Sleeping on your left side positions your stomach below your esophagus, helps waste move through your intestines with gravity, and improves blood flow during pregnancy. It’s one of the simplest changes you can make to support digestion, reduce snoring, and help your body clear waste more efficiently overnight.
Reduced Acid Reflux
Your stomach sits slightly to the left of your midline. When you lie on your left side, the stomach falls below the junction where it connects to the esophagus. This means stomach acid has to work against gravity to travel back up into your throat. Roll to your right side and the geometry flips: your stomach ends up above that junction, making it easier for acid to splash upward.
This isn’t just theoretical. Researchers at Amsterdam UMC confirmed that left-side sleeping reduces acid reflux episodes precisely because of this gravitational advantage. If you deal with heartburn at night or have been diagnosed with gastroesophageal reflux, switching to your left side is often one of the first lifestyle changes recommended.
Better Waste Movement Through Your Gut
Your digestive tract has a built-in route for waste. The small intestine empties into the large intestine through a valve called the ileocecal valve, located in your lower right abdomen. From there, waste travels up the right side of your abdomen (the ascending colon), across the top (the transverse colon), and down the left side (the descending colon) before reaching the rectum.
When you sleep on your left side, gravity helps push waste along this natural path, from the ascending colon through the transverse colon and into the descending colon. The result is that waste accumulates where it needs to be by morning, encouraging a more regular bathroom trip when you wake up. People who deal with sluggish digestion or constipation sometimes notice a difference just from this positional change.
Improved Blood Flow During Pregnancy
For pregnant women, left-side sleeping becomes especially important in the second and third trimesters. A large vein called the inferior vena cava runs along the right side of your spine, carrying blood from your lower body back to your heart. As the uterus grows, lying on your back presses its weight directly onto this vein, reducing blood flow to both you and the baby.
Sleeping on your left side takes pressure off the inferior vena cava entirely, allowing maximum blood flow to the placenta. It also improves kidney function, which helps reduce swelling in the feet and ankles. While sleeping on either side is better than sleeping on your back during pregnancy, the left side provides the most circulatory benefit.
Lymphatic Drainage and the Spleen
Your lymphatic system, the network that filters waste products, excess fluid, and cellular debris from your tissues, is predominantly left-sided. The thoracic duct, which is the largest lymphatic vessel in your body, drains into the left side of the heart. The spleen, which acts as a giant filter for both lymph fluid and blood, also sits on the left.
Unlike your circulatory system, the lymphatic system doesn’t have a pump. It relies on muscle contractions and body movement to push fluid along. When you’re asleep and mostly still, gravity becomes one of the few forces helping lymph fluid reach the thoracic duct and the spleen. Lying on your left side lets gravity assist that drainage rather than working against it. This may help your body process metabolic waste and filter blood more effectively during the hours you’re least active.
Brain Waste Clearance During Sleep
Your brain has its own waste-removal system that becomes most active during sleep. This system flushes cerebrospinal fluid through brain tissue to clear out metabolic byproducts, including proteins associated with neurodegenerative diseases. A study published in the Journal of Neuroscience found that this flushing process works most efficiently when sleeping on your side compared to sleeping on your back or stomach.
The researchers used imaging on anesthetized rodents and found that side sleeping produced less waste retention and better clearance than other positions. They proposed that the widespread human preference for side sleeping may have evolved specifically to optimize this overnight brain-cleaning process. The study tested the right lateral position rather than the left, so the advantage here appears to be about sleeping on your side in general rather than one side over the other.
Reduced Snoring and Sleep Apnea
Snoring and obstructive sleep apnea get worse when you sleep on your back. In that position, gravity pulls the tongue and soft tissues of the throat backward, partially blocking the airway. Switching to either side opens the airway back up.
The numbers are significant. A meta-analysis of positional therapy studies found that moving from back sleeping to side sleeping reduced apnea events by 54% on average and improved the lowest blood oxygen levels by about 3.3%. For snoring specifically, one study found that total snoring time dropped from roughly 37% of the night to about 16% when people were kept off their backs. That’s a reduction of more than 60% in snoring. If your partner has mentioned that you snore or you wake up feeling unrested, side sleeping is worth trying before more involved interventions.
When Left-Side Sleeping Isn’t Ideal
Left-side sleeping isn’t the best choice for everyone. People with heart failure often feel worse on their left side because the position shifts the heart slightly, increasing the sensation of pressure in the chest. Many heart failure patients experience shortness of breath that worsens on the left, according to the American Heart Association, and naturally prefer sleeping on their right side instead.
People with shoulder injuries or chronic pain on the left side may also find the position uncomfortable or counterproductive. If lying on your left side causes numbness, tingling, or worsening pain, the digestive and circulatory benefits aren’t worth the trade-off. Sleeping on your right side still provides many of the same advantages over back sleeping, particularly for snoring, brain waste clearance, and general spinal alignment.
How to Make Left-Side Sleeping Comfortable
The most common complaint about side sleeping is hip and lower back strain. When your upper leg collapses onto your lower leg, your spine twists, and you wake up sore. Placing a pillow or rolled-up towel between your knees keeps your hips stacked and your spine in a neutral position. This single adjustment makes a noticeable difference for most people.
Your head pillow matters too. It should be thick enough to fill the gap between your ear and the mattress so your neck stays straight rather than bending toward the bed. Too flat and your neck kinks downward; too thick and it pushes your head up at an angle. If you’re pregnant, a full-body pillow or an extra pillow tucked under your belly or behind your lower back can take pressure off the ligaments that are already working overtime. Positioning your arms in front of you rather than under your head helps prevent shoulder compression and the pins-and-needles feeling that wakes you up at 3 a.m.
If you tend to roll onto your back during the night, try placing a firm pillow behind you. Some people tuck a tennis ball into the back of a sleep shirt as a low-tech reminder to stay on their side. It feels silly, but it works while your body builds the habit.
