Why Does Wine Give Me a Headache: Causes and Fixes

Wine triggers headaches in a surprising number of people, with research suggesting up to one-third of people experience a pulsing, pounding headache after drinking red wine, sometimes within 30 minutes of their first glass. The cause isn’t one single substance. Several compounds in wine work together (and against your body’s natural defenses) to produce that familiar pain. The answer also depends on whether your headache hits quickly or shows up the next morning, because different mechanisms are at play in each case.

How Alcohol Itself Causes Head Pain

Before getting into what makes wine special, it helps to understand what alcohol does to your head on its own. Ethanol directly stimulates pain-sensing nerve fibers in the membranes surrounding your brain and triggers inflammation in the trigeminal system, a network of nerves responsible for sensation in your face and head. This inflammation causes blood vessels in those membranes to widen, which is a well-established trigger for headache pain.

The process works through a specific chain of events: ethanol activates a receptor on nerve endings called TRPV1, the same receptor that responds to chili peppers and heat. Once activated, it allows calcium to rush into the nerve cells, which then release inflammatory signaling molecules. These molecules cause the blood vessels in and around your brain to dilate. That vascular swelling presses on surrounding tissue and creates the throbbing sensation you feel.

Your body also has to break alcohol down, and the intermediate step produces a toxic compound called acetaldehyde. Normally, acetaldehyde is short-lived because a second enzyme quickly converts it into harmless acetate. But if that second enzyme is slow, overwhelmed, or partially blocked, acetaldehyde lingers in your bloodstream. It’s a known irritant that contributes to nausea, flushing, and headache. Anything that interferes with this cleanup process makes the problem worse, and several compounds in wine do exactly that.

Why Red Wine Is Worse Than White

Red wine is made from the whole grape, including the skin, seeds, and sometimes stems. White wine is primarily made from just the juice. That difference in production means red wine carries far higher concentrations of several headache-triggering compounds.

Histamine

Histamine, the same compound your body releases during an allergic reaction, is found in grape skins. Red wine contains significantly more histamine than white wine because of its prolonged contact with those skins during fermentation. Your small intestine normally produces an enzyme that breaks histamine down before it enters your bloodstream, but some people naturally produce less of this enzyme. Alcohol itself also inhibits the enzyme. So when you drink red wine, you’re getting a double hit: a large dose of histamine and a reduced ability to process it. Elevated histamine dilates blood vessels in your head and can trigger headache pain directly.

Tannins

Tannins are the compounds that give red wine its dry, mouth-puckering quality. They come from grape skins and seeds and are present in much higher amounts in red wine due to longer contact time during fermentation. Research has shown that tannins trigger shifts in serotonin levels in the brain. These serotonin fluctuations are a known pathway for migraine-like symptoms, which explains why some people describe their wine headache as similar to a migraine, with throbbing pain, sensitivity to light, or nausea.

Congeners

Congeners are byproducts of fermentation other than ethanol itself, and they include small amounts of methanol and various flavor compounds. Red wine contains notably higher concentrations of methanol than beer or clear spirits like vodka. Congeners have long been suspected of worsening hangover symptoms, though one study measuring urine methanol levels found no direct correlation between methanol concentration and hangover severity. Their role may be more about how your liver prioritizes breaking them down, temporarily slowing the processing of ethanol and its toxic byproducts.

The Quercetin Connection

A newer line of research points to quercetin, a natural flavonoid found in grape skins that’s concentrated by sun exposure. When your body metabolizes quercetin, it converts it into a form that appears to interfere with the enzyme responsible for clearing acetaldehyde from your blood. This means even a small amount of wine could cause acetaldehyde to build up faster than your body can handle it, producing headache, flushing, and nausea in susceptible people. Grapes that get more sun exposure during growing tend to have higher quercetin levels, which may explain why certain wines are more likely to cause headaches than others, even within the same varietal.

Sulfites Are Probably Not the Cause

Sulfites are the most commonly blamed culprit, but the evidence doesn’t support it. True sulfite sensitivity affects roughly 4% to 5% of people with asthma, and its symptoms are respiratory: wheezing, shortness of breath, chest tightness, coughing, and nasal congestion. Headache is not a clinically recognized symptom of sulfite sensitivity. White wine actually contains more added sulfites than red wine in most cases, yet red wine is far more likely to trigger headaches. Dried fruits like apricots contain sulfite levels many times higher than wine, and people who get wine headaches rarely report the same problem after eating dried fruit.

Quick-Onset vs. Next-Day Headaches

The timing of your headache offers clues about what’s causing it. A headache that starts within 30 minutes to two hours of drinking is likely driven by histamine, tannins, quercetin metabolites, or the direct effect of ethanol on your trigeminal nerve fibers. These are sometimes called “red wine headaches” specifically because they happen too fast to be a standard hangover.

A headache the next morning is more of a classic hangover response. It involves dehydration, lingering acetaldehyde, disrupted sleep, and the inflammatory aftereffects of alcohol metabolism. Congeners likely play a larger role here. Both types involve vasodilation and inflammation, but the quick-onset version suggests your body is reacting to something specific in the wine beyond the alcohol content alone.

How to Reduce Wine Headaches

If wine consistently gives you headaches but you’d rather not give it up entirely, a few strategies can help. Drinking a full glass of water between each glass of wine slows your alcohol intake and counters the dehydration that amplifies headache symptoms. Eating a substantial meal before or while drinking slows alcohol absorption, giving your liver more time to process acetaldehyde before it accumulates.

Switching from red to white wine eliminates most of the histamine, tannin, and quercetin exposure. If you prefer red, choosing wines from cooler climates or shaded vineyards may mean lower quercetin levels, though this isn’t something typically listed on a label. Some people find that wines with lower tannin levels, such as Pinot Noir compared to Cabernet Sauvignon, are easier on their heads.

Drinking slowly matters more than most people realize. Your liver processes roughly one standard drink per hour. Anything faster than that creates a backlog of acetaldehyde and amplifies every other headache trigger in the wine. If one glass over dinner causes no problems but a second glass within the same hour brings on pain, pace is likely the critical variable for you.

Some people take an antihistamine before drinking wine, targeting the histamine pathway specifically. This can help if histamine is your primary trigger, but it won’t address tannin-related serotonin changes or quercetin’s effect on acetaldehyde metabolism. Paying attention to which wines cause problems and which don’t can help you narrow down your personal trigger over time.
