A typical bruise heals completely in about two weeks. If yours has lingered beyond that, something is slowing your body’s ability to break down the pooled blood beneath your skin. The cause could be as simple as a medication you’re taking, or it could point to something worth investigating with a doctor.
How a Normal Bruise Heals
When you bump into something hard enough to break tiny blood vessels under the skin, blood leaks into the surrounding tissue. Your body then sends cleanup cells to break down that trapped blood, which is why a bruise changes color over time. It typically starts dark blue or purple within the first few hours, shifts to green and yellow as the blood pigments are broken down, and fades completely within about 14 days.
If your bruise is still visible after two weeks, or if the color seems stuck at a dark purple stage without progressing through the usual green-yellow shift, your body’s cleanup process is being disrupted.
Medications That Slow Healing
This is one of the most common reasons a bruise sticks around. Several widely used medications reduce your blood’s ability to clot, which means more blood pools under the skin initially and the bruise takes longer to clear. The main culprits include:
- Blood thinners prescribed for heart conditions or stroke prevention
- NSAIDs like aspirin, ibuprofen (Advil, Motrin), and naproxen (Aleve)
- Steroids like prednisone, which thin the skin over time and weaken blood vessel walls
- Chemotherapy drugs, which can lower platelet counts
Taking a blood thinner alongside an over-the-counter pain reliever like ibuprofen compounds the effect. If you’re on any combination of these and noticing bruises that linger, the medication is the likely explanation. Don’t stop taking prescribed medications on your own, but it’s worth raising the issue at your next appointment.
Age and Sun Damage
If you’re over 60 and bruises keep appearing on your forearms and the backs of your hands, often without any injury you can remember, you’re likely dealing with a condition called senile purpura. It happens because years of sun exposure and natural aging break down the connective tissue in your skin, making blood vessels fragile and easy to rupture.
The skin in affected areas looks visibly thin. Bruises show up as dark purple patches and, once they fade, often leave behind a brownish stain from iron deposits in the tissue. That discoloration can take weeks to months to clear, and in some cases it becomes permanent. This is a cosmetic issue rather than a dangerous one, but it explains why bruises in older adults can seem to never fully disappear.
Nutritional Gaps
Two vitamins play direct roles in how your body handles bruising. Vitamin K is essential for forming blood clots. Without enough of it, even minor bumps produce larger bruises that take longer to resolve. Vitamin K deficiency is uncommon in adults who eat a varied diet (leafy greens are a rich source), but it can develop in people with digestive conditions that impair nutrient absorption.
Vitamin C is needed to build collagen, the protein that gives your blood vessel walls their strength. A shortage weakens those walls, making you bruise more easily and heal more slowly. Smokers, people who eat very few fruits and vegetables, and those with certain chronic illnesses are most at risk for low vitamin C levels.
When a Bruise Is Actually a Hematoma
A standard bruise is flat. If your bruise feels firm, raised, or swollen, and the area is warm to the touch, you may have a hematoma, which is a larger, more organized collection of blood trapped under the skin. Hematomas form after more forceful injuries and can take significantly longer to resolve because your body has a bigger volume of blood to reabsorb. A large hematoma sometimes needs to be drained by a doctor rather than left to heal on its own, especially if it’s painful or continues to grow.
Platelet and Blood Disorders
Your blood contains tiny cells called platelets that clump together to stop bleeding when a vessel breaks. A normal platelet count ranges from 150,000 to 450,000 per microliter of blood. When that count drops, a condition called thrombocytopenia, you bruise more easily and those bruises heal slowly because your body can’t form clots efficiently.
Platelet counts can drop for many reasons: autoimmune conditions where the immune system destroys platelets, certain medications, viral infections, or bone marrow problems. Liver disease also contributes to prolonged bruising because the liver produces many of the proteins involved in clotting. Leukemia and other blood cancers can crowd out normal platelet production in the bone marrow.
These conditions almost always come with other symptoms beyond a single stubborn bruise. Frequent nosebleeds, bleeding gums, blood in your urine or stool, or bruises appearing in unusual locations without any known injury are patterns that point toward a systemic problem rather than a one-off slow-healing bruise.
Helping a Bruise Heal Faster
For a fresh bruise, the basics still work: ice the area for 15 to 20 minutes several times during the first day to limit the initial blood pooling, then switch to gentle warmth after 48 hours to help increase blood flow and speed reabsorption. Elevating the bruised area above heart level, when practical, also helps.
Topical treatments like arnica gel and vitamin K creams are widely used, and some clinical research has tested their ability to speed resolution. A controlled trial compared 20% arnica, 5% vitamin K, a vitamin K and retinol combination, and plain petroleum jelly applied twice daily for two weeks. Results on their comparative effectiveness have been mixed, so these products may offer a modest benefit but aren’t a guaranteed fix.
Signs That Deserve Medical Attention
A single bruise from a known injury that takes three weeks instead of two to fade is rarely a sign of anything serious. But certain patterns warrant a call to your doctor:
- Bruising that lasts more than two weeks without any color change
- Frequent large bruises that appear without clear cause
- A firm lump forming inside the bruised area
- Painful swelling that worsens rather than improves
- A bruise that keeps recurring in the same spot
- Unusual bleeding elsewhere, such as nosebleeds, bloody urine, or blood in bowel movements
- A black eye accompanied by vision changes
A simple blood test can check your platelet count, clotting function, and vitamin levels, giving your doctor a clear picture of whether something beyond normal variation is at play.
