Why Does Pepto-Bismol Turn Your Tongue Black?

Pepto-Bismol turns your tongue black because its active ingredient, bismuth subsalicylate, reacts with trace amounts of sulfur in your mouth to form a dark compound called bismuth sulfide. The discoloration is harmless and temporary, typically clearing up within a few days after you stop taking the medication.

The Chemical Reaction Behind It

Your mouth is home to billions of bacteria, many of which naturally produce sulfur compounds as they break down proteins. Certain types of bacteria metabolize sulfur-containing amino acids (the building blocks of protein) and release tiny amounts of hydrogen sulfide and related gases. These are the same compounds responsible for bad breath.

When bismuth from Pepto-Bismol meets those sulfur traces in your saliva, the two combine to form bismuth sulfide. Bismuth sulfide is jet black, and even a small amount is enough to coat the surface of your tongue with a dark, sometimes alarming-looking film. The reaction happens quickly, which is why discoloration can show up after just one or two doses.

It Happens in Your Gut, Too

The same reaction occurs throughout your digestive tract. Once Pepto-Bismol reaches your stomach, it breaks down into bismuth and salicylic acid. The bismuth continues through your intestines, encountering sulfur produced by gut bacteria along the way. The result: black or very dark stools. This is one of the most commonly reported side effects of bismuth subsalicylate, and like the tongue discoloration, it’s completely harmless.

The important distinction is between bismuth-darkened stool and a condition called melena, which is black, tarry, sticky stool caused by bleeding in the upper digestive tract. Melena has a distinctive sticky texture and a strong, unusual odor. If you’ve been taking Pepto-Bismol and your stool turns dark, the medication is almost certainly the cause. But if you notice black, tarry stools that persist after stopping the medication, or you haven’t taken any bismuth-containing products, that warrants medical attention.

How Long the Black Tongue Lasts

Pepto-Bismol itself clears your system within about a day, but the staining on your tongue can linger for several days after your last dose. The bismuth sulfide sits on the surface of the tongue’s tiny bumps (called papillae), and it takes time for normal cell turnover and oral hygiene to clear it away. Most people see the discoloration fade within two to three days without doing anything special.

Speeding Up the Process

You can help the black coating disappear faster with basic tongue hygiene. Gently brush your tongue each time you brush your teeth, using either a soft-bristled toothbrush or a tongue scraper. This removes the layer of bismuth sulfide along with dead cells, bacteria, and food debris that accumulate on the tongue’s surface. Brushing at least twice a day and flossing once a day also helps by reducing the overall bacterial load in your mouth, which means less sulfur production for the bismuth to react with.

There’s no need for any special mouthwash or treatment. The discoloration resolves on its own even if you do nothing.

Why Some People Get It and Others Don’t

Not everyone who takes Pepto-Bismol notices a black tongue. The intensity of the reaction depends on how much sulfur-producing bacteria you have in your mouth at the time. People with higher levels of certain gram-negative anaerobic bacteria, the types most active in breaking down sulfur-containing amino acids, are more likely to see dramatic discoloration. Factors like oral hygiene habits, diet, and the overall makeup of your oral microbiome all influence how much sulfur is available to react with the bismuth.

The dose matters, too. Liquid Pepto-Bismol coats the tongue more directly than tablets, so it tends to produce more noticeable staining. If the appearance bothers you, chewable tablets or caplets may reduce (though not eliminate) the effect.

When Black Tongue Means Something Else

Bismuth sulfide staining is different from a condition called black hairy tongue, where the papillae on the tongue grow unusually long and trap bacteria, food, and dead cells, creating a dark, fur-like appearance. Black hairy tongue can be triggered by antibiotics, tobacco use, excessive coffee or tea consumption, or poor oral hygiene. It affects anywhere from 0.6% to 11.3% of people depending on the population studied.

If your tongue remains discolored for more than a week after stopping Pepto-Bismol, or if the texture of your tongue looks visibly different with elongated or hair-like projections, that’s a separate issue worth mentioning to your dentist. But if you took Pepto-Bismol in the last few days and your tongue turned dark, you’re looking at a straightforward chemical reaction that will resolve on its own.
