptoms often appear in the feet: tingling, numbness, burning, or that distinctive crawling feeling.

Multiple sclerosis can also produce these sensations. In MS, the immune system attacks the protective coating on nerves in the brain and spinal cord. Depending on where the damage occurs, symptoms can include numbness, tingling, weakness, or unusual sensory disturbances in the arms or legs. These episodes typically last more than 24 hours and may come and go over time.

Other conditions that damage peripheral nerves, including thyroid disorders, autoimmune diseases, and pinched nerves from herniated discs, can produce similar skin-crawling feelings.

Anxiety and Stress

High anxiety doesn’t just live in your head. It creates real physical symptoms, and skin-crawling sensations are among them. When you’re in a prolonged state of stress, your nervous system stays on high alert. Your brain becomes hypervigilant to sensory input, sometimes interpreting completely normal signals (a slight breeze, a clothing fiber, a tiny muscle twitch) as something alarming. In some cases, the brain generates the sensation entirely on its own, with no external trigger at all.

People who experience health anxiety can get caught in a feedback loop: the crawling sensation creates worry, the worry heightens nervous system sensitivity, and the heightened sensitivity makes the sensation feel stronger or more frequent. If you notice the feeling is worse during stressful periods or when you’re focused on it, anxiety is a likely contributor.

Medications and Substances

Stimulant drugs are closely linked to formication. Cocaine and methamphetamine use can trigger intense crawling or biting sensations, sometimes accompanied by the belief that bugs are living under the skin. This specific combination of sensation and belief is well-documented in stimulant psychosis.

Withdrawal from certain medications can also cause crawling skin. Stopping antidepressants, particularly SSRIs, too quickly sometimes produces sensory disturbances that include electric-shock feelings and skin crawling. Alcohol withdrawal, opioid withdrawal, and withdrawal from some anti-anxiety medications can produce similar effects. If the sensation started shortly after changing or stopping a medication, that timing is important information for your doctor.

Formication vs. Delusional Parasitosis

There’s an important distinction between feeling like something is crawling on you and being convinced that parasites have infested your body. Formication is the sensory experience itself: the crawling, tingling, or biting feeling. People with formication can generally accept proof that no insects are present, even though the sensation feels very real.

Delusional parasitosis is different. It involves a fixed belief that parasites are causing the sensation, one that persists even after medical examination and testing rule out any infestation. Before this diagnosis is considered, true infestations like scabies and other physical causes of itching have to be excluded first. The distinction matters because the two conditions are treated very differently.

What a Medical Workup Looks Like

If the crawling sensation persists, a doctor will typically start with a neurological exam: checking your reflexes, muscle strength, ability to feel different sensations, and coordination. Blood tests can reveal low vitamin levels, diabetes, thyroid problems, signs of inflammation, or metabolic issues that commonly cause nerve problems.

If those initial tests don’t provide an answer, more specialized testing may follow. Nerve conduction studies measure how quickly electrical signals travel through your nerves, and electromyography records the electrical activity in your muscles to detect nerve damage. In some cases, imaging like an MRI can identify pinched nerves or spinal issues. Rarely, a small skin biopsy may be taken to count the nerve endings in a specific area, which can reveal damage to the smallest nerve fibers that standard tests miss.

Managing the Sensation at Home

While you work toward identifying the underlying cause, a few practical strategies can reduce the intensity of the crawling feeling. Keeping your skin well-moisturized helps, especially if dryness is contributing to nerve irritation. Cool compresses or a cool shower can temporarily calm overactive nerve endings. Loose, soft clothing made from natural fibers reduces friction against sensitized skin.

Stress management matters more than you might expect. Techniques that lower your overall nervous system activation, like slow breathing, regular exercise, and consistent sleep, can reduce the frequency and intensity of phantom sensations. Avoiding caffeine and alcohol, both of which can increase nerve sensitivity, may also help. If you notice the sensation worsens at night or when you’re idle, gentle distraction (movement, a change of environment, cold water on the skin) can interrupt the signal.
