A sore throat happens when tissues in your pharynx become inflamed, triggering pain nerve endings that make every swallow uncomfortable. The cause is usually a virus, but everything from dry winter air to stomach acid can set off the same painful response. Understanding what’s behind the soreness helps you figure out whether you need to ride it out, change something in your environment, or get tested for a bacterial infection.
What Happens Inside Your Throat
When a virus or irritant reaches the lining of your throat, your immune system launches an inflammatory response. White blood cells flood the area, and your body releases chemical messengers, including one called bradykinin, that directly stimulate pain nerve endings in the tissue. Blood vessels in the area dilate and leak fluid, which causes the swelling you can feel when you press on your neck or try to swallow. That swelling puts even more pressure on pain receptors, creating a cycle of inflammation and soreness that persists until the underlying trigger is resolved.
This process is essentially the same regardless of whether the trigger is a cold virus, bacteria, or an allergen. The difference lies in how intense the response is and how long it lasts.
Viral Infections: The Most Common Cause
Viruses account for 50% to 80% of all acute sore throats. Cold and flu viruses are the usual culprits, but respiratory syncytial virus, adenovirus, and the Epstein-Barr virus (which causes mono) can all inflame the throat. A viral sore throat typically arrives alongside other cold symptoms: a runny nose, coughing, sneezing, red or watery eyes, and mild body aches.
Most viral sore throats resolve within three to ten days without any specific treatment. Your body clears the virus on its own, and the inflammation gradually fades. Pain relievers, warm liquids, and throat lozenges can make those days more bearable, but they don’t speed up recovery.
Strep Throat Feels Different
Bacteria cause roughly 20% to 50% of sore throats, and the one that matters most is group A streptococcus, responsible for 5% to 15% of sore throats in adults and up to 30% in children. Strep throat has a distinct pattern: it tends to come on suddenly and is not accompanied by coughing or sneezing. Instead, you’re more likely to notice a fever, swollen lymph nodes in your neck, headache, loss of appetite, and belly pain (especially in kids).
If you look in the mirror, red and swollen tonsils with white patches are a strong visual clue pointing toward strep. A rapid strep test or throat culture confirms the diagnosis. Strep needs to be treated with antibiotics, typically a 10-day course, because untreated strep can lead to complications affecting the heart and kidneys. A useful rule of thumb doctors use: if you have a fever above 100.4°F, swollen neck lymph nodes, white patches on your tonsils, and no cough, the odds of strep are high enough to warrant testing.
Dry Air and Mouth Breathing
Not every sore throat involves an infection. Dry air is a surprisingly common offender, especially during winter when indoor heating strips moisture from the air. Your throat depends on a thin layer of mucus to stay comfortable and protected. When humidity drops, that mucus layer thins out, leaving your throat scratchy and inflamed. Keeping indoor humidity between 30% and 50% helps prevent this.
Mouth breathing at night produces the same effect. If you wake up with a raw throat that improves after drinking water and moving around for an hour, dry air or mouth breathing during sleep is the likely explanation rather than an infection. Nasal congestion from allergies or a deviated septum often forces mouth breathing, so addressing the nasal issue can fix the recurring morning sore throat.
Acid Reflux You Might Not Feel
Stomach acid can reach your throat without causing the classic heartburn sensation. This condition, called laryngopharyngeal reflux (or “silent reflux”), happens when stomach contents travel all the way up to the larynx and pharynx. The damage comes from a combination of acid, digestive enzymes, and bile salts making direct contact with throat tissue that has no natural protection against them.
What makes silent reflux especially tricky is that one of those digestive enzymes, pepsin, can linger in throat tissue and reactivate later when the local pH shifts. This means a single reflux episode can cause irritation that continues well after the acid itself is gone. People with silent reflux often describe a chronic, low-grade sore throat, a feeling of a lump in the throat, or frequent throat clearing. If your sore throat keeps coming back without cold symptoms, reflux is worth considering.
Post-Nasal Drip and Allergies
Allergies to pollen, dust mites, mold, or pet dander trigger excess mucus production in the sinuses. That mucus drains down the back of your throat, a process called post-nasal drip. The soreness comes from several overlapping factors: the sheer volume of mucus irritating the tissue, the thicker consistency of allergy-related mucus compared to normal secretions, and the underlying inflammation that makes the throat lining more sensitive to any irritation at all. Impaired ciliary function, where the tiny hair-like structures lining your airways can’t clear mucus efficiently, makes the problem worse.
Allergy-related sore throats tend to be persistent and seasonal, often worse in the morning after mucus has pooled overnight. They come with itchy eyes, sneezing, and nasal congestion rather than fever or body aches.
Other Everyday Triggers
Several non-infectious irritants can inflame your throat in the same way a virus does:
- Cigarette smoke and air pollution deliver chemical irritants directly to the throat lining, triggering chronic low-level inflammation.
- Vocal strain from shouting, singing, or prolonged talking inflames the larynx and surrounding tissue.
- Dehydration thins the protective mucus layer, leaving tissue exposed and vulnerable.
These causes tend to produce a milder, more persistent soreness rather than the acute pain of an infection. They also respond quickly to removing the irritant, unlike viral sore throats that need time to run their course.
How Long a Sore Throat Should Last
Most sore throats clear up within three to ten days. If yours lasts longer than a week, gets progressively worse instead of better, or arrives with a fever over 100.4°F, those are signals to get it evaluated. Certain symptoms call for prompt medical attention: difficulty breathing, trouble swallowing, blood in your saliva or phlegm, excessive drooling in young children, a visible bulge in the back of the throat, a rash anywhere on your body, or joint swelling and pain. These can indicate complications like a peritonsillar abscess or, rarely, a more serious inflammatory reaction that needs treatment beyond rest and fluids.
