Why Does My Dog Keep Getting UTIs: Causes & Prevention

Dogs that keep getting urinary tract infections almost always have an underlying reason, whether it’s an anatomical issue, a hormonal condition, or incomplete treatment of a previous infection. A one-off UTI is common and usually resolves with antibiotics, but three or more episodes in 12 months (or two in six months) qualifies as recurrent bacterial cystitis, and that pattern signals something deeper is going on.

What Counts as Recurrent

A single UTI in an otherwise healthy dog is considered sporadic. Your dog fits the “sporadic” category if she’s a healthy, non-pregnant female or neutered male with no known anatomical abnormalities, no underlying diseases like diabetes, and fewer than three UTI episodes in the past year. Once infections start clustering beyond that threshold, vets shift their approach from simply treating each infection to investigating why it keeps coming back.

Recurrent UTIs fall into two patterns. Relapse means the same bacterial strain returns after treatment, which usually points to incomplete antibiotic courses, stones harboring bacteria, or deep-seated infection the original treatment didn’t fully clear. Reinfection means a new strain of bacteria causes a fresh infection, which suggests something about your dog’s body is making it easy for bacteria to repeatedly gain a foothold.

Anatomy That Invites Bacteria

Female dogs get UTIs far more often than males simply because they have a shorter urethra, giving bacteria a shorter path to the bladder. But some dogs have structural features that make things worse. A recessed vulva, where excess skin folds cover the vulvar opening, traps moisture and bacteria against the urogenital tract and predisposes dogs to repeated ascending infections. Breeds with naturally heavy skin folds, like English Bulldogs and Pugs, are especially prone.

Ectopic ureters, where one or both tubes from the kidneys connect to the wrong spot, also create conditions for chronic infection. These are congenital, meaning your dog was born with them, and they’re more common in breeds like Labrador Retrievers, Golden Retrievers, and Siberian Huskies. If your dog has had UTIs since puppyhood alongside urine dribbling, this is worth investigating.

Hormonal and Immune Conditions

Two endocrine disorders are strongly linked to recurrent UTIs in dogs: Cushing’s disease (where the body overproduces cortisol) and diabetes. Both suppress the immune system, making it harder for your dog’s body to fight off bacteria that enter the urinary tract. Dogs with these conditions are considered immunocompromised, and their infections carry a higher risk of spreading to the kidneys or bloodstream if left untreated.

Cushing’s disease also causes dogs to drink and urinate excessively, which dilutes the urine. While you might think more urination would flush bacteria out, the diluted urine actually has a lower concentration of the natural antibacterial compounds that normally help keep the bladder sterile. Diabetes produces a similar effect, with the added problem that sugar in the urine provides fuel for bacterial growth.

Spayed females with low estrogen levels can develop changes in the vaginal tissue that reduce its natural resistance to bacterial colonization. This is more common in older spayed dogs and sometimes contributes to a cycle of recurring infections.

Bladder Stones as a Hidden Reservoir

Bladder stones and recurrent UTIs often feed each other in a frustrating loop. Struvite stones, the most common type in dogs, typically form because of a UTI rather than causing one. Certain bacteria produce an enzyme called urease that changes the chemistry of urine, making it alkaline enough for mineral crystals to clump together into stones. Staphylococcus and Proteus species are the usual culprits.

Once a stone forms, bacteria can embed themselves inside it, shielded from antibiotics. The infection clears temporarily with medication, but bacteria re-emerge from the stone and the cycle starts over. This is why recurrence after treatment often traces back to incomplete dissolution or removal of stones combined with lingering infection. Your vet can detect most bladder stones with X-rays or ultrasound.

Incomplete Treatment and Resistant Bacteria

Sometimes the problem isn’t your dog’s body but the treatment itself. Stopping antibiotics early because your dog seems better, or using the wrong antibiotic for the specific bacteria involved, can leave a low-grade infection smoldering in the bladder. That infection flares back up weeks later, looking like a “new” UTI when it’s really the old one returning.

Antibiotic resistance is a growing concern in canine UTIs. E. coli, the most common bacterium behind these infections, shows alarming resistance patterns. A study of E. coli from dog UTIs found that about 71% of isolates were multidrug-resistant, meaning they didn’t respond to multiple classes of antibiotics. Nearly 60% were resistant to amoxicillin, and about 31% resisted the combination of trimethoprim and sulfonamides, both of which are commonly prescribed as first-line treatments.

This is exactly why urine cultures matter so much for dogs with recurring infections. A culture identifies the exact bacteria causing the problem, and sensitivity testing reveals which antibiotics will actually work against it. Research from Michigan State University’s veterinary lab estimates that roughly 40% of the time, the antibiotic a vet initially prescribes should be changed once culture results come back. Without a culture, you might be giving your dog a medication that never had a chance of clearing the infection.

Why Urine Cultures Are Essential

For a first-time, straightforward UTI, many vets will prescribe antibiotics based on a urinalysis alone. That’s reasonable. But for a dog on her second or third infection, skipping a culture is a mistake. The culture tells you two critical things: what organism is growing and which drugs can kill it. A standard urinalysis can confirm that bacteria and white blood cells are present, but it can’t tell you whether those bacteria will respond to amoxicillin or need something more targeted.

If your vet hasn’t run a culture on your dog’s recurring infections, ask for one. The sample should ideally be collected by cystocentesis (a needle directly into the bladder through the belly wall), which sounds uncomfortable but is quick, well-tolerated, and avoids contamination from bacteria on the skin or in the lower urinary tract. A follow-up culture five to seven days after finishing antibiotics confirms the infection is truly gone, not just temporarily suppressed.

What Your Vet Will Look For

When a dog presents with multiple UTIs, the workup typically goes beyond a urine test. Your vet will likely recommend bloodwork to screen for diabetes and Cushing’s disease, abdominal imaging (X-rays or ultrasound) to check for bladder stones, kidney abnormalities, or masses, and a physical exam that includes evaluating the vulva for excessive skin folds. In some cases, advanced imaging or cystoscopy (a tiny camera inserted into the bladder) may be needed to identify structural problems.

The goal is to find and address the root cause rather than just treating each infection as it pops up. A dog with Cushing’s disease will keep getting UTIs until the cortisol overproduction is controlled. A dog with a large struvite stone will relapse until the stone is dissolved or removed. A dog with a deeply recessed vulva may benefit from a surgical procedure called vulvoplasty to reduce skin folding and improve airflow around the area.

What Actually Helps With Prevention

You may have seen cranberry supplements or D-mannose marketed for dogs with UTI problems. The evidence behind these products is thin. The International Society for Companion Animal Infectious Diseases reviewed the available data and concluded there is insufficient evidence to recommend cranberry extract products or other alternative therapies for preventing cystitis in dogs. That doesn’t mean they’re harmful, but they shouldn’t replace proper veterinary investigation.

Practical steps with better support include encouraging your dog to drink more water (which promotes more frequent urination and natural flushing of bacteria), allowing frequent bathroom breaks so urine doesn’t sit in the bladder for hours, and keeping the area around the vulva clean and dry, especially in dogs with skin folds. For overweight dogs, weight loss can reduce skin folding around the vulva and improve overall immune function.

If your dog has been prescribed a full course of antibiotics, finishing every dose matters. Even if symptoms resolve in two or three days, the bacteria may not be fully eliminated. Cutting treatment short is one of the most common drivers of both relapse and the development of resistant bacteria.
