Why Does the Bottom of My Feet Burn: Causes and Fixes

Burning sensations on the soles of your feet usually come from nerve damage, a condition called peripheral neuropathy. Diabetes is the single most common cause, but the list of possibilities ranges from simple fungal infections to vitamin deficiencies, kidney disease, and even the shoes you’re wearing. The burning can feel mild and occasional or constant and intense enough to disrupt sleep, and figuring out the underlying cause is the key to making it stop.

Peripheral Neuropathy: The Most Common Cause

Peripheral neuropathy happens when something damages the sensory nerves that run from your spinal cord out to your arms and legs. Because these nerves are longest in your legs and feet, they tend to be affected first. The damaged nerves misfire, sending pain signals to your brain even when there’s nothing actually hurting your skin. People typically describe the sensation as burning, tingling, or stabbing.

Many different conditions can trigger this nerve damage: autoimmune diseases like lupus and rheumatoid arthritis, exposure to toxic chemicals, certain chemotherapy drugs (particularly taxanes and platinum-based agents), inherited neurological disorders, and chronic infections. Even prolonged pressure on a nerve, as in tarsal tunnel syndrome, where the main nerve running through your ankle gets compressed, can produce burning and tingling along the bottom of your foot.

Diabetes and Blood Sugar Damage

Diabetes deserves its own discussion because it’s by far the leading driver of burning feet. Persistently high blood sugar does two things to your nerves. First, it directly damages nerve fibers and disrupts their ability to transmit signals properly. Second, it weakens the walls of the tiny blood vessels (capillaries) that deliver oxygen and nutrients to those nerves. Over time, the nerves starve and deteriorate.

Diabetic neuropathy develops gradually and tends to get worse over time if blood sugar stays poorly controlled. It almost always starts in the feet and may eventually move upward into the legs and hands. If you have diabetes and notice burning in your soles, especially at night, that’s a signal your blood sugar management needs attention. Tighter glucose control can slow or even halt further nerve damage, though nerves that are already injured may not fully recover.

Vitamin Deficiencies and Alcohol Use

Your nerves need B vitamins, particularly B12, to maintain their protective outer coating. When you’re deficient, that coating breaks down and the exposed nerves start misfiring. B12 deficiency is common in older adults, people who follow strict plant-based diets, and anyone with absorption problems in their gut.

Heavy alcohol use causes burning feet through a double hit. Alcohol itself appears to be directly toxic to nerve tissue. At the same time, chronic alcohol use often leads to poor nutrition and difficulty absorbing B vitamins, compounding the damage. The result is a burning, tingling sensation that typically starts in the feet and can progress to the hands.

Kidney Disease and Toxin Buildup

When your kidneys can’t filter waste properly, medium-sized toxic molecules accumulate in your blood. These toxins are believed to damage nerves by disrupting the electrical charge across nerve cell membranes, essentially depolarizing them so they can no longer function normally. The resulting neuropathy, called uremic neuropathy, affects the legs more than the arms and produces numbness, burning, and weakness that starts at the feet and works its way up.

Uremic neuropathy typically shows up in people with end-stage kidney disease. The encouraging finding is that it improves with dialysis and can resolve after a kidney transplant, confirming that the problem is the buildup of filterable toxins rather than permanent structural damage to the nerves.

Fungal Infections and Skin Conditions

Not every case of burning feet involves nerve damage. Athlete’s foot, a fungal infection of the skin, can cause a burning or stinging sensation along with cracked, peeling skin between your toes and on your soles. This is one of the most treatable causes. Over-the-counter antifungal creams typically clear it up within a few weeks, and keeping your feet dry and changing socks regularly helps prevent it from coming back.

Contact dermatitis from materials in your shoes, socks, or laundry detergent can also produce burning. If the sensation appeared after you switched footwear or detergent brands, that’s worth investigating before assuming something more serious.

Erythromelalgia: A Rarer Possibility

If your feet turn visibly red and hot during burning episodes, you may be dealing with erythromelalgia. This condition causes flare-ups of pain, redness, and swelling, primarily in the hands and feet. Episodes are triggered by anything that raises body temperature: exercise, entering a warm room, wearing thick socks or tight shoes, and sometimes eating spicy food or drinking alcohol. In some people, the flares are so painful that wearing shoes or walking becomes difficult. Over time, the redness can become constant and spread from the feet up into the legs.

How Doctors Find the Cause

Because so many conditions can produce burning feet, diagnosis usually involves a combination of approaches. Your doctor will start with your medical history and a physical exam. A simple blood test can check for diabetes, kidney problems, and vitamin deficiencies, which together account for the majority of cases.

If the cause isn’t obvious from blood work, nerve function tests may be the next step. An electromyogram (EMG) measures how well your nerves conduct electrical signals and can pinpoint where damage is occurring. In some cases, an MRI of the ankle or foot can reveal whether a nerve is being physically compressed, as in tarsal tunnel syndrome, where a doctor may also tap the tibial nerve near your ankle to see if it reproduces your symptoms. For small-fiber neuropathy, where standard nerve tests come back normal, a small skin biopsy can reveal damage to the finest nerve endings in your skin.

Managing the Burning

Treatment depends entirely on what’s causing the problem. When diabetes is the culprit, bringing blood sugar into a healthy range is the most important step. If a vitamin deficiency is involved, supplementation can help nerves repair over time. Alcohol-related neuropathy requires stopping or significantly reducing alcohol intake alongside nutritional support. Kidney-related nerve damage improves when dialysis removes the accumulated toxins.

For the burning sensation itself, several types of medications can calm overactive nerves. Current treatment guidelines give the strongest recommendation to three drug classes that work by changing how your nervous system processes pain signals: certain older antidepressants, medications originally developed for seizures, and a newer category of antidepressants that boost two specific brain chemicals involved in pain modulation. These don’t cure the underlying nerve damage, but they can significantly reduce the burning.

For immediate relief at home, soaking your feet in cool (not ice-cold) water can temporarily quiet the burning. Wearing breathable shoes and moisture-wicking socks reduces heat buildup. Elevating your feet and avoiding long periods of standing may also help. If a fungal infection is involved, keeping feet clean and dry while using an antifungal treatment addresses the root issue directly.

Patterns Worth Paying Attention To

Burning that only happens after long periods on your feet and goes away with rest is more likely mechanical, related to footwear, foot structure, or standing habits. Burning that worsens at night and wakes you up is more characteristic of neuropathy. Burning accompanied by visible redness and heat that flares with warmth suggests erythromelalgia. And burning with cracked, peeling skin points toward a fungal infection.

Progressive burning that starts in both feet and gradually moves upward, or burning paired with numbness, weakness, or balance problems, suggests the nerve damage is advancing and warrants evaluation sooner rather than later. Early treatment of the underlying cause gives nerves the best chance of recovery or, at minimum, prevents further deterioration.
