Why Is My Cough Worse at Night — and How to Stop It

Your cough gets worse at night mainly because lying down changes how gravity acts on your body. During the day, mucus drains downward through your throat and sinuses without much trouble. When you lie flat, that drainage pools in your airways instead of clearing, and stomach acid can creep upward into your esophagus. On top of that, your body’s internal clock naturally narrows your airways in the early morning hours. Each of these factors can trigger coughing on its own, and they often overlap.

Lying Down Changes How Mucus Drains

When you’re upright, gravity helps mucus move steadily through your airways and down your throat. The moment you lie flat, that system stalls. Secretions pool on the lower sides of your airways, and the upper surfaces can actually dry out. This combination of pooled mucus below and dried-out tissue above irritates the lining of your airways and triggers the cough reflex.

If you have any degree of congestion from a cold, allergies, or a sinus infection, the effect is amplified. Mucus that was dripping harmlessly down the back of your throat all day now collects and sits against sensitive tissue. Your body’s response is to cough it out, which is why you may feel fine sitting on the couch but start hacking the moment your head hits the pillow.

Post-Nasal Drip Activates Cough Receptors

Post-nasal drip is one of the most common reasons for a nighttime cough, and the mechanism is surprisingly direct. Mucus draining from your sinuses physically touches cough receptors located in your throat and voice box. Those receptors respond to the mechanical contact itself, not just to inflammation or irritation. The signal travels along nerve fibers connected to the vagus nerve, which relays the message to your brainstem, and you cough.

During the day, you swallow frequently and breathe through your mouth when your nose is stuffy, which diverts much of that drainage before it can trigger a cough. At night, your swallowing rate drops dramatically, and mucus flows more freely into your throat. If you’re dealing with allergies, a sinus infection, or even just a lingering cold, this is often the primary culprit behind that persistent nighttime cough.

Acid Reflux Can Trigger Coughing Two Ways

Gastroesophageal reflux (commonly called acid reflux or GERD) causes nighttime coughing through two distinct pathways. The first doesn’t even require acid to reach your throat. When stomach contents irritate nerve endings in your lower esophagus, they activate a reflex arc through the vagus nerve that triggers coughing. Your esophagus and your airways share the same nerve wiring, so irritation in one can produce symptoms in the other.

The second pathway is more direct: tiny amounts of stomach acid travel all the way up through your esophagus and spill into your airway, a process called micro-aspiration. Lying flat makes this far more likely because gravity is no longer keeping acid in your stomach. The risk increases further if your protective throat reflexes are sluggish during sleep, which they naturally are. Many people with reflux-related cough don’t experience obvious heartburn, so this cause is easy to miss.

Your Body Clock Narrows Your Airways

Even without any illness, your lungs don’t perform the same way at 3 AM as they do at 3 PM. Research published in the Proceedings of the National Academy of Sciences confirmed that an internal circadian rhythm causes airway resistance to peak around 4:00 AM, independent of sleep itself. Lung function is at its worst during the biological night, roughly from 11:00 PM through 11:00 AM, with the lowest point near 5:00 AM.

Several hormonal shifts drive this pattern. Your body’s levels of adrenaline and related stress hormones drop to their lowest point in the middle of the night. These hormones normally relax and open your airways while also keeping certain immune cells from releasing histamine. When they fall, your airways tighten, and histamine release increases. At the same time, activity in the vagus nerve (which constricts airways) peaks during those same hours. For anyone with asthma or reactive airways, this creates a window of vulnerability that can produce coughing, wheezing, or both.

Dry or Overly Humid Air Makes It Worse

The air in your bedroom plays a bigger role than most people realize. Dry air pulls moisture from the mucus lining your airways, making it thicker and stickier. That thickened mucus is harder to clear and more likely to irritate cough receptors. Heated air in winter is a common offender, as furnaces and radiators can drop indoor humidity well below comfortable levels.

Too much humidity creates a different problem: it encourages mold and dust mite growth, both potent allergens that trigger inflammation and post-nasal drip. The Environmental Protection Agency recommends keeping indoor relative humidity between 30% and 50%, and never above 60%. A simple hygrometer (available for a few dollars at most hardware stores) can tell you where your bedroom falls. A humidifier helps in dry climates or during winter; a dehumidifier or air conditioner is more useful in humid environments.

Practical Ways to Reduce Nighttime Coughing

Elevate Your Upper Body

Propping yourself up changes the physics that cause most nighttime coughing. A wedge pillow angled at 30 to 45 degrees, elevating your head six to twelve inches, helps mucus drain forward rather than pooling in your airways. It also reduces acid reflux by keeping your stomach below your esophagus. Stacking regular pillows is less effective because they tend to bend you at the waist rather than creating a gradual slope, which can actually increase abdominal pressure and worsen reflux.

Try Honey Before Bed

Honey has genuine cough-suppressing properties, and it works well enough that a study published by the American Academy of Pediatrics tested it in a double-blind, placebo-controlled trial. Children who received about 10 grams of honey (roughly two teaspoons) 30 minutes before bedtime had measurable improvement in cough severity and sleep quality compared to the placebo group. The effect likely comes from honey coating and soothing irritated throat tissue. This works for adults too, though honey should never be given to children under one year old due to the risk of botulism.

Manage Your Specific Trigger

The most effective fix depends on what’s causing your cough. If post-nasal drip is the issue, a saline nasal rinse before bed clears mucus from your sinuses and reduces the volume of drainage reaching your throat overnight. If reflux is the culprit, avoid eating within two to three hours of bedtime and consider sleeping on your left side, which positions your stomach below your esophagus. If allergies are involved, keeping your bedroom free of dust, washing bedding weekly in hot water, and running an air purifier with a HEPA filter can reduce exposure to triggers while you sleep.

Over-the-counter cough suppressants have a mixed track record. Clinical guidelines from the American College of Chest Physicians found that common cough suppressants have limited effectiveness for coughs caused by upper respiratory infections, which are the most frequent reason people reach for them. They work better for coughs related to bronchitis. Expectorants, which are designed to thin mucus, have not been shown to suppress coughing either. For most people with a short-term nighttime cough, the non-medication strategies above are more reliably helpful.

Signs Your Nighttime Cough Needs Attention

Most nighttime coughs are annoying but harmless, caused by colds, allergies, or mild reflux. Certain symptoms, however, signal something more serious. Coughing up blood, unexplained weight loss, persistent fever, night sweats, or significant shortness of breath all warrant prompt evaluation. A cough lasting longer than eight weeks falls into the “chronic” category and deserves investigation even if it feels mild, since persistent coughs can point to undiagnosed asthma, reflux, or less common conditions. Thick, discolored sputum that persists for more than a week or two may indicate a bacterial infection rather than a simple viral illness.
