Why Is One of My Teeth Sensitive? Causes and Fixes

A single sensitive tooth almost always means something has exposed the inner layer of that tooth, called dentin, or irritated the nerve inside it. Unlike generalized sensitivity across many teeth, pain in one specific tooth points to a localized cause: a patch of worn enamel, a receding gumline on that side, a tiny crack, early decay, or even a recent dental procedure. The good news is that most of these causes are treatable, and many resolve on their own or with simple interventions.

What Happens Inside a Sensitive Tooth

Your tooth’s outer shell, the enamel, is designed to insulate the softer layer underneath called dentin. Dentin is filled with microscopic tubes that run from the outer surface straight to the nerve at the center of your tooth. When enamel wears away or the gum pulls back to expose a root surface, those tiny tubes become open pathways.

The fluid inside these tubes expands when it contacts something hot and contracts when it contacts something cold. That movement triggers pressure-sensitive nerve receptors, which is why you feel a sharp zing when you sip ice water or bite into something sweet. The thermal expansion of the fluid inside each tube is roughly ten times greater than the expansion of the tube wall itself, so even small temperature changes create enough movement to fire the nerve.

The Most Likely Causes

Enamel Wear

Brushing too hard, using a stiff-bristled brush, or choosing an overly abrasive toothpaste can thin the enamel on a single tooth, especially one that sticks out slightly in the arch or sits in a spot where your brush hits hardest. Acidic foods, beverages like citrus juice or soda, acid reflux, and conditions that bring stomach acid into the mouth (like bulimia) also dissolve enamel over time. If the erosion happens unevenly, one tooth loses its protective layer before the others.

Gum Recession on One Tooth

Your gums can pull away from just one tooth for several reasons. Aggressive brushing on one side of the mouth is a common contributor, as is a tooth that sits slightly out of alignment. When a tooth receives excessive biting force (from a high spot on a filling, a misaligned bite, or clenching), the bone and gum tissue around it can break down. That leaves the root surface exposed, and root surfaces have no enamel at all, making them especially reactive to temperature and touch.

Decay or a Failing Filling

Bacteria in plaque produce acid that can eat through enamel in a concentrated spot, opening a direct line to the dentin and its nerve endings. You may feel sensitivity to temperature and pain when biting down. Old fillings present a similar risk: they don’t last forever, and when one loosens or cracks, bacteria can seep underneath and cause new decay. The sensitivity in this case often feels like it came on suddenly or got worse over a short period.

A Cracked or Fractured Tooth

A crack can be invisible to the naked eye but still deep enough to flex when you chew, irritating the nerve with each bite. The hallmark symptom is a sharp pain when you bite down that vanishes the moment you release. Cracks are more common in teeth that have large fillings, teeth you’ve ground or clenched over time, or teeth that took an impact during sports or eating something unexpectedly hard.

Recent Dental Work

If your sensitivity started shortly after a filling, crown preparation, or whitening treatment, the cause is likely temporary inflammation of the nerve inside the tooth. Removing deep decay or reshaping a tooth irritates the pulp tissue, and it needs time to calm down. This type of sensitivity typically fades within a few days to two weeks, though it can occasionally linger for several months. If it hasn’t improved after about four weeks, or if it’s getting worse rather than better, that warrants a follow-up visit.

Grinding or Clenching

Nighttime grinding (bruxism) puts enormous repeated force on your teeth. One tooth often bears the brunt, especially if your bite is slightly uneven. Over time, this can wear through enamel on the biting surface, cause microcracks, or irritate the nerve directly through sheer mechanical stress.

What the Type of Pain Tells You

The character of your sensitivity carries useful diagnostic information. Cold sensitivity that lasts only a second or two after you remove the stimulus, like taking a sip of ice water, generally means the nerve is irritated but healthy. This is called reversible inflammation, and it usually resolves with a simple filling or a desensitizing treatment.

Heat sensitivity that lingers for minutes after the source is gone is more concerning. It suggests the tissue inside the tooth is becoming severely inflamed or is starting to die. Spontaneous pain, meaning pain that shows up without any trigger at all, points in the same direction. At this stage, a filling alone won’t fix the problem because the nerve damage has gone too far.

Pain that responds mainly to biting pressure, especially if the tooth no longer reacts to hot or cold at all, can indicate that the nerve has already died and an infection is developing at the root tip. Fever, persistent throbbing, or facial swelling alongside tooth pain are signs of an abscess, which needs prompt treatment.

Sinus Pain That Mimics a Toothache

Your upper back teeth sit directly below the maxillary sinuses. When a sinus infection causes swelling and pressure in that space, it can press on the roots of those teeth and create pain that feels exactly like a dental problem. The clue is that sinus-related tooth pain usually affects several upper teeth at once, feels worse when you bend forward, and comes alongside nasal congestion or a recent cold. If your dentist examines the tooth and finds nothing wrong, a sinus issue is worth investigating.

What You Can Do at Home

Desensitizing toothpaste containing 5% potassium nitrate is the most accessible first step. The potassium ions penetrate the open dentin tubes and reduce the excitability of the nerve fibers inside. In clinical testing, this concentration cut sensitivity scores by about 60% over the course of regular use, compared to 20% for a placebo. You’ll get the best results by brushing with it twice daily and, for extra effect, rubbing a small amount directly onto the sensitive tooth and leaving it in place for a minute before rinsing.

Switching to a soft-bristled toothbrush and brushing with gentle, circular strokes protects against further enamel loss and gum recession. If you suspect grinding, a simple over-the-counter night guard can reduce the mechanical stress on the tooth while you arrange a dental visit. Limiting highly acidic foods and drinks, or at least rinsing with plain water after consuming them, slows enamel erosion.

Professional Treatment Options

When home care isn’t enough, your dentist has several ways to seal off exposed dentin. Fluoride varnish, a concentrated gel painted onto the tooth surface, blocks the open tubes and reduces sensitivity. A dental bonding agent, a thin resin coating cured with a light, provides even stronger results. In a head-to-head comparison of 66 adults with hypersensitive teeth, the bonding agent group saw their pain scores drop from about 7 out of 10 to under 2 within four weeks. Nearly two-thirds of those patients reported being “very satisfied,” compared to about a third in the fluoride varnish group.

If the sensitivity comes from decay, a filling removes the damaged tooth structure and seals the dentin. If gum recession has exposed a significant stretch of root, a gum graft can cover it back up. For a cracked tooth, treatment depends on how deep the crack runs: a crown can stabilize a surface crack, while a crack that reaches the nerve may need root canal therapy. And in cases where the nerve is already dying or infected, a root canal removes the compromised tissue and eliminates the pain at its source.

How to Tell if It’s Serious

Brief, mild zings when you eat something cold are common and often manageable with desensitizing toothpaste alone. But certain patterns signal that the problem needs professional attention sooner rather than later. The American Association of Endodontists recommends seeing a dentist if sensitivity lingers more than 30 seconds after contact with a hot or cold stimulus, if pain is severe or spontaneous, or if it’s getting progressively worse. Swelling, fever, or a bad taste in your mouth alongside tooth pain suggests infection and shouldn’t wait.

Even mild sensitivity that sticks around for more than a few weeks is worth getting checked. Early-stage problems like small cavities, minor cracks, or beginning gum recession are far simpler to treat before they progress to nerve damage.
