Why Does My Wrist Hurt When I Put Pressure on It?

Wrist pain during weight-bearing or pressing down usually comes from a soft tissue or bone problem that only becomes apparent under load. Your wrist is a complex joint made up of eight small bones, multiple ligaments, and a cartilage disc, and pressure forces all of these structures to absorb and distribute weight simultaneously. When any one component is damaged or inflamed, pushing down on your palm can concentrate stress on that weak point and trigger sharp or aching pain.

The location of your pain, whether it started suddenly or gradually, and what movements make it worse all point toward different causes. Here’s how to narrow it down.

Pain on the Pinky Side: TFCC Injury

The most common reason for ulnar-sided wrist pain (the pinky side) during weight-bearing is a tear in the triangular fibrocartilage complex, or TFCC. This is a small disc of cartilage and ligaments that cushions the joint between your forearm bones and stabilizes your wrist when you rotate your hand or push against a surface. When you press your palm flat on a table or do a pushup, axial load travels straight through this disc. If it’s torn or worn, that pressure produces pain right at the bony bump on the outside of your wrist.

TFCC tears happen from falls onto an outstretched hand, repetitive twisting motions, or simple age-related wear. You’ll typically notice the pain worsens when you twist a doorknob, use a can opener, or push yourself up from a chair. Some people also feel a clicking sensation when rotating their forearm.

Pain Near the Thumb: Possible Scaphoid Fracture

If pressing down causes pain on the thumb side of your wrist, a scaphoid fracture is worth considering, especially if you recently fell on an outstretched hand. The scaphoid is a small bone at the base of your thumb that bears a disproportionate amount of force during any pushing or gripping motion. Pinching, grasping, pushing, and pulling all make the pain worse.

The telltale spot is the “anatomical snuffbox,” the small hollow that forms between the tendons on the back of your wrist when you stick your thumb up in a hitchhiking position. Tenderness there is a classic sign. Scaphoid fractures are notorious for not showing up on initial X-rays. If you fell in the past few weeks and this area is tender, imaging is important because untreated scaphoid fractures heal poorly due to the bone’s limited blood supply.

Pain on Top of the Wrist During Extension

If pain hits the back (dorsal side) of your wrist when you bend it backward or press your palm flat, dorsal impingement is a likely culprit. This happens when the wrist is loaded in an extended position, like during pushups, planks, or yoga poses. An MRI study of people with this type of pain found that 76% had dorsal ganglion cysts and 24% had damage to the scapholunate ligament, a key connector between two of the wrist’s carpal bones. The repetitive stress of weight-bearing in extension creates both high tensile forces and mechanical pinching of these structures, leading to inflammation and degeneration over time.

Ganglion cysts deserve special mention because they’re extremely common. These fluid-filled sacs typically form on the back of the wrist and can fluctuate in size. They’re often painless on their own, but if one presses on a nearby nerve, it can cause pain, tingling, numbness, or even muscle weakness. You might notice a visible or palpable lump that gets larger with increased wrist activity.

Pain Along the Thumb-Side Tendons

De Quervain’s tenosynovitis is inflammation of the tendons that run along the thumb side of your wrist. It makes gripping, twisting, and pressing painful. You can test for it at home: bend your thumb across your palm, fold your fingers over it, then tilt your wrist toward your pinky finger. If this reproduces a sharp pain on the thumb side of your wrist, de Quervain’s is the likely cause.

This condition is common in new parents (from repeatedly lifting a baby), people who use their thumbs heavily for phone scrolling or gaming, and anyone whose work involves repetitive hand motions.

Wrist Sprains and Ligament Damage

A sprained wrist can make weight-bearing painful for weeks. Sprains are graded by severity. A grade 1 sprain stretches a ligament without tearing it, causing mild pain and swelling while you can still use your wrist fairly normally. A grade 2 sprain involves a partial tear, producing noticeable pain even at rest and difficulty using the wrist without discomfort. A grade 3 sprain is a complete ligament tear that causes severe pain, significant swelling, and reduced range of motion, often making the wrist feel unstable.

The tricky part is that grade 1 and even some grade 2 sprains can feel manageable enough that people assume they’ll resolve on their own. But when a damaged ligament goes untreated, particularly the scapholunate ligament, it can lead to a progressive pattern of joint collapse called SLAC wrist. This condition develops in stages: it starts with localized arthritis near the tip of the radius bone, then spreads to involve more of the joint, and eventually causes one of the carpal bones to shift upward into the gap left by the torn ligament. Grip strength drops, range of motion shrinks, and the wrist becomes arthritic. People with SLAC wrist who require surgery recover to roughly 53% of their normal flexion-extension arc and about 80% of their grip strength compared to the uninjured side.

Less Common but Worth Knowing

Kienböck’s disease is a condition where the lunate bone, located roughly in the center of your wrist, loses its blood supply and gradually deteriorates. It produces pain that feels like a sprained wrist, either a dull ache that comes and goes or a sharper, more constant pain. You may notice stiffness, weakness, creaking or crackling sounds during movement, and reduced grip strength. Some people have no symptoms at all until the disease is discovered during an exam for something else. The pain is typically felt right over the center of the wrist and worsens with use.

What the Pain Location Tells You

  • Pinky side: TFCC tear, ulnar impaction, or grade 2-3 sprain of the ulnar ligaments
  • Thumb side: Scaphoid fracture, de Quervain’s tenosynovitis, or radial-side ligament sprain
  • Top/back of wrist: Dorsal impingement, ganglion cyst, or scapholunate ligament damage
  • Center of wrist: Kienböck’s disease, capitate stress injury, or generalized arthritis
  • Diffuse or hard to pinpoint: Wrist sprain, early osteoarthritis, or repetitive strain

Managing the Pain at Home

If your pain is mild and there’s no obvious deformity or severe swelling, modifying how you load your wrist is the first step. During exercises like pushups, try using dumbbells or pushup bars to keep your wrist in a neutral position rather than fully extended. For yoga, folding the edge of your mat under your palms reduces the extension angle. Avoid pressing through pain, as this can worsen ligament and cartilage injuries.

Strengthening the muscles around the wrist helps stabilize the joint and reduce pain over time. Grip strengthening with a rubber ball (squeeze and hold for five seconds, three sets of ten) builds the forearm muscles that support your wrist. Tendon glides, where you slowly curl and straighten your fingers through their full range, keep the tendons moving smoothly through their sheaths. Wrist range-of-motion exercises, bending your wrist gently forward and backward and holding each position for five seconds, maintain flexibility without heavy loading. Scapular squeezes, pulling your shoulder blades together and holding for five seconds, address the postural component that can shift extra load onto the wrists.

If pain and swelling persist beyond a few days or worsen over time, imaging is important. Delayed diagnosis of fractures, ligament tears, and conditions like Kienböck’s disease leads to poorer healing outcomes, reduced range of motion, and long-term disability. This is especially true for scaphoid fractures and scapholunate injuries, where early treatment can prevent the kind of progressive joint damage that becomes much harder to fix later.
