Why Does My Foot Hurt After Running? Common Causes

Foot pain after running usually comes from repetitive stress on one of several structures in the foot, and where exactly it hurts is the strongest clue to what’s going on. About a quarter of all running injuries involve the foot and ankle, with plantar fasciitis, stress fractures, and tendon problems topping the list. The good news: most causes are treatable and don’t require you to stop running permanently.

Heel Pain: Likely Plantar Fasciitis

The most recognizable post-run foot pain hits the bottom of your heel, right where a thick band of tissue called the plantar fascia attaches to the heel bone. The signature symptom is sharp pain with your first steps in the morning or after sitting for a while. It can also flare up after a long run or extended time on your feet. In a study of recreational runners, the median recovery time for plantar fasciitis was about 35 days, though some cases lingered significantly longer.

A recent jump in mileage, speed work, or time spent standing are common triggers. The pain tends to be worst at the inner edge of the heel and may ease up once you’re moving, only to return after you stop. If pressing your thumb firmly into the inside of your heel reproduces the pain, plantar fasciitis is the most likely culprit.

Top of Foot Pain: Stress Fractures or Tendon Irritation

Pain across the top of the foot often points to one of two problems: a stress fracture in one of the metatarsal bones, or irritation of the tendons that run along the top of your foot.

Stress fractures account for up to 16% of all running injuries, and 55% of foot and ankle stress fractures occur in the metatarsals, most often the first and second. The pain typically starts as a vague ache in the midfoot that gets worse with activity and better with rest. There’s usually a specific tender spot over the affected bone. Unlike muscle soreness, this pain doesn’t warm up and fade during a run. It gets progressively worse.

Tendon irritation on the top of the foot, called extensor tendonitis, feels more like a broad ache or stiffness along the top of your foot rather than a single pinpoint. Shoes that are too tight or laced too aggressively across the top of the foot are a common cause. You might notice swelling, warmth, or pain that increases when you pull your toes upward. Loosening your laces or switching to a shoe with more room across the top often makes an immediate difference.

Ball of Foot Pain: Morton’s Neuroma

If you feel sharp, burning pain in the ball of your foot, especially between the third and fourth toes, a thickened nerve called Morton’s neuroma is a common explanation. People describe it as feeling like there’s a pebble or marble under the ball of the foot, or like a sock is bunched up inside the shoe. Tingling, pins and needles, and numbness in the toes often accompany it. Narrow running shoes that squeeze the forefoot tend to make this worse, and switching to a wider toe box can provide significant relief.

Arch and Inner Ankle Pain

Pain along the inner arch or behind the inner ankle bone often involves the tendon that supports your arch. Early signs include swelling along the inside of the ankle, tenderness when you flex your foot, and weakness when you try to push off while walking or running. Over time, the arch can gradually flatten, and you might notice your heel or toes pointing outward more than usual. This type of injury tends to worsen if ignored, so early attention matters more here than with most running complaints.

Achilles Tendon Pain

Pain at the back of the heel or along the lower calf, especially when you push off or go uphill, points to the Achilles tendon. Achilles injuries account for roughly 7% of running injuries and tend to recover slowly. In one study of recreational runners, the median recovery time was 56 days, with wide variation: some runners healed in weeks, while others dealt with symptoms for six months or longer. Morning stiffness in the tendon that loosens up with movement is a hallmark sign.

Why Running Causes These Injuries

Most running foot injuries come down to load exceeding capacity. Your bones, tendons, and connective tissue can handle a certain amount of stress, and they actually get stronger when challenged. But when the load increases too quickly, whether from a sudden mileage bump, a new pair of shoes, harder surfaces, or simply not enough recovery, tissue breaks down faster than it can rebuild.

Your shoes play a role too. Running shoes lose their cushioning properties over time, and the general recommendation is to replace them every 500 to 700 kilometers (roughly 300 to 430 miles). Worn-out midsoles create higher impact forces on the bottom of the foot, which can contribute to plantar fasciitis, stress fractures, and general soreness. If you can’t remember when you bought your current pair, that’s worth checking.

What to Do When Your Foot Hurts

The old RICE protocol (rest, ice, compression, elevation) has been the default advice for decades, but the thinking has shifted. A newer framework called PEACE and LOVE emphasizes protecting the injury initially, then gradually reintroducing movement rather than immobilizing the foot completely. The “optimal loading” part is key: gentle, pain-free movement promotes blood flow and tissue repair, while complete rest can actually slow recovery.

Ice still works for short-term pain relief, but there’s evidence it may interfere with the inflammatory process your body needs for healing. The same goes for anti-inflammatory medications. They can help you feel better in the moment, but using them routinely in the first few days may not be doing you any favors long-term.

For most minor tendon and soft tissue injuries, reducing your running volume by 30 to 50 percent, avoiding speed work and hills temporarily, and cross-training with low-impact activities like cycling or swimming allows the tissue to recover while maintaining fitness. Plantar fasciitis responds well to calf stretching, rolling a frozen water bottle under the arch, and supportive insoles. Stress fractures are the exception: they typically require a period of no running at all, sometimes in a walking boot, before a gradual return.

Signs That Need Professional Attention

Most post-run foot pain improves within a few weeks of reduced activity. But certain patterns warrant a visit to a sports medicine provider or podiatrist:

  • Pain that persists beyond six weeks despite backing off your training
  • Inability to bear weight for more than 48 hours after the pain starts
  • Significant swelling, bruising, or visible deformity without a clear traumatic event
  • Numbness, tingling, or weakness that is progressive or affects your balance
  • Pain that worsens at night or doesn’t respond at all to rest

Pain that keeps coming back in the same spot after it seems to resolve, or symptoms that last beyond three months despite conservative treatment, also justify further evaluation. These patterns sometimes indicate an incomplete healing process or an underlying structural issue that won’t resolve on its own.
