The most likely reason your heel hurts when you walk is plantar fasciitis, a condition where the thick band of tissue running along the bottom of your foot becomes irritated and inflamed. It accounts for the majority of heel pain cases in adults. But several other conditions can cause heel pain during walking, and where exactly you feel the pain, when it’s worst, and how it started all point toward different causes.
Plantar Fasciitis: The Most Common Cause
The plantar fascia is a strong band of connective tissue that stretches from your heel bone to the base of your toes, supporting the arch of your foot like a bowstring. When this tissue is repeatedly stretched and stressed, it develops small tears that cause inflammation and pain right where it connects to the heel bone, on the underside of your foot.
The hallmark symptom is pain with your first few steps after waking up. While you sleep, the fascia tightens in a shortened position. When you stand and load weight onto it, those micro-tears get pulled apart again, producing a sharp or stabbing pain at the bottom of the heel. The same thing happens after long periods of sitting. Most people notice the pain eases after a few minutes of walking as the tissue warms up and loosens, only to return after extended time on their feet.
Risk factors include spending long hours standing, a sudden increase in activity, tight calf muscles, higher body weight, and shoes with poor arch support. Runners and people who work on their feet are especially prone. The condition tends to develop gradually over weeks rather than appearing overnight.
Pain at the Back of the Heel
If your pain is at the back of the heel rather than the bottom, the problem is more likely related to the Achilles tendon. Achilles tendinitis causes pain along the tendon or where it attaches to the heel bone (called insertional tendinitis). This pain typically worsens with activity, not with first morning steps, and you may notice it flares when wearing shoes that press against the back of the heel. Bone spurs can develop at the attachment point over time, making shoes even more uncomfortable.
The key distinction: plantar fasciitis hurts on the underside of the heel, Achilles tendinitis hurts on the back. Both worsen with walking, but the timing and location differ.
Fat Pad Syndrome
Your heel bone sits on a cushion of specialized fatty tissue that absorbs shock with every step. As you age, this fat pad thins and loses elasticity, leaving the heel bone less protected against hard surfaces. The result is a deep, bruise-like pain in the center of the heel that gets worse with prolonged standing, walking on hard floors, or going barefoot.
Fat pad syndrome feels different from plantar fasciitis. The pain is more diffuse, centered in the middle of the heel rather than at the front edge of the heel bone. You can often reproduce it by pressing a thumb firmly into the center of your heel. Mild cases may only bother you when walking barefoot on tile or concrete, while more advanced cases hurt with any weight-bearing activity. People over 50 and those with a history of high-impact sports are most commonly affected.
Stress Fractures
A calcaneal stress fracture is a small crack in the heel bone caused by repetitive impact, not a single injury. The pain develops slowly over days to weeks and hurts with a generalized ache across the heel rather than one specific spot. A classic sign is pain when you squeeze both sides of the heel between your hands. If that squeeze reproduces your pain, a stress fracture is a real possibility.
Recovery typically follows a three-phase timeline spanning about 12 weeks. The first month involves rest and protected weight-bearing, followed by gradual loading over the next month, then a return to full activity by weeks 9 through 12. Stress fractures don’t always show up on initial X-rays, so your doctor may order advanced imaging if one is suspected.
Nerve Entrapment
Less commonly, heel pain comes from a pinched nerve rather than a structural problem. A nerve running along the inner side of the heel can become compressed, producing sharp, radiating pain that often worsens at night and after physical activity like walking. You might also notice tingling, numbness, or a burning sensation that spreads toward the outside of the foot. Over time, the small muscles on the outer edge of the foot can weaken and shrink slightly.
Nerve-related heel pain is often misdiagnosed as plantar fasciitis because the location overlaps. The distinguishing features are the radiating or electric quality of the pain, nighttime symptoms, and any tingling or numbness. If your heel pain doesn’t improve with typical plantar fasciitis treatments after several weeks, nerve entrapment is worth investigating.
Heel Pain in Children
If your child complains of heel pain during or after sports, the most likely cause is Sever’s disease, an irritation of the growth plate in the heel bone. It typically appears between ages 8 and 12, during rapid growth spurts, and is uncommon after age 15 once the heel bone has finished developing. A doctor can usually diagnose it based on symptoms and a physical exam alone. X-rays and blood tests aren’t normally needed.
Inflammatory Conditions
Heel pain that doesn’t clearly fit the patterns above, especially if combined with low back stiffness, can signal a systemic inflammatory condition. Enthesitis, inflammation where tendons and ligaments attach to bone, is a hallmark of conditions like ankylosing spondylitis. The heel is one of the most common sites, both at the Achilles tendon attachment and under the heel. This type of pain tends to be worse with rest and improves with movement, which is the opposite pattern from most mechanical heel pain.
What Helps Heel Pain
For the majority of cases, particularly plantar fasciitis, conservative measures resolve the problem within several months. The approach focuses on reducing stress on the affected tissue while it heals.
Footwear makes a significant difference. Look for shoes with a heel-to-toe drop of 8 to 12 millimeters, which reduces the load on your heel during each step. A firm heel counter (the rigid cup around the back of the shoe) minimizes wobble and strain on the fascia. Moderate cushioning that absorbs impact without collapsing, a roomy toe box, and removable insoles that can accommodate orthotics or heel cups all help. Some shoes with rocker-style soles promote a smoother roll from heel to toe, reducing pressure on the heel at impact.
Stretching the calves and the plantar fascia itself, rolling a frozen water bottle under the foot, and reducing high-impact activities during flare-ups are the most effective daily strategies. Over-the-counter arch supports or heel cups provide relief for many people, while custom orthotics may be appropriate for those with structural foot issues like overpronation.
Signs That Need Prompt Attention
Most heel pain is manageable at home, but certain patterns warrant faster evaluation. Severe pain and swelling immediately following an injury, inability to bend your foot downward or rise onto your toes, and heel pain accompanied by fever or numbness all call for prompt medical attention. Heel pain that persists even when you’re not standing or walking, or that lingers beyond a few weeks despite rest and icing, is also worth getting checked.
