Why Is My Heart Rate Lower Than Normal: Causes & Signs

A resting heart rate below 60 beats per minute is technically called bradycardia, but it isn’t always a problem. For many people, a lower-than-expected heart rate is simply a sign of cardiovascular fitness or a side effect of medication they’re already taking. The key factor is whether you feel fine or whether the slow rate is causing symptoms like dizziness, fatigue, or fainting.

What Counts as a Low Heart Rate

The standard normal range for adults is 60 to 100 beats per minute (bpm) at rest. Anything below 60 bpm falls into the bradycardia category by the traditional definition. However, cardiologists and population studies often use a lower cutoff of 50 bpm before considering the rate clinically meaningful. The 2018 ACC/AHA guidelines define potential sinus node dysfunction as a rate below 50 bpm, not 60, recognizing that plenty of healthy people sit comfortably in the 50s.

Your heart rate also drops naturally while you sleep, typically running 20% to 30% lower than your daytime resting rate. So if your resting rate is 65 bpm during the day, seeing numbers in the mid-40s on a sleep tracker overnight is expected and not a sign of trouble.

Fitness Is the Most Common Reason

Regular exercise, especially endurance training like running, cycling, or swimming, physically remodels the heart’s pacemaker cells. Up to 80% of endurance athletes develop resting bradycardia. This was long attributed solely to stronger signals from the vagus nerve (the nerve that tells your heart to slow down), but newer research shows the heart’s internal pacemaker actually changes its baseline firing rate independent of nerve input. Mechanosensitive ion channels within the heart’s upper chambers appear to play a bigger role than previously thought, and genetics contribute as well.

The numbers can get surprisingly low. In a study of 465 endurance athletes, 38% had a minimum heart rate at or below 40 bpm on a 24-hour monitor, and 2% dropped to 30 bpm or lower. One in four had pauses of two seconds or more between beats. These findings were well tolerated, meaning the athletes had no symptoms and needed no treatment. If you exercise regularly and your resting heart rate sits in the 40s or 50s without symptoms, your heart is likely just efficient at its job.

Medications That Slow Your Heart

Several common drug classes lower heart rate as either their primary purpose or a side effect. Beta-blockers are the most frequent culprit. They’re prescribed for high blood pressure, anxiety, migraines, and heart conditions, and slowing the heart rate is exactly how they work. Calcium channel blockers, another blood pressure medication, can do the same. Other medications linked to a slower heart rate include certain antiarrhythmia drugs, digoxin (used for heart failure), opioid pain medications, some older antidepressants, and lithium.

If you recently started a new medication or had a dose change and noticed your heart rate dropping, that’s a likely explanation. Don’t stop the medication on your own, but it’s worth bringing up with your prescriber, especially if you’re also feeling lightheaded or unusually tired.

Thyroid and Other Medical Causes

An underactive thyroid (hypothyroidism) is one of the most common medical conditions that slows the heart. Thyroid hormones directly regulate the electrical activity of heart cells, controlling how sodium, potassium, and calcium move across cell membranes. When thyroid levels are low, the heart’s contractile machinery and electrical signaling both become sluggish. Other signs of hypothyroidism include cold intolerance, fatigue, weight gain, and dry skin, so a slow heart rate rarely appears in isolation.

Electrolyte imbalances can also affect heart rate. High potassium levels (hyperkalemia), which can result from kidney problems or certain medications, slow the heart’s electrical conduction. Heart attacks or reduced blood flow to the heart can damage the pacemaker cells or the electrical pathways that keep the heart beating at a normal pace. These are less common causes but more urgent ones.

Symptoms That Signal a Problem

A low heart rate only becomes a medical concern when it prevents your brain and organs from getting enough blood. The symptoms to watch for are:

  • Dizziness or lightheadedness, especially when standing
  • Fainting or near-fainting
  • Unusual fatigue, particularly during physical activity
  • Shortness of breath
  • Chest pain
  • Confusion or memory problems

If you have none of these symptoms, a heart rate in the 40s or 50s is rarely dangerous. The clinical guideline is straightforward: treatment for a slow heart rate is almost never considered unless symptoms are present. Fainting, difficulty breathing, or chest pain lasting more than a few minutes warrants emergency care.

How a Low Heart Rate Is Evaluated

The primary tool is an electrocardiogram (ECG), which records your heart’s electrical activity through sensors placed on your chest. It can reveal whether the slow rate is coming from a normal pacemaker rhythm or from a block somewhere in the heart’s wiring. Because a slow heart rate can come and go, a standard ECG done in a clinic might miss it entirely. In that case, your doctor may have you wear a Holter monitor, a portable device that tracks your heart rhythm continuously for a day or more while you go about your normal routine.

If fainting is your main symptom, a tilt table test can help. You lie flat on a table while your heart rate and blood pressure are monitored, then the table tilts upward to simulate standing. This reveals how your heart and nervous system respond to position changes and can pinpoint whether a slow heart rate is behind the fainting episodes. Blood work to check thyroid function and electrolyte levels is also standard when investigating an unexplained drop in heart rate.

What Happens if Treatment Is Needed

For most people who find this article, the answer is reassuring: no treatment is needed. If you’re physically active, taking a heart-rate-lowering medication, or simply running on the lower end of normal, your body is likely functioning well.

When symptoms are present and linked to the slow rate, the first step is addressing any reversible cause. That might mean adjusting a medication dose, treating hypothyroidism, or correcting an electrolyte imbalance. If no reversible cause exists and symptoms are significant, a pacemaker is the standard long-term solution. Modern pacemakers are small devices implanted under the skin near the collarbone that monitor the heart’s rhythm and deliver a tiny electrical impulse only when the heart rate drops too low. The procedure is minimally invasive, and most people return to normal activity within a few weeks.
