A scalp that always feels sore, tender, or painful to the touch is surprisingly common, and it usually points to one of a handful of causes: inflammation from a skin condition, nerve sensitivity tied to migraines or tension, irritation from hair products, or mechanical stress from tight hairstyles. The medical term for this type of chronic scalp pain is trichodynia, and studies show it affects anywhere from 26% to 74% of people experiencing hair thinning or hair loss, depending on the type. But you don’t need to be losing hair for your scalp to hurt. Here’s what could be going on and what you can do about it.
Skin Conditions That Inflame the Scalp
Two of the most common culprits behind ongoing scalp soreness are seborrheic dermatitis and scalp psoriasis. Both cause inflamed, scaly patches that can burn, itch, and feel tender. They look similar, but there are differences. Psoriasis scales tend to be thicker and drier, and the patches often extend past the hairline onto the forehead or behind the ears. If you also notice changes on your elbows, knees, lower back, or fingernails (small pits or ridges), psoriasis is more likely. Seborrheic dermatitis, on the other hand, stays on the scalp and produces oilier, flakier patches. It’s essentially a more intense version of dandruff.
Folliculitis is another possibility. This happens when hair follicles get infected, usually by bacteria, and it shows up as clusters of small red bumps or pus-filled blisters around the follicles. The skin feels painful and tender, and it can burn or itch. Bacteria are the most common cause, but yeast infections and even ingrown hairs can trigger it. If you use hot tubs or heated pools regularly, bacteria that thrive in improperly treated water can cause a specific type of folliculitis on the scalp and body.
Migraines and Nerve Sensitivity
If your scalp soreness comes and goes with headaches, the cause may be neurological rather than dermatological. Between 40% and 70% of people with migraines experience something called allodynia during an attack. This is a pain-processing glitch where your nervous system becomes so sensitized that normally painless sensations, like brushing your hair, resting your head on a pillow, or even a light breeze, register as pain.
People with allodynia often describe it by saying “my hair hurts.” It comes in three forms: pain from light touch on the skin, pain triggered by movement across the skin (like running a brush through your hair), and pain from mild temperature changes. The underlying problem is central sensitization, where the nerves carrying pain signals overreact to inputs that shouldn’t trigger them. This isn’t limited to migraines. It also occurs in conditions like fibromyalgia and chronic tension headaches, which could explain why your scalp feels perpetually tender even when you don’t have a visible rash or bumps.
Your Hair Products May Be the Problem
Contact dermatitis from shampoos, conditioners, and styling products is an underrecognized source of chronic scalp pain. The hallmark symptoms are a burning sensation on the scalp, itching, and eczema-like patches. At least 1% of adults have a fragrance allergy, and fragrances are in nearly every hair product on the shelf. Beyond fragrance, the most common allergens in hair care include preservatives (especially formaldehyde-releasing chemicals like imidazolidinyl urea and quaternium-15), cocamidopropyl betaine (a foaming agent), and hair dye ingredients, particularly a compound called PPD that’s the leading cause of allergic reactions to hair color.
Even basic shampoo ingredients can irritate sensitive scalps. Sodium lauryl sulfate, a common detergent and foaming agent, is a recognized skin irritant. Its use has declined in recent years, but closely related surfactants like sodium laureth sulfate remain widespread in commercial shampoos. If you’ve recently switched products and your scalp started burning, that’s a strong clue. But even long-term use of the same product can eventually trigger sensitization, meaning your immune system decides to react to an ingredient it previously tolerated.
If you use topical minoxidil for hair growth, the soreness may not come from the active ingredient itself. The solvents in the formula, particularly propylene glycol, are a common trigger for contact dermatitis.
Tight Hairstyles and Physical Strain
Ponytails, braids, buns, and extensions pull on the soft tissue around the scalp, and that sustained traction causes real pain. The International Classification of Headache Disorders formally recognizes this as “external-traction headache” (previously called ponytail headache). The pain is strongest at the site of pulling but often spreads to other areas of the head. Importantly, it typically resolves within an hour of letting your hair down.
If you wear tight styles daily, the soreness can feel constant. Over time, chronic traction doesn’t just hurt; it can damage hair follicles and lead to traction alopecia, a form of hair loss along the hairline or wherever the pull is strongest. Loosening your style or alternating between updos and loose hair gives the follicles and surrounding nerves a chance to recover.
Scalp Soreness and Hair Loss
There’s a strong connection between scalp pain and hair shedding. In telogen effluvium, a type of diffuse hair loss triggered by stress, illness, or hormonal shifts, between 40% and 74% of patients report scalp pain. In androgenetic alopecia (pattern hair loss), about 26% to 32% experience it. The inflammation around hair follicles appears to drive both the pain and the shedding simultaneously.
The more concerning link is with scarring alopecia, a group of conditions where inflammation destroys hair follicles and replaces them with scar tissue. About 56% of people with scarring alopecia report scalp pain, along with burning, tingling, or tenderness. Because the follicle destruction is permanent, early detection matters. If your scalp soreness comes with noticeable hair thinning, especially in patches, and the skin in those areas looks smooth or shiny (no visible follicle openings), that warrants a professional evaluation. Once scarring alopecia reaches its end stage, treatment becomes much less effective because the active inflammation has already done its damage.
Even chemotherapy-related hair loss is closely tied to scalp discomfort. In one study of breast cancer patients undergoing chemotherapy, 100% reported unpleasant scalp sensations, with 87% experiencing both itching and pain.
What Helps at Home
The right approach depends on the cause, but several strategies provide relief across the board. Gentle scalp massage with your fingertips in circular motions can ease tension-related soreness and improve blood flow. Applying a cold pack or warm compress for 10 minutes at a time helps with both inflammatory pain and nerve-related tenderness. If you’ve been wearing your hair up all day, let it down slowly rather than yanking out the elastic.
For product-related irritation, simplify your routine. Switch to a fragrance-free, sulfate-free shampoo and stop using any styling products for a few weeks to see if the soreness improves. Reintroduce products one at a time so you can identify which one triggers symptoms. If you color your hair and suspect the dye, a patch test before your next application can confirm a reaction.
Stretching and relaxation techniques help when the soreness is linked to tension headaches or stress. Neck and shoulder tension often refers pain upward into the scalp, so loosening those muscles can make a real difference.
Signs That Need a Closer Look
Scalp soreness alone is usually manageable, but certain combinations of symptoms point to something that needs professional attention. Hair loss in patches, especially when the skin looks scarred or the follicle openings have disappeared, suggests scarring alopecia. Pus-filled bumps that keep coming back could mean a bacterial or fungal infection that needs targeted treatment. Scalp pain that always coincides with headaches, especially if it gets worse over time, may indicate a migraine pattern that responds well to preventive treatment. And burning or redness that started after switching hair products but hasn’t resolved after a few weeks of avoidance may require patch testing to identify the specific allergen.
