A 3-week-old who seems to fight sleep or wake constantly is, in most cases, behaving exactly as biology intended. At this age, babies sleep around 16 hours a day, but that sleep comes in short, unpredictable bursts spread across day and night. Your baby isn’t broken. Their brain simply hasn’t developed the internal clock that distinguishes daytime from nighttime, and their tiny stomach empties fast enough to wake them every couple of hours.
That said, there are specific reasons some 3-week-olds seem to sleep even less than expected, and understanding them can help you troubleshoot what’s going on.
Their Brain Can’t Tell Day From Night Yet
The hormone that drives nighttime sleepiness, melatonin, isn’t produced in a reliable rhythm until a baby is 9 to 12 weeks old. Before that point, there’s little evidence of any consistent day-night pattern in melatonin output. At 3 weeks, your baby’s brain simply doesn’t have the biological machinery to consolidate sleep into longer nighttime stretches. This is what pediatricians call “day-night confusion,” and it’s the single biggest reason new parents feel like their baby never sleeps.
What this looks like in practice: your baby may sleep for a 3-hour stretch in the middle of the afternoon, then be wide awake and fussy at 2 a.m. This isn’t a habit problem or something you caused. It resolves on its own as the brain matures, typically between 6 and 12 weeks. You can nudge the process along by exposing your baby to natural light during the day and keeping nighttime feeds dim and quiet, but there’s no way to rush melatonin production.
Their Stomach Is Tiny
At 3 weeks old, a baby’s stomach holds about 2 to 3 ounces per feeding. That’s roughly the volume of a shot glass. Whether breastfed or formula-fed, this small capacity means your baby needs to eat at least 8 to 12 times in a 24-hour period. In practice, that works out to a feeding roughly every 2 to 3 hours around the clock.
If your baby seems to wake even more frequently than that, they may be going through a growth spurt. The 3-week mark is a classic time for one. During a growth spurt, babies feed more often (sometimes hourly) for a few days, which naturally fragments their sleep further. This is temporary and usually resolves within 2 to 4 days.
They’re Overtired
This one is counterintuitive: a baby who has been awake too long actually has a harder time falling asleep. At 3 weeks, a baby’s wake window (the amount of time they can comfortably stay awake between naps) is only 30 to 90 minutes. That includes feeding time. If your baby has been awake for an hour and a half, they’re likely already overtired.
An overtired newborn looks wired rather than sleepy. They may seem alert with wide eyes, fuss intensely, or fight being held. The stress hormones their body releases when overtired make it harder to settle into sleep, which creates a frustrating cycle: too tired to sleep, too awake to calm down. Watch for early sleep cues like yawning, turning away from stimulation, or making jerky movements, and start your soothing routine before full-blown crying begins.
Half Their Sleep Is Active Sleep
About half of a newborn’s total sleep time is spent in active sleep, the infant equivalent of REM sleep. During active sleep, babies twitch, grunt, grimace, flutter their eyelids, and sometimes cry out briefly. This can look exactly like waking up, and many parents respond by picking the baby up, feeding, or changing them, which actually does wake them.
Before you intervene when your baby stirs, pause for 30 to 60 seconds. If they’re in active sleep, they’ll often settle back down on their own. True waking usually involves sustained crying, open eyes, and rooting (turning their head and opening their mouth as if searching for a breast or bottle).
Gas, Reflux, and Physical Discomfort
If your baby seems to sleep fine when held upright but wakes the moment you lay them flat, reflux may be the issue. Signs of reflux in newborns include arching the back during or after feeding, gagging or difficulty swallowing, irritability right after eating, and frequent spitting up. Some babies have “silent” reflux where they don’t spit up visibly but still experience the discomfort of stomach acid moving upward, especially when lying flat.
Another common culprit at this age is infant dyschezia, a condition where babies strain, grunt, cry, and turn red while trying to poop, even though the poop itself is perfectly normal and soft when it comes out. This happens because newborns haven’t yet learned to coordinate relaxing their pelvic floor muscles while pushing with their abdomen. It looks alarming, and it can absolutely disrupt sleep, but it resolves on its own as the baby’s coordination matures. The key difference between dyschezia and colic: dyschezia is specifically tied to the act of pooping and stops once the baby passes stool. Colic involves inconsolable crying with no clear trigger.
If your baby cries for extended periods with clenched fists, a rigid body, and pulled-up legs but isn’t trying to poop, colic is a possibility. Colic is typically diagnosed when no other medical explanation fits, and it tends to peak around 6 weeks before gradually improving.
The Sleep Environment Matters
Newborns sleep best in a predictable, safe setup. The American Academy of Pediatrics recommends placing your baby on their back on a firm, flat mattress in their own sleep space, such as a crib, bassinet, or portable play yard. No blankets, pillows, stuffed animals, or bumpers. Room sharing (your baby sleeping in your room but not in your bed) is recommended for at least the first 6 months.
A few environmental factors that commonly disrupt newborn sleep: a room that’s too warm or too cold (aim for 68 to 72°F), sudden noises in a quiet room (white noise can help mask these), and light exposure during nighttime feeds. Swaddling can also help at this age by reducing the startle reflex, where a baby’s arms jerk outward and wake them up. Make sure the swaddle is snug around the arms but loose enough around the hips to allow leg movement.
What “Normal” Actually Looks Like at 3 Weeks
A realistic picture of normal newborn sleep: your baby sleeps 14 to 17 hours total in a 24-hour period, broken into chunks of 45 minutes to 3 hours, with no predictable schedule. They wake frequently to eat. They may have one slightly longer stretch of 3 to 4 hours, but it might fall at noon instead of midnight. They grunt, squirm, and make noise during sleep. They sometimes fight falling asleep even when exhausted.
What falls outside normal: a baby who is extremely difficult to wake for feedings, a baby who hasn’t regained their birth weight by 2 weeks, a baby who seems to be in pain (not just fussy) most of the time, or a baby with a rectal temperature above 100.4°F. These warrant a call to your pediatrician. But a 3-week-old who sleeps in short bursts and seems to “never sleep” is almost always doing exactly what a 3-week-old is supposed to do.
