Why Does My Ankle Hurt When I Walk? Causes & Care

Ankle pain during walking is most commonly caused by a sprain, tendon inflammation, or early arthritis, though the specific location and quality of your pain narrows the possibilities significantly. Where you feel it, whether it came on suddenly or built up over weeks, and what the pain actually feels like are the three biggest clues to what’s going on.

Pain on the Outside of Your Ankle

The outer ankle is the most frequently injured part of the joint, and two conditions dominate here: ankle sprains and peroneal tendinitis.

An ankle sprain happens when the ligaments on the outside of your ankle stretch or tear, usually after rolling your foot inward. You don’t need a dramatic injury for this to happen. Stepping off a curb at an awkward angle or walking on an uneven surface can be enough. Sprains cause swelling, bruising, and a tenderness that flares with each step. Mild sprains involve stretching without a full tear and typically improve within a few weeks. Moderate sprains, where ligament fibers are partially torn, can take six weeks or longer. Severe sprains with a complete tear may need a cast or brace for a couple of weeks before rehabilitation begins, and full recovery stretches into months.

Peroneal tendinitis is inflammation in the tendons that run along the outer ankle bone and down the side of your foot. It tends to develop gradually rather than from a single event, often from a sudden increase in walking distance, hiking on slopes, or wearing shoes with poor lateral support. The pain usually worsens with activity and improves with rest. If you notice the outer ankle aching more at the end of the day or after longer walks, this is a likely culprit.

Pain on the Inside of Your Ankle

Inner ankle pain during walking often points to a problem with the posterior tibial tendon, which runs behind your inner ankle bone and supports the arch of your foot. When this tendon becomes chronically inflamed and starts to weaken, it can no longer hold the arch up properly. This condition progresses through stages, and understanding where you are in that progression matters.

Early on, you feel pain and mild swelling along the inner ankle, especially during or after walking, running, or climbing stairs. You might notice weakness when trying to push off your foot or rise onto your toes. As it worsens, the pain spreads, and physical changes in your foot become visible: the arch flattens, the ankle rolls inward, and the heel or toes may turn outward. Once the arch collapses, it changes how your foot distributes your body weight with every step. Over time, that altered loading pattern can damage ligaments and cartilage in other parts of the foot, creating a cascade of new problems.

If you’ve noticed your arch looking lower than it used to, or one foot appears flatter than the other, don’t dismiss it as normal aging. Early intervention with supportive footwear and targeted exercises can slow or stop the progression.

Deep, Aching Pain in the Joint

If your ankle pain feels like a deep stiffness that’s worst in the first few steps of the morning or after sitting for a while, ankle osteoarthritis is a common cause. Unlike knee or hip arthritis, ankle arthritis is usually “post-traumatic,” meaning it develops years or even decades after a previous injury like a fracture or severe sprain. The protective cartilage inside the joint gradually wears down, and the bones start to develop spurs and cysts.

Walking on an arthritic ankle often produces a grinding or catching sensation. Range of motion decreases over time, and you may find yourself unconsciously shortening your stride. Diagnosis typically involves weight-bearing X-rays that reveal narrowing of the joint space, bone spurs, and other changes. In some cases, an MRI or CT scan is needed to assess cartilage damage, cysts beneath the bone surface, or problems with surrounding ligaments and tendons.

Burning, Tingling, or Numbness

Ankle pain that comes with burning sensations, tingling, or pins-and-needles feelings points toward nerve involvement. Tarsal tunnel syndrome occurs when the tibial nerve, which passes through a narrow bony channel on the inner side of your ankle, becomes compressed or damaged. The pain typically radiates from the inner ankle into the bottom of the foot. It can feel electric or buzzing rather than the dull ache of a tendon or joint problem.

Nerve-related ankle pain often worsens with prolonged standing or walking and may linger after you stop. Swelling from other conditions, flat feet, or even a small cyst near the tunnel can all compress the nerve and trigger symptoms.

Sharp Pain That Worsens With Every Step

Stress fractures in the ankle area cause pain that develops gradually, worsens during weight-bearing activities, and is pinpointed to a very specific spot. Unlike soft tissue injuries where pain can feel diffuse, stress fracture pain is typically limited to the area directly over the injured bone. Pressing on that exact spot reproduces the pain. You may also notice swelling on the top of the foot or the outer ankle, and sometimes bruising.

The talus, one of the bones that forms the ankle joint, can develop stress fractures that cause hard-to-localize pain in the heel or ankle area. The fibula, the thinner bone on the outside of your lower leg, is another common site. Stress fractures are most often caused by a rapid increase in walking or running volume, especially on hard surfaces or in worn-out shoes. They require reduced weight-bearing to heal and are sometimes missed on initial X-rays, needing an MRI for confirmation.

How to Manage Ankle Pain at Home

For most soft tissue ankle injuries, current sports medicine guidance has moved beyond the old “RICE” advice of rest, ice, compression, and elevation. A more comprehensive framework emphasizes two phases: protecting the injury early on, then gradually reloading it.

In the first few days, protect the ankle from further damage, avoid movements that increase pain, and let natural inflammation do its job. This last point surprises many people. While ice numbs pain effectively, there’s no strong evidence it speeds healing, and it may actually slow the body’s repair process by disrupting blood flow and delaying the immune cells that clean up damaged tissue. Anti-inflammatory medications carry a similar trade-off: they reduce pain and swelling but may interfere with optimal tissue repair. If the pain is manageable without them, that’s worth considering.

After the initial protection phase, the goal shifts to gradually loading the ankle again. Gentle movement within a pain-free range, progressive weight-bearing, and eventually strengthening exercises help the tissues rebuild stronger than if you simply rested until the pain disappeared. Complete immobilization beyond the first few days tends to produce weaker, stiffer outcomes.

Footwear That Reduces Ankle Strain

What you put on your feet has a direct impact on ankle pain. If you have flat feet or your ankle tends to roll inward, shoes with firm medial support (the inner side of the sole) help control that motion. A rigid heel counter, the stiff cup at the back of the shoe, reduces strain on the Achilles tendon compared to soft, flexible designs.

For ankles that feel unstable or have a history of sprains, shoes that extend slightly above the ankle bone provide meaningful lateral support. High-top designs with lace-up closures let you adjust tightness precisely. A slight heel elevation of around 10 to 12 millimeters (the difference in height between the heel and forefoot of the shoe) can reduce tension on structures along the bottom of the foot and lower leg.

Signs You Need Medical Evaluation

Some ankle symptoms warrant prompt attention. If you felt a pop at the time of injury, especially if it felt like someone kicked the back of your leg when nobody was there, you should be seen within 24 to 48 hours, as this can indicate a ruptured Achilles tendon. Other symptoms that call for evaluation include visible deformity (the ankle looks misshapen), inability to bear weight or lift your foot, sudden sharp pain, and bruising or swelling that persists beyond 48 hours.

One less obvious cause worth knowing: a blood clot in the leg can produce ankle pain along with swelling and pain in other parts of the leg, such as behind the knee. If ankle pain appears alongside calf swelling or tenderness without an obvious injury, that combination needs same-day evaluation.
