Why Your Feet and Ankles Swell: Causes and Warning Signs

Swollen feet and ankles happen when fluid builds up in the tissues of your lower legs, a condition called peripheral edema. Gravity pulls blood and fluid downward throughout the day, and your body relies on vein valves, muscle movement, and organ function to push that fluid back up. When any part of that system falters, fluid pools in your feet and ankles. The causes range from something as simple as sitting too long to serious conditions involving the heart, kidneys, or liver.

How Fluid Ends Up Trapped in Your Lower Legs

Your veins contain a series of one-way valves that open to let blood flow toward your heart and close to prevent it from falling back toward your feet. Muscles in your calves act like a pump, squeezing veins with each step to push blood upward. At the same time, proteins in your blood (especially one called albumin) work like tiny sponges that hold fluid inside your blood vessels rather than letting it seep into surrounding tissue.

Swelling happens through a few basic mechanisms. Pressure inside your veins can rise too high, forcing fluid out into the tissues. Your blood can lose too much protein, weakening its ability to hold onto fluid. Your lymphatic system, which normally drains excess fluid from tissues, can become overwhelmed or damaged. Or your body can retain too much salt and water, increasing fluid volume overall. Most causes of swollen feet and ankles trace back to one or more of these pathways.

Everyday Causes That Aren’t Dangerous

Prolonged sitting or standing is the most common reason for puffy ankles at the end of the day. Without regular calf muscle contractions, blood pools in the lower legs and pressure builds. Long flights, desk jobs, and car rides are typical triggers. Heat makes it worse because warm temperatures cause blood vessels to widen, allowing more fluid to escape into tissues.

Eating a high-sodium meal can cause your body to retain extra water, and that fluid tends to settle in your feet. Tight shoes or socks that leave indentations can slow circulation enough to contribute. For many people, the swelling is mild, symmetrical (both legs equally), and resolves overnight when the legs are elevated.

Chronic Venous Insufficiency

When the valves inside your leg veins become damaged or weakened, blood flows backward instead of toward the heart. This is called venous reflux, and it’s one of the most common causes of persistent ankle swelling. The backward flow increases pressure in the veins, which forces fluid into the surrounding tissue. Over time, the high pressure can stretch nearby veins and damage their valves too, creating a cascade of worsening circulation.

The most frequent cause of deep vein valve damage is a previous blood clot. Even after a clot dissolves, the valves it damaged may never fully recover. The veins refill quickly with blood that should be moving upward, and pressure stays elevated between steps rather than dropping the way it should. Varicose veins are a visible sign that this process is underway in the superficial veins closer to the skin’s surface. People with chronic venous insufficiency typically notice that swelling worsens throughout the day and improves after sleeping with their legs elevated.

Heart Failure

When the heart can’t pump blood forcefully enough, blood backs up in the veins. That backup raises venous pressure, which forces fluid out of blood vessels and into tissues. Swollen ankles are often one of the earliest noticeable signs of congestive heart failure, particularly right-sided heart failure where blood returning from the body gets congested before reaching the lungs.

The swelling from heart failure is typically symmetrical and worsens over the course of the day. You might also notice shortness of breath (especially when lying flat), fatigue, and rapid weight gain from fluid retention. If pressing a finger into your swollen ankle leaves a visible dent that takes several seconds to fill back in, that’s called pitting edema, and deeper, slower-recovering pits suggest more significant fluid overload. Clinicians grade this on a 1 to 4 scale: a grade 1 pit is about 2 millimeters deep and rebounds immediately, while a grade 4 pit reaches 8 millimeters and can take two to three minutes to fill back in.

Kidney Problems

Your kidneys regulate how much sodium and water your body holds onto. When they aren’t filtering properly, excess fluid accumulates. One specific pattern, called nephrotic syndrome, occurs when damaged filtering units in the kidneys leak large amounts of protein into the urine. That protein loss lowers albumin levels in the blood, which weakens the blood’s ability to hold fluid inside vessels. The result is widespread swelling, often noticeable in the feet, ankles, and around the eyes.

People with nephrotic syndrome may also experience unexplained weight gain from fluid retention and foamy-looking urine (a sign of excess protein). Reducing salt intake helps limit fluid buildup, and water pills are commonly used to help the kidneys flush out excess fluid.

Liver Disease

The liver produces albumin, so advanced liver disease (particularly cirrhosis) leads to low albumin levels and reduced ability to keep fluid inside blood vessels. But that’s only part of the problem. Cirrhosis also causes blood to pool in the vessels around the digestive organs, which tricks the body into thinking blood volume is low. The body responds by activating hormonal systems that retain sodium and water, making swelling even worse. This is why people with cirrhosis often develop fluid buildup in both the abdomen and the lower legs.

Medications That Cause Swelling

Several common drug classes can cause your feet and ankles to swell as a side effect. Calcium channel blockers, a widely prescribed type of blood pressure medication, are among the most frequent culprits. They relax blood vessel walls, which can allow more fluid to leak into tissues. Anti-inflammatory painkillers (like ibuprofen and naproxen) cause the body to retain sodium and water. Corticosteroids, hormone therapies including estrogen and testosterone, and some diabetes medications can also contribute.

If you notice new swelling after starting a medication, it’s worth bringing up with your prescriber. For calcium channel blockers specifically, adding a second type of blood pressure medication can sometimes counteract the swelling without needing to stop the original drug.

Pregnancy-Related Swelling

Some degree of ankle swelling during pregnancy is normal, especially in the third trimester. The growing uterus compresses pelvic veins, slowing blood return from the legs, and hormonal changes cause the body to retain more fluid. Mild, gradual swelling in both feet is expected.

What’s not normal is sudden swelling in the hands and face, which can be a sign of preeclampsia, a serious pregnancy complication involving high blood pressure and organ stress. Preeclampsia requires prompt medical attention because it can progress rapidly and affect both the mother and baby.

When Swelling Signals an Emergency

Swelling in only one leg is a red flag. A deep vein thrombosis (a blood clot in a deep leg vein) typically causes swelling, pain or cramping (often starting in the calf), warmth, and skin color changes such as redness or a purplish tint. This is urgent because the clot can break free and travel to the lungs, causing a pulmonary embolism. If one leg suddenly swells and feels warm or painful, seek immediate care.

Other warning signs that swelling needs prompt evaluation include rapid onset, swelling that keeps getting worse over days, shortness of breath, chest pain, or reduced urine output.

Managing Mild Swelling at Home

For everyday, non-dangerous swelling, a few strategies make a real difference. Elevating your feet above heart level for 15 to 30 minutes helps gravity drain fluid back toward your core. Moving regularly, even short walks or calf raises at your desk, activates the muscle pump that pushes blood upward. Cutting back on sodium reduces the amount of water your body retains.

Compression socks apply graduated pressure that supports your veins and prevents fluid from settling. For mild puffiness after sitting or standing, over-the-counter socks rated at 15 to 20 mmHg are effective for most people. Moderate swelling typically responds to 20 to 30 mmHg with medical guidance. Severe conditions like lymphedema or chronic venous insufficiency often require prescription-grade compression at 30 to 40 mmHg as part of a broader treatment plan. Put compression socks on first thing in the morning before swelling starts for the best results.
