Why Does My Cavity Filling Hurt? Causes and Fixes

Some sensitivity after a cavity filling is normal, but pain that lingers or gets worse usually points to a specific, fixable problem. For most people, mild discomfort fades within two to four weeks. If yours hasn’t, or if the pain is sharp, throbbing, or triggered by biting down, something beyond routine healing is likely going on.

Normal Sensitivity vs. a Real Problem

Every filling involves drilling into tooth structure, which irritates the nerve inside. Your tooth needs time to calm down after that. During the first few days, you can expect twinges when you eat something hot, cold, or sweet. This is your tooth’s nerve reacting to the trauma of the procedure, and it gradually settles as the tooth heals.

The key number to remember is four weeks. If sensitivity is still fading and getting milder over that window, you’re on track. Pain that lasts longer than a month, gets worse instead of better, or wakes you up at night is not part of normal recovery. That’s your signal to call your dentist.

Your Filling May Be Too High

This is one of the most common reasons a filling hurts, and also one of the easiest to fix. When your dentist shapes the filling, your mouth is numb, which makes it hard to bite down naturally. If the filling sits even slightly higher than your other teeth, it absorbs extra force every time you chew. That repeated impact sends pain signals through the tooth.

A high filling doesn’t just cause soreness at the filled tooth. It can trigger headaches, jaw pain, and facial muscle tension, especially first thing in the morning if you clench or grind at night. Over time, biting down hard on a high spot can actually crack the filled tooth, turning a simple adjustment into a root canal or extraction. If your bite feels “off” or you notice pain specifically when chewing, ask your dentist to check the height. The fix takes a few minutes of reshaping and requires no numbing.

The Filling Was Close to the Nerve

Deep cavities sit closer to the pulp, the soft tissue inside your tooth that contains nerves and blood vessels. When a dentist drills away decay that extended near the pulp, the remaining layer of tooth between the filling and the nerve is thin. That thin barrier transmits temperature changes and pressure more easily, which is why deep fillings tend to produce sharper, longer-lasting sensitivity than shallow ones.

In most cases, the pulp recovers. But if the decay was severe enough to inflame the pulp before or during treatment, you may develop pulpitis, an inflammation of that inner tissue. Early-stage pulpitis is reversible: the tooth can heal once the cavity is sealed. But if you notice a lingering sensitivity to hot or cold that doesn’t fade after the source is removed, the inflammation may have progressed. At that point the pulp tissue can no longer recover on its own, and the tooth typically needs root canal treatment to resolve the pain.

What Different Types of Pain Mean

The character of your pain is a useful clue:

  • Sharp, momentary zap with hot or cold: This is the most common post-filling complaint. The pain hits fast and disappears within seconds once the temperature source is removed. It usually reflects minor nerve irritation that resolves on its own.
  • Lingering sensitivity to temperature: If the ache continues for 30 seconds or more after you pull the hot coffee or ice cream away, the pulp inside the tooth may be inflamed beyond its ability to heal.
  • Constant, throbbing ache: A toothache that throbs on its own, without any trigger, suggests the decay reached the pulp or an infection is developing. This type of pain tends to get worse, not better.
  • Pain only when biting down: Almost always points to a filling that’s sitting too high or, less commonly, a small crack in the tooth.

Galvanic Shock From Mixed Metals

If you have metal fillings or crowns made of different materials, you may occasionally feel a sharp, electric-like jolt when your teeth touch. This happens through a process called oral galvanism: saliva acts as a conductor between two dissimilar metals (gold and amalgam, for instance), generating a tiny electrical current. It’s most common right after getting a new metal filling near an existing one made from a different alloy. The sensation is startling but typically harmless, and it often diminishes as the metal surfaces oxidize over time. Replacing one of the metals with a non-metallic material eliminates it entirely.

Signs of Infection

A filling seals out bacteria, but if the seal breaks down or decay was left beneath it, infection can develop. A tooth abscess produces a distinct set of symptoms that go beyond ordinary sensitivity: severe, constant, throbbing pain that radiates into your jaw, neck, or ear. You may notice swelling in your face or cheek, tender lymph nodes under your jaw, fever, or a foul taste in your mouth. Sometimes a small pimple-like bump appears on the gum near the affected tooth.

If an abscess ruptures, you’ll get a sudden rush of salty, bad-tasting fluid and the pain may temporarily ease. That relief is misleading. The infection is still present. Fever combined with facial swelling, difficulty breathing, or trouble swallowing means the infection is spreading and warrants emergency care.

Managing Sensitivity at Home

While you’re waiting for normal post-filling sensitivity to fade, a few changes can make the process more comfortable. Desensitizing toothpaste, the kind labeled for sensitive teeth, works by blocking pain signals from the tooth surface. It takes a week or two of regular use to build up its effect, so consistency matters more than brand.

Switch to a soft-bristled toothbrush and use gentle strokes. Aggressive scrubbing can wear down the margins of a filling and expose sensitive tooth structure. Stick with fluoride toothpaste and avoid any product labeled “whitening” or “abrasive,” as these can increase sensitivity. When drinking acidic beverages like citrus juice or soda, use a straw to keep the liquid away from the filled tooth, and follow up with plain water to neutralize the acid. If you grind your teeth at night, a mouth guard reduces the constant pressure that keeps the nerve irritated.

How Modern Bonding Techniques Help

Post-filling pain has become less common over the past decade thanks to improvements in how fillings are bonded to teeth. Newer bonding agents seal the microscopic tubes in your tooth’s inner layer that connect to the nerve, reducing the amount of stimulation that reaches the pulp. Some of these agents work without a separate etching step, which simplifies the procedure and makes the result less dependent on perfect technique. Clinical studies show these bonding agents provide sensitivity relief comparable to dedicated desensitizing treatments, and the protective layer remains intact for at least 12 months. If you’ve had older fillings that were sensitive and a newer one that wasn’t, the bonding material is likely the difference.
