Novartis held key patents on Entresto, including one that didn’t expire until January 2025 after a patent term extension. When generic manufacturers like MSN and Torrent Pharma filed applications with the FDA to sell cheaper versions, Novartis took them to court. These patent disputes wound through the federal appeals system, delaying generic entry and preserving Novartis’s pricing power for years. Without a generic on pharmacy shelves, there was no market pressure to lower the price.

The Scale of Clinical Investment

Bringing a new drug class to market is extraordinarily expensive, and pharmaceutical companies build those costs into the price. The PARADIGM-HF trial alone enrolled over 8,000 patients across multiple countries and ran long enough to measure differences in death rates, not just symptoms. Novartis then conducted additional trials in different patient populations, including studies on starting Entresto in the hospital after an acute heart failure episode. Each of these trials required thousands of patients, years of follow-up, and regulatory submissions in dozens of countries. While Novartis has not disclosed a specific R&D figure for Entresto, the scope of this clinical program was among the largest in cardiovascular medicine.

Medicare Negotiation Cuts the Price in Half

Entresto was one of the first ten drugs selected for price negotiation under the Inflation Reduction Act’s Medicare Drug Price Negotiation Program. The results were significant. Against a 2023 list price of $628 for a 30-day supply, the negotiated price came in at $295, a 53% discount. This new price takes effect January 1, 2026, and applies to Medicare Part D enrollees.

The scale of Medicare spending on Entresto helps explain why it was targeted. In 2023, total Part D costs for the drug exceeded $3.4 billion, with roughly 664,000 Medicare enrollees filling prescriptions. That combination of high per-patient cost and large patient volume made it a priority for negotiation. What Independent Analysts Say About Fair Pricing

ICER concluded that the drug’s wholesale acquisition cost of $4,560 per year would need to drop to about $4,168, roughly a 9% discount, to avoid placing an excessive cost burden on the health care system. That relatively modest recommended discount reflects the fact that Entresto genuinely reduces hospitalizations and deaths. In other words, even independent analysts acknowledged the drug delivers substantial value. The problem for patients has always been that value-based pricing at the system level doesn’t help much when you’re staring at a $700 monthly pharmacy bill.

Ways to Lower Your Cost Now

If you’re uninsured or on government insurance, the Novartis Patient Assistance Foundation provides Entresto at no cost to eligible patients. It’s a nonprofit program, separate from Novartis’s commercial operations. To qualify, you need to live in the United States, be treated by a licensed provider on an outpatient basis, and meet income guidelines. The application requires proof of income and, in some cases, evidence that you were denied Medicare’s Extra Help program. You or a legal guardian must submit the application directly. Expect a decision within about four weeks.

For patients with commercial insurance, Novartis offers copay assistance cards that can reduce monthly out-of-pocket costs significantly. Your pharmacist or cardiologist’s office can typically point you to the right program. It’s also worth asking your pharmacist to compare prices across pharmacies, since cash prices for Entresto can vary by hundreds of dollars depending on where you fill the prescription.

The Bigger Picture on Heart Failure Drug Costs

Heart failure is one of the most common reasons for hospital admission in the United States, and each hospitalization can cost tens of thousands of dollars. Entresto’s ability to reduce those admissions is part of why insurers generally cover it despite the high sticker price. For the health care system as a whole, paying $600 a month for a drug that prevents a $30,000 hospital stay can be a net savings. That math doesn’t make the pharmacy counter any less painful, but it explains why payers have been willing to keep the drug on formularies rather than forcing patients onto cheaper, less effective alternatives.

With the Medicare negotiated price arriving in 2026 and generic applications working through the FDA, the cost picture for Entresto is likely to look very different within the next few years.
