Why Does My Bruise Have Dots? Causes and When to Worry

The tiny dots you’re seeing in or around your bruise are most likely petechiae, pinpoint spots caused by broken capillaries leaking small amounts of blood into the skin. They’re under 4 millimeters in diameter (roughly the size of a pinhead) and typically appear red, purple, or dark brown depending on your skin tone. While they often show up alongside a regular bruise from the same injury, they can also signal something else going on beneath the surface.

What Those Dots Actually Are

Your skin contains a dense network of capillaries, the tiniest blood vessels connecting your arteries and veins. When these capillaries break, blood leaks into the surrounding tissue. A standard bruise forms when a larger pocket of blood pools under the skin. Petechiae form when individual capillaries rupture separately, each creating its own tiny dot rather than one large discolored area.

The size of the spot determines its medical name. Dots under 4 mm are petechiae. Spots between 4 and 10 mm are classified as purpura. Anything larger than 1 centimeter is an ecchymosis, which is what most people simply call a bruise. You can have all three at once from a single injury, which is why a bruise sometimes looks like it has a speckled border or a cluster of pinpoint dots scattered around it.

Why Dots Appear Around a Bruise

The most common reason is straightforward: the same impact that caused your bruise also damaged smaller surrounding capillaries. A hard bump to the shin, for example, can create a central bruise with a halo of petechiae where the force radiated outward. This is normal and not a sign of a deeper problem.

But petechiae don’t always need a direct hit. Physical straining is a surprisingly common trigger. Intense coughing, vomiting, heavy weightlifting, or even giving birth can generate enough pressure to burst capillaries, particularly in the face, neck, and chest. If you’ve been sick with a stomach bug or had a bad coughing spell, that alone could explain the dots.

Medications That Increase Dotting

Certain medications make your capillaries more fragile or reduce your blood’s ability to clot, which means even minor bumps can produce petechiae alongside bruises. Blood thinners are the most well-known culprits. Anti-inflammatory painkillers like ibuprofen and aspirin also interfere with clotting and can make you bruise more easily, with more visible dots around the edges.

If you recently started a new medication and notice you’re bruising more easily or seeing dots you didn’t used to get, that’s worth mentioning to your prescriber. The dots themselves aren’t dangerous, but they can be a visible sign that your clotting is more suppressed than expected.

When Dots Point to a Platelet Problem

Platelets are the tiny cell fragments in your blood responsible for plugging leaks in damaged blood vessels. When your platelet count drops too low, a condition called thrombocytopenia, your body struggles to seal off even minor capillary breaks. People with platelet counts in the lower ranges often develop spontaneous petechiae, purpura, and easy bruising without any obvious injury.

A wide range of things can lower platelets: viral infections, autoimmune conditions, certain medications, heavy alcohol use, or bone marrow problems. The pattern to watch for is petechiae showing up repeatedly in places you haven’t injured, especially the lower legs, or bruises appearing from very light contact. A simple blood test can check your platelet count.

Nutritional Deficiencies and Capillary Health

Your body needs vitamin C to build and maintain the collagen that gives capillary walls their strength. Without enough of it, those walls weaken and break more easily. Severe vitamin C deficiency (scurvy) is rare in developed countries, but milder deficiencies are more common than you’d think, especially in people with very restricted diets or heavy smokers. Unexplained petechiae, slow wound healing, and bleeding gums together can all point to low vitamin C.

Vitamin K plays a different but equally important role. It’s essential for your blood to form clots properly. When vitamin K levels are low, even small amounts of capillary bleeding don’t seal off the way they should, leading to petechiae and prolonged bruising. Leafy greens are the richest dietary source, so people who eat very few vegetables are at higher risk.

The Glass Test for Dots That Worry You

One key feature of petechiae is that they don’t blanch, meaning they don’t temporarily disappear when you press on them. You can check this at home by pressing a clear drinking glass firmly against the dots. If the spots stay visible through the glass while you’re pressing down, they’re non-blanching, confirming that the color comes from blood trapped outside the vessels rather than blood still flowing through them.

This test is sometimes associated with meningitis screening because a non-blanching rash can accompany serious infections like meningococcal disease. That said, most petechiae are not caused by infection. The glass test is useful as one piece of information, not a diagnosis on its own. Non-blanching dots combined with fever, severe headache, neck stiffness, or feeling rapidly unwell is a combination that warrants emergency attention.

How Long the Dots Take to Fade

Petechiae from mild causes typically fade within two to three days. They don’t usually cycle through the yellow-green color changes of a regular bruise because the amount of trapped blood is so small that your body reabsorbs it quickly. A regular bruise, by contrast, can take two weeks or more to fully resolve because there’s a much larger volume of blood to clear.

If your dots are fading on their own and you can trace them back to an obvious cause (a bump, a coughing fit, a new medication), that’s usually reassuring. Dots that keep appearing in new locations without an explanation, grow in number over days, or come with other symptoms like fatigue, frequent nosebleeds, or unusually heavy periods suggest something systemic that’s worth investigating with a blood test.
