Why Does My Lower Rib Cage Hurt: Common Causes

Lower rib cage pain most often comes from a musculoskeletal cause, such as a strained muscle, inflamed cartilage, or a rib that moves more than it should. Musculoskeletal conditions account for roughly 20% to 47% of chest wall pain cases seen in primary care. But because your lower ribs sit right over several organs, the same pain can sometimes signal a digestive, lung, or abdominal issue that needs attention.

Intercostal Muscle Strain

The muscles between your ribs, called intercostal muscles, are easy to strain through everyday movements you might not think twice about. Twisting your torso while lifting, playing golf or tennis, painting a ceiling, holding a baby at an awkward angle, or even doing certain yoga poses can all pull or tear these small muscles. The hallmark sign is pain that gets noticeably worse when you cough, sneeze, or take a deep breath.

Recovery depends on severity. A mild strain often resolves within a few days. Moderate strains can take three to seven weeks, and a complete tear may take longer. Most rib-area muscle injuries heal within about six weeks. In the meantime, ice for swelling, gentle movement, and avoiding the motion that caused the strain are the standard approach.

Slipping Rib Syndrome

Your bottom three rib pairs (ribs 8, 9, and 10) don’t attach directly to the breastbone. Instead, they connect to the rib above through cartilage. If that cartilage loosens or is damaged, the rib tip can slip under the one above it, producing a clicking or popping sensation along with sharp pain in the lower chest or upper abdomen. Some people are born with a tighter or looser connection that makes this more likely.

Slipping rib syndrome often gets misdiagnosed as a stomach problem because the pain sits so low. A doctor can check for it with a physical exam, curling fingers under the lower rib margin and pulling forward. Dynamic ultrasound is also effective, correctly identifying the condition in about 89% of cases in one study. Treatment ranges from physical therapy and nerve blocks to, in persistent cases, surgical stabilization of the rib.

Costochondritis

Costochondritis is inflammation where a rib connects to its cartilage. It accounts for about 42% of nontraumatic chest wall pain and is usually diagnosed through a physical exam alone, without imaging. The pain tends to be sharp or aching and worsens when you press on the affected area or take a deep breath.

Physical therapy can help, typically over four to six weeks of one to two sessions per week. Useful exercises include stretches that open the chest (like a pectoral corner stretch), shoulder blade squeezes, and gentle rib mobilizations that restore normal movement during breathing. Postural strengthening exercises reduce ongoing pressure on the inflamed cartilage. Heat can improve circulation to the area, while ice brings down pain and swelling. Most cases resolve on their own with time.

Trapped Gas and Digestive Causes

Gas can mimic rib pain surprisingly well. Your colon makes a sharp bend near your spleen on the left side, called the splenic flexure. When gas builds up at that turn, it can cause sharp pain in the upper left abdomen, right under the lower ribs. Some people have a naturally tighter bend in the colon that makes them more prone to this. The pain usually passes once the gas moves through, though it can be intense enough to worry you.

On the right side, gallbladder problems are a common culprit. The gallbladder sits just below the right rib cage, and inflammation or gallstones can produce pain that flares after eating fatty meals. Liver issues can also cause a dull ache or feeling of fullness under the right ribs.

Nerve-Related Pain

Sometimes the nerves that run along the underside of each rib become irritated or compressed, a condition called intercostal neuralgia. This produces a distinctly different sensation from a muscle strain: sharp, stabbing, or burning pain that can wrap around the rib cage from back to front. You may also notice tingling or numbness along the path of the nerve. The pain can be constant or come and go unpredictably, and in severe cases it affects your ability to move the area normally.

Intercostal neuralgia can develop after surgery, shingles, a rib injury, or sometimes without an obvious trigger. It’s worth mentioning to your doctor because the treatment approach differs from musculoskeletal pain. Nerve-targeted therapies tend to work better than standard anti-inflammatory strategies.

Spleen and Kidney Pain

Your spleen sits just below your left rib cage, tucked next to your stomach. When it becomes enlarged from infection, liver disease, or blood disorders, it can cause pain or a sense of fullness in the left upper belly that sometimes spreads to the left shoulder. You may feel full quickly when eating because the enlarged spleen presses against the stomach.

The kidneys sit toward the back, just below the lower ribs on each side. Kidney stones or infections often produce pain that wraps from the back around to the front of the lower rib area, sometimes accompanied by nausea or changes in urination.

When the Pain Needs Urgent Attention

Most lower rib pain turns out to be something manageable, but certain patterns warrant a prompt medical visit. Pain accompanied by fever could point to an infection in the lungs, kidneys, or abdominal organs. Shortness of breath alongside rib pain raises concern for pleurisy (inflammation of the lung lining, often from a viral or bacterial infection) or, more rarely, a blood clot in the lungs. Pain that radiates to the shoulder, jaw, or arm, or that comes with dizziness and sweating, needs immediate evaluation to rule out a cardiac cause.

How Doctors Figure Out the Cause

A chest X-ray is the standard first step for lower rib pain that doesn’t have an obvious muscular explanation. It can reveal fractures, lung problems, or bone abnormalities. Rib-specific X-ray views may be added when there’s a focal tender spot, since the most commonly fractured ribs from things like severe coughing are the mid to lower ribs, especially the 10th rib.

If the X-ray looks normal but your doctor suspects something deeper, such as an infection, inflammatory condition, or possible tumor, a CT scan is the usual next step. For suspected slipping rib syndrome, a dynamic ultrasound lets the radiologist watch the rib move in real time, which a static X-ray can’t capture. Many musculoskeletal causes, especially costochondritis, don’t need imaging at all and can be confidently diagnosed through a hands-on exam.
