Why Your Cough Gets Worse at Night: Causes and Tips

Coughing gets worse at night because of a combination of gravity, hormones, and your body’s internal clock all working against you at once. Lying flat allows mucus to pool in the back of your throat, your airways naturally narrow in the overnight hours, and your body’s built-in anti-inflammatory defenses drop to their lowest point while you sleep. Any one of these factors can trigger more coughing, but at night they tend to stack on top of each other.

Gravity Stops Working in Your Favor

During the day, gravity pulls mucus down through your nasal passages and throat, where you swallow it without thinking. The moment you lie flat, that drainage has nowhere to go. Instead of flowing downward, mucus collects at the back of your throat and triggers your cough reflex. This postnasal drip effect happens with colds, allergies, and sinus infections alike, and it’s often the single biggest reason a manageable daytime cough becomes relentless at bedtime.

Lying flat also affects the airways themselves. In some people, the larger airways partially collapse in the supine position, creating irritation and a persistent cough that only appears when they’re on their back. Research published in the journal Chest documented patients whose nighttime cough was entirely caused by this positional airway collapse, and it resolved when the airways were kept open with positive airway pressure.

Your Body’s Internal Clock Shrinks Your Airways

Your lungs follow a circadian rhythm that has nothing to do with whether you’re lying down or standing up. Lung function peaks around 4:00 p.m. and hits its lowest point around 4:00 a.m. In a healthy person, this variation is modest. But if you have asthma or any kind of airway sensitivity, the overnight dip can be dramatic, with lung function dropping more than 15% from its daytime peak. That narrowing makes your airways more reactive to irritants, cold air, or mucus that wouldn’t bother you during the day.

This circadian pattern also influences the inflammatory cells and chemical signals in your lungs. Inflammatory activity naturally rises at night, which is part of why asthma attacks are far more common in the early morning hours than during the afternoon.

Cortisol Drops and Inflammation Rises

Cortisol, the hormone your body uses to keep inflammation in check, follows its own daily cycle. Levels climb in the early morning and stay relatively high through the day, helping suppress swelling in your airways and sinuses. By evening, cortisol levels fall significantly. As Harvard Health explains, this nighttime drop allows symptoms that were held in check during the day to flare again. If you’ve noticed that a cold feels tolerable during the afternoon but miserable by 10 p.m., this hormonal shift is a major reason why.

Acid Reflux Gets Worse Lying Down

Stomach acid doesn’t have to cause heartburn to make you cough. When acid travels up the esophagus and reaches the throat, it irritates the tissue and triggers a reflexive cough. Many people with reflux-related cough have no burning sensation at all, just a persistent, dry, tickling cough that worsens after meals or at bedtime.

Lying flat removes the gravitational barrier that normally helps keep stomach acid where it belongs. This is why reflux symptoms of all kinds, including cough, tend to spike at night. If your cough consistently worsens within an hour or two of going to bed, especially after a late dinner, reflux is worth considering as a contributing factor.

Dry or Humid Air in Your Bedroom

The air in your bedroom can either soothe or irritate your airways while you sleep. Dry air, common in winter when heating systems run constantly, pulls moisture from the lining of your throat and airways. This makes them more sensitive and more likely to trigger a cough. On the other hand, air that’s too humid encourages dust mites and mold growth, both of which are common allergens that can worsen nighttime coughing.

The Environmental Protection Agency recommends keeping indoor humidity between 30% and 50%, with 60% as the absolute upper limit. If you’re unsure where your bedroom falls, a simple hygrometer (available for a few dollars) can tell you. In dry climates or heated rooms, a cool-mist humidifier can help keep your airways from drying out overnight.

Blood Pressure Medications Can Play a Role

A specific class of blood pressure drugs called ACE inhibitors is well known for causing a dry, persistent cough. Research published by the American Heart Association tracked cough severity in people taking these medications and found that the cough was equally bothersome during the day and at night. But because all the other nighttime factors (gravity, low cortisol, narrow airways) are stacking on top of the medication side effect, the nighttime experience often feels worse. If you started a new blood pressure medication and developed a cough that won’t go away, that connection is worth raising with your prescriber.

When It Happens to Children

Kids are especially prone to nighttime coughing, partly because their airways are smaller and more easily affected by swelling. Croup, a viral infection that causes the upper airway to swell, is a classic example. It produces a distinctive barking cough that is consistently worse at night. Symptoms typically last three to five days, and the cough tends to peak after midnight. Crying and distress make the cough worse, which creates a frustrating cycle: the cough frightens the child, the distress tightens the airway further, and the coughing intensifies.

Cool night air can actually help with croup. Some parents find that sitting outside in cool air for a few minutes, or running a cool-mist humidifier in the child’s room, eases the barking cough temporarily.

A Cough That Deserves Closer Attention

Most nighttime coughs trace back to colds, allergies, or the postnasal drip and reflux factors described above. But a cough that wakes you up and comes with breathlessness, frothy or pink-tinged mucus, or swelling in your legs and ankles can signal something more serious. Heart failure causes fluid to accumulate in the lungs, a condition called pulmonary edema, and the cough your body produces to clear that fluid is often worse at night when lying flat redistributes fluid toward the chest. This type of cough typically improves when you sit upright or prop yourself up on pillows.

Practical Ways to Reduce Nighttime Coughing

The single most effective change, according to Cleveland Clinic, is elevating your head. Adding an extra pillow or raising the head of your bed helps prevent mucus from pooling in your throat and makes it harder for stomach acid to travel upward. You don’t need a dramatic incline: just enough to create a gentle slope. Too much elevation can cause neck pain, so find a comfortable angle rather than stacking pillows aggressively.

If your cough is dry rather than productive, sleeping on your side instead of your back can reduce irritation. Lying flat on your back is the worst position for nearly every type of nighttime cough. Beyond positioning, keeping your bedroom humidity in the 30% to 50% range, avoiding large meals close to bedtime, and running a humidifier during dry months all target the specific mechanisms that make nighttime coughing worse. A warm shower before bed can also loosen mucus in your sinuses and give you a head start before lying down.
