Why Does Sugar Hurt My Teeth? Causes Explained

Sugar hurts your teeth through two distinct mechanisms: bacteria on your teeth convert sugar into acid that dissolves enamel over time, and sugar can pull fluid through tiny tubes in your teeth to trigger nerve pain almost instantly. Which one you’re experiencing depends on whether your teeth are healthy or already have some damage.

The Immediate Sting: Osmotic Pressure

If you bite into something sweet and feel a sharp, sudden pain, that’s not acid at work. It’s physics. Your teeth contain thousands of microscopic tubes called dentinal tubules that run from the outer surface toward the nerve at the center. These tubes are filled with fluid. When sugar lands on an area where enamel has worn thin or a filling has a gap, it creates an osmotic pull on that fluid, drawing it outward. The movement is tiny, but it’s enough to trigger a pressure receptor connected to the nerve inside your tooth.

This is called the hydrodynamic theory, and it’s the most widely accepted explanation for tooth sensitivity. Cold air, ice water, and sugar all cause pain through the same basic mechanism: they disturb fluid inside those tubules, and the nerve interprets that disturbance as pain. Sugar does it through osmotic changes rather than temperature, but the result feels similar.

The Slow Burn: Acid From Bacteria

The longer-term damage comes from bacteria, especially a species called Streptococcus mutans that thrives in your mouth. When you eat sugar, these bacteria eat it too. They break it down through a process that produces lactic acid as a byproduct, and that acid is surprisingly strong. It drives down the pH on your tooth surfaces, and once the pH drops below about 5.5, the mineral structure of your enamel starts dissolving.

Every time you consume sugar, you trigger one of these acid attacks. Your mouth doesn’t return to a safe pH instantly. Depending on what you consumed, it can take anywhere from 6 to 15 minutes for saliva to bring the pH back to baseline. Sweetened milk clears fastest at around 6.5 minutes, while sodas and fruit drinks can keep the pH low for 13 to 15 minutes. If you’re sipping a sugary drink throughout the afternoon, you’re essentially keeping your teeth in an acid bath for hours.

Sucrose, ordinary table sugar, is the worst offender. It’s considered the most cavity-causing carbohydrate because it’s particularly easy for oral bacteria to metabolize, and it promotes bacterial growth patterns that favor tooth decay more than other sugars do. Sticky sugary foods are even more damaging because they cling to tooth surfaces longer, extending the acid attack. Combining sugar with starch, like in cookies or pastries, may be more harmful than sugar alone.

How Your Teeth Fight Back

Your saliva is essentially a repair solution. It contains calcium and phosphate ions that can rebuild the mineral structure of enamel after an acid attack, a process called remineralization. Above a pH of 5.5, these minerals in saliva actually deposit back onto your teeth, patching the microscopic damage. Proteins in saliva also form a protective film over enamel.

Here’s the catch: remineralization only works if your saliva gets enough downtime between sugar exposures. When acid attacks come faster than your saliva can repair, damage accumulates. This is why frequency of sugar consumption matters more than total amount. Three candies eaten at once cause one acid attack. Three candies eaten an hour apart cause three separate attacks, tripling your enamel’s exposure to damaging pH levels.

Not All Sugar Pain Means the Same Thing

The type of pain you feel from sugar tells you something important about what’s happening inside your tooth. A sharp zing that disappears within a few seconds after the sugar is gone suggests reversible pulpitis, meaning the nerve inside your tooth is irritated but not permanently damaged. This is common with minor cavities, receding gums, or worn enamel. The tooth can still recover with treatment.

If the pain lingers for more than a few seconds after you swallow or rinse the sugar away, especially if it becomes a deep throb or ache, that points toward irreversible pulpitis. The nerve tissue is inflamed beyond the point of healing on its own. At this stage, you’ll typically also notice sensitivity to heat, not just sweets and cold. Interestingly, if a tooth that used to hurt with sugar suddenly stops hurting altogether but still aches when you press on it, the nerve may have died. The lack of sensitivity isn’t a good sign in that case.

How Much Sugar Your Teeth Can Handle

The World Health Organization recommends keeping free sugars below 10% of your total daily calories to reduce cavity risk, and ideally below 5% for even better protection. For an adult eating 2,000 calories a day, 5% works out to about 25 grams, or roughly 6 teaspoons. A single can of soda contains about 35 to 40 grams, already exceeding that entire daily target.

What counts as “free sugars” includes any sugar added to food or drinks, plus sugars naturally present in honey, syrups, and fruit juices. It doesn’t include the sugar locked inside whole fruits or the lactose in plain milk, because those are released more slowly and don’t produce the same sharp acid spikes.

Reducing Sugar Pain Day to Day

If sugar is causing you pain right now, the practical question is how to limit the damage while you address the underlying problem. Eating sugary foods with meals rather than as standalone snacks reduces the number of separate acid attacks your teeth face, because your mouth is already producing extra saliva to handle the meal. Rinsing with plain water after something sweet helps dilute acid faster. Waiting about 30 minutes before brushing is important, because brushing while enamel is softened from an acid attack can actually scrub away more mineral than it protects.

Chewing sugar-free gum after eating stimulates saliva flow, which speeds up the delivery of calcium and phosphate back to your enamel. Fluoride toothpaste strengthens the mineral structure of teeth, making them more resistant to acid dissolution at low pH levels. These steps slow the cycle, but they can’t fully compensate for constant sugar exposure throughout the day.

If a specific tooth consistently hurts when you eat something sweet, that tooth has lost enough of its protective structure to expose the sensitive layer underneath. The pain is a signal that damage has already occurred, whether from a cavity, a crack, a worn filling, or gum recession that’s left the root surface exposed. The sooner that gets addressed, the better the chance the nerve inside can be preserved.
