Why Your Leg Muscles Hurt and When to See a Doctor

Leg muscle pain most often comes from something straightforward: overuse, dehydration, or holding the same position too long. But the cause can range from a tough workout to poor circulation or nerve irritation, and knowing what your pain feels like and when it shows up is the key to figuring out what’s going on.

Overuse and Delayed Soreness

The most common reason your leg muscles hurt is that you’ve asked more of them than they’re used to. A longer walk, a new exercise routine, yard work, or even an unusually active day can cause micro-damage to muscle fibers that your body then repairs and strengthens. This process is called delayed onset muscle soreness, or DOMS, and it typically starts one to three days after the activity, not during it. That lag catches people off guard because the pain seems to come out of nowhere.

DOMS usually resolves within five days. The soreness peaks around day two and then gradually fades. If your pain sticks around for a week or more, that’s a sign you may have an actual muscle strain rather than normal post-exercise soreness. A strain feels like a sharper, more localized pain, and you might notice tenderness when pressing on one specific spot. Stretching a strained muscle tends to reproduce the pain rather than relieve it.

Dehydration and Low Electrolytes

Your muscles depend on a balance of minerals in your blood to contract and relax properly. When levels of potassium, magnesium, sodium, or calcium drop too low, muscles can cramp, spasm, or ache. This happens more easily than most people realize. You don’t need to be severely dehydrated. A hot day, a diuretic medication (including common blood pressure drugs), heavy sweating, or simply not drinking enough water can shift the balance enough to trigger leg pain.

Magnesium and potassium are the two minerals most directly tied to muscle function. Magnesium supports nerve signaling to muscles, while potassium helps regulate how muscles contract. If your leg muscles cramp up at night or during light activity, low electrolytes are one of the first things worth considering. Eating potassium-rich foods like bananas, potatoes, and leafy greens, along with staying well hydrated, often makes a noticeable difference within days.

Sitting Too Long or Standing All Day

Holding any muscle in the same position for extended periods causes fatigue and stiffness. If you sit at a desk for hours, the muscles in your calves and thighs stay shortened or compressed, reducing blood flow. Standing in one spot all day creates the opposite problem: sustained contraction without rest. Both patterns lead to that heavy, achy feeling in the legs by the end of the day. Frequent position changes, even just standing up for a minute every half hour, help keep blood circulating and muscles from tightening up.

Pain That Travels: Nerve Irritation

Not all leg pain actually starts in the leg. Sciatica, caused by pressure on the sciatic nerve in your lower back, sends pain radiating down through the hip, thigh, and sometimes all the way to the foot. It feels distinctly different from a muscle problem. Instead of a sore, tight sensation in one spot, nerve pain tends to burn, tingle, or feel electric. It follows a path down the leg rather than staying in one muscle group.

The other telling difference is that sciatica doesn’t always improve with rest the way a pulled muscle does. Sitting can actually make it worse, since it increases pressure on the lower spine. If your leg pain started around the same time as lower back discomfort, or if it changes with your posture rather than your activity level, the source is likely your spine rather than the muscles themselves.

Circulation Problems

When arteries in your legs narrow from a buildup of fatty deposits, your muscles don’t get enough blood during activity. This causes a specific pattern called intermittent claudication: cramping or aching in the calves or thighs that starts when you walk and stops when you rest. Early on, the pain only appears during exertion. As the condition progresses, it can show up at rest too.

This type of pain is more common in people over 50, smokers, and those with diabetes or high blood pressure. The key distinction is the predictable pattern. Your legs feel fine when you’re sitting, but a certain amount of walking consistently triggers the cramping. That exercise-triggered, rest-relieved cycle is the hallmark of a circulation issue rather than a muscle or nerve problem.

Medication Side Effects

Several common medications cause leg muscle pain as a side effect. Statins, prescribed for high cholesterol, are one of the most well-known culprits. Diuretics can trigger cramps by depleting electrolytes. If your leg pain started shortly after beginning a new medication or changing a dose, that connection is worth bringing up with whoever prescribed it. The timing alone is often the biggest clue.

Warning Signs That Need Prompt Attention

Most leg muscle pain is harmless, but a few patterns signal something more serious. A blood clot in a deep leg vein, known as DVT, causes pain or cramping that often starts in the calf along with swelling, warmth, and skin that looks red or purple. It can also occur without obvious symptoms. DVT risk goes up after long periods of immobility, like a long flight or car ride, or after surgery.

Get emergency care if you:

  • Can’t walk or bear weight on your leg
  • Notice sudden swelling, redness, and warmth in one lower leg
  • Heard a popping or grinding sound when the pain started
  • Have swelling in both legs along with trouble breathing

See a doctor soon if you have a fever with leg tenderness, one leg that looks pale or feels unusually cool, or calf pain that developed after prolonged sitting.

Managing Muscle Pain at Home

For a standard muscle strain or soreness from overuse, the current sports medicine approach has shifted away from the old advice of icing and resting for days. Updated guidelines published in the British Journal of Sports Medicine recommend protecting the muscle for one to three days by limiting the movement that caused the pain, compressing the area with a bandage if there’s swelling, and elevating the leg above heart level when possible.

What’s changed is the thinking around anti-inflammatory painkillers and ice. Inflammation is actually part of the healing process, and suppressing it aggressively, especially with high doses of anti-inflammatory drugs, may slow tissue repair. Ice can reduce pain in the moment, but there’s no strong evidence it speeds recovery, and it may interfere with the blood flow your muscles need to heal.

After the first few days, the priority shifts to gradual movement. Adding light activity as soon as pain allows, rather than waiting until you’re completely pain-free, promotes stronger tissue repair and faster recovery. Walking, gentle stretching, and progressively returning to normal activity help muscles rebuild tolerance. The pain itself is your guide: if an activity makes the soreness noticeably worse, dial it back, but don’t avoid movement entirely.
